Autistic persons in the perinatal context: A mapping review of midwifery care, experiences, needs, and evidence.

BACKGROUND: Autistic people represent a substantial and probably under-recognised population within reproductive and maternity services. Maternity care nevertheless continues to rely on neurotypical assumptions about communication, sensory tolerance, pain expression, emotional display, and help-seeking. AIM: To map the scope, distribution, and thematic content of literature on autistic people's experiences and support needs across pregnancy, childbirth, postpartum care, and infant feeding, and to identify implications for maternity service design and midwifery practice. METHODS: A mapping re

A survey of postoperative pain treatments and unmet needs.

More than 300 million patients undergo surgery worldwide each year. Pain associated with these procedures is associated with short- and long-term negative sequelae for patients, healthcare providers, and healthcare systems. The following chapter is a review of the reality of postoperative pain management in everyday clinical routine based on survey- and registry-derived data with a focus on care in adults. Between 30% and up to 80% of patients report moderate to severe pain in the days after surgery. Structures, processes, and outcomes vary widely between hospitals and indicate gaps between ev

We implemented a series of quality improvement interventions with the objectives to decrease patient wait times and to increase the variety of services provided. METHODS: A needs assessment was performed. Problems related to time management, communication between staff and providers, clinic resources, and methods for assessing clinic performance were identified as targets to reduce wait times a…

Health and safety issues relating to Maine's fishing industry.

The Maine Agricultural Safety and Health Program performed a needs assessment to determine the health and safety concerns of Maine's fishing community. Information for the assessment was obtained from clinicians, focus groups of wives of fishers, and government agencies. Reports from the United States Coast Guard showed that during 1993-1994, sixty-five people were injured on commercial fishing vessels and eight people died. Decompression illness was the most frequently reported non-fatal injury (n = 15). Chronic injuries reported by clinicians included back pain, tendinitis, carpal tunnel syn…

Feasibility and self-perceived effectiveness of an online training program on dysphagia in residential aged care homes in mainland China.

BACKGROUND: With a rapidly aging population in mainland China, dysphagia has become one of the common geriatric disorders which creates a huge demand on speech and language therapists (SLTs). The major challenge is the shortage of SLTs in China. In addition, frontline practitioners in mainland China may not be well equipped with the knowledge and practical skills in dysphagia management due to lack of systematic training and the work nature. AIMS: This study evaluates the self-perceived effectiveness and feasibility of an online training program that aims to enhance the self-assessed knowledg

Routine neonatal circumcision: opportunities for improving residency training.

OBJECTIVES: Primary care physicians are the routine providers of neonatal circumcision, yet urologists commonly manage the complications. We previously identified a need for improved formalized training in neonatal circumcisions among ob-gyn residents. Here we extend the needs assessment to urology residents. METHODS: From Nov 2008 to Nov 2009, ob-gyn and urology residents at our institution were given an online survey to assess comfort, education, and proficiency in pre-operative evaluation and performance of circumcisions. RESULTS: 26/35 (74%) ob-gyn and 12/17 (65%) urology residents respo…

The 22nd Kellersberger Memorial Lecture, 1997. Preventing loss of sight from leprosy.

Multidrug treatment of leprosy is being dramatically successful in sterilising the infection. However complications are still occurring, spoiling the result in some patients by residual damage, including to the eye. Prevention of this damage is imperative. It is stressed that eye complications will only lead to loss of sight if they are neglected. The lesions caused by leprosy in the eye are briefly described. The dangerous lesions are lagophthalmos causing corneal exposure, and iritis causing pupil block glaucoma. Frequency of the lesions were very common in the past, but seem to be becoming
