Musculoskeletal Issues in Children and Adolescents: Common Childhood Musculoskeletal Injuries.

Active children and adolescents have unique risk factors for musculoskeletal injuries compared with adults. Physes and developing bones are at higher risk of injury than tendons and ligaments. Children's bone remodeling is robust, allowing most clavicle fractures and torus fractures of the forearm to be managed conservatively. Radial head subluxation is managed with reduction. Apophyseal injuries are traction or overuse injuries that typically can be managed nonoperatively. Osteochondritis dissecans and other osteochondroses require frequent monitoring and occasionally surgical intervention.

[Homeostasis and Disorder of Musculoskeletal System.BMP and TGF signaling and locomotive tissues.].

BMP(bone morphogenetic protein)and TGF-β(transforming growth factor-β)form a family and play an important role from formation of bone, cartilage and skeletal muscle to their homeostasis. BMP and TGF-β transduce their signals through the cell surface type Ⅰ and type Ⅱ serine/threonine kinase receptors and the intracellular Smad proteins. Several factors regulate BMP and TGF-β signals including activation mechanisms and antagonist outside the cell, co-receptor in the cell membrane, and dephosphorylation by phosphatase and modification/degradation by the ubiquitin-proteasome system in the cell. Bone marrow aspirate clot: A technical complication or a smart approach for musculoskeletal tissue regeneration?

One of the methods employed to improve healing of damaged tissues is the use of cellular based therapies. A number of regenerative medicine based strategies, from in vitro expanded mesenchymal stem cells (MSCs) to "one-step" procedures using bone marrow (BM) in toto (BM aspirate; BMA) or BM concentrate (BMC), have been developed. Recently, orthopedic researchers focused their attention on the clinical therapeutic potential of BMC and BMA for musculoskeletal regeneration. BMA is reported as an excellent source of cells and growth factors. However, the quality of BM harvest and aspirate is extre

Track and Field Throwing Sports: Injuries and Prevention.

Throwing events in track and field are among the oldest sporting events in recorded history and include shot put, discus, hammer, and javelin. All throwing athletes use the kinetic chain to transfer energy from the lower extremities, through the pelvis, trunk, shoulder, arm, wrist, hand, and finally into the implement to generate maximum force. Throughout this sequential activation, mobilization, and stabilization of body segments, there is opportunity for breakdown in the chain, which can lead to injury. This review describes the biomechanics and kinetic chain components of each event and hig

Lengthening our perspective: morphological, cellular, and molecular responses to eccentric exercise.

The response of skeletal muscle to unaccustomed eccentric exercise has been studied widely, yet it is incompletely understood. This review is intended to provide an up-to-date overview of our understanding of how skeletal muscle responds to eccentric actions, with particular emphasis on the underlying molecular and cellular mechanisms of damage and recovery. This review begins by addressing the question of whether eccentric actions result in physical damage to muscle fibers and/or connective tissue. We next review the symptomatic manifestations of eccentric exercise (i.e., indirect damage mark

Musculoskeletal examination--an ignored aspect. Why are we still failing the patients?

Over the past two decades, rheumatologists from around the world have not only championed the musculoskeletal system examination but also modified the undergraduate teaching curriculum. This has led to the development and adoption of the gait, arms, legs and spine (GALS) screening along with regional examination techniques. The purpose of this study is to review current practice, determining the frequency of patient exposure to appropriate examination and confidence of junior doctors when dealing with MSK conditions. Two district-general hospitals (non-teaching) and one teaching hospital in No…

Non-steroidal anti-inflammatory drugs in sports medicine: guidelines for practical but sensible use.

INTRODUCTION: Non-steroidal anti-inflammatory drugs (NSAID) are commonly used in sports medicine. NSAID have known anti-inflammatory, analgesic, antipyretic and antithrombotic effects, although their in-vivo effects in treating musculoskeletal injuries in humans remain largely unknown. NSAID analgesic action is not significantly greater than paracetamol for musculoskeletal injury but they have a higher risk profile, with side-effects including asthma exacerbation, gastrointestinal and renal side-effects, hypertension and other cardiovascular diseases. DISCUSSION: The authors recommend an appr

Musculoskeletal ultrasound including definitions for ultrasonographic pathology.

Ultrasound (US) has great potential as an outcome in rheumatoid arthritis trials for detecting bone erosions, synovitis, tendon disease, and enthesopathy. It has a number of distinct advantages over magnetic resonance imaging, including good patient tolerability and ability to scan multiple joints in a short period of time. However, there are scarce data regarding its validity, reproducibility, and responsiveness to change, making interpretation and comparison of studies difficult. In particular, there are limited data describing standardized scanning methodology and standardized definitions o
