Correlation of MPV/Platelet Count Ratio With Blood Culture Positivity in Late-Onset Neonatal Sepsis: Implications for Nursing Practice.

BACKGROUND: Late-onset neonatal sepsis (LOS) is a major cause of morbidity and mortality. Pathogens damage endothelium and platelets, triggering the release of larger platelets. PURPOSE: To find an association between mean platelet volume (MPV) and the MPV/platelet count ratio (MPR) as a rapid, inexpensive screening tool for LOS. METHODS: Secondary analysis of a sepsis bundle study (Level IV neonatal intensive care unit, January-December 2023) included nonsyndromic newborns >3 days old undergoing sepsis evaluation. Demographics, MPV, platelet count, and culture results were extracted from th…

Polygenic, cell-envelope adaptations drive high-frequency daptomycin resistance in Staphylococcus capitis NRCS-A from neonatal sepsis and NEC.

Staphylococcus capitis NRCS-A is a major cause of neonatal sepsis worldwide and exhibits resistance to multiple antibiotics. We assessed the prevalence and mechanisms of daptomycin resistance (DAP-R) in bloodstream isolates from a German neonatal intensive care unit. Ten of 11 NRCS-A isolates (91%) were resistant to daptomycin, and 18% displayed decreased susceptibility to vancomycin. Genomic analysis revealed diverse polymorphisms in genes associated with DAP-R in Staphylococcus aureus, but no single amino acid variant explained resistance. Phospholipid composition remained unchanged, whereas…

Neonatal Hyperbilirubinemia: A Case of Complex Management Involving ABO Incompatibility, Sepsis, and Suspected G6PD Deficiency treated with Methyl Prednisolone.

Newborn jaundice (NNJ), especially due to ABO incompatibility, is a major global health concern. Phototherapy is standard treatment, with exchange transfusions reserved for severe cases. However, in some babies these therapies may be ineffective, requiring additional immunomodulatory treatments. Limited access to these treatments in developing countries creates a critical gap, worsening jaundice severity. A 22-hour old term neonate presented with rapidly progressive severe neonatal hyperbilirubinemia (NNJ) within 15 hours of life, consistent with ABO incompatibility based on discordant materna

Home-based management of neonatal sepsis: 23 years of sustained implementation and effectiveness in rural Gadchiroli, India, 1996-2019.

INTRODUCTION: Although hospitalisation remains the preferred management for neonatal sepsis, it is often not possible in resource-limited settings. The Home-Based Newborn Care (HBNC) study in Gadchiroli, India (1995-1998) was the first trial to demonstrate that neonatal sepsis can be managed in the community. HBNC continues to operate in Gadchiroli. In 2015, WHO recommended community-based management of neonatal sepsis when hospitalisation is not feasible but called for implementation research. We studied the implementation and effectiveness of home-based management of neonatal sepsis over 23 …

Development of an Immunochromatographic Strip Using Conjugated Gold Nanoparticles for the Rapid Detection of Klebsiella pneumoniae Causing Neonatal Sepsis.

Neonatal sepsis is a leading cause of death among newborns and infants, especially in the developing world. The problem is compounded by the delays in pinpointing the causative agent of the infection. This is reflected in increasing mortality associated with these cases and the spread of multi-drug-resistant bacteria. In this work, we deployed bioinformatics and proteomics analyses to determine a promising target that could be used for the identification of a major neonatal sepsis causative agent, Klebsiella pneumoniae. A 19 amino acid peptide from a hypothetical outer membrane was found to be…

The Relationship Among Intra-Amniotic Inflammatory Response, The Progression of Inflammation in Chorionic Plate and Early-Onset Neonatal Sepsis.

Background: The chorionic plate (CP) has been denigrated by the well-known route of the extraplacental membranes from the decidua parietalis through the chorion to the amnion in the progression of ascending intrauterine infection among preterm births (PTBs). However, considering previous studies reporting the relationship among intra-amniotic inflammatory response (IAIR), the progression of inflammation in extraplacental membranes and early-onset neonatal sepsis (EONS), and the anatomic connection between extraplacental membranes and CP, there is a good chance that IAIR would be more likely an…
