[IgA Nephropathy Update 2025/2026].

Immunoglobulin A nephropathy (IgAN) is the most common form of primary glomerulonephritis and has undergone a therapeutic revolution in just a few years that is unprecedented in nephrology. For the first time, the guidelines define remission as a realistic treatment goal and solidify the so-called "dual-pillar" approach. This educational article summarizes the current recommendations and developments and evaluates the clinical evidence.

A questionnaire survey of radiological diagnosis and management of renal dysplasia in children.

BACKGROUND: The condition called renal dysplasia is considered to be a frequent cause of chronic kidney disease in children. Formally, it is defined by histological parameters. In current nephrology practice, however, the appearance of the kidneys on ultrasound scanning is often used as a basis for the diagnosis. METHODS: The European Society for Pediatric Nephrology Working Group on Congenital Anomalies of the Kidney and Urinary Tract hypothesized that the current diagnostic approach with regard to renal dysplasia was not homogeneous. Accordingly, we here report the results of a survey targe…

Monoclonal gammopathy of renal significance (MGRS): the characteristics and significance of a new meta-entity.

Monoclonal gammopathy of renal significance (MGRS) is a new nosological group of entities (meta-entity) defined in 2012, whose pathogenesis depends on monoclonal immunoglobulins (Ig) secreted by low-grade lymphoproliferative disorders, which belong to M-protein-related diseases. Renal damage is the result of monoclonal Ig deposit or its activity as autoantibodies, which can compromise any nephronal area. MGRS does not include kidney diseases produced by high-grade lymphoproliferative disorders as well as those whose pathogenesis are independent of monoclonal Ig (such as drug toxicity or metabo

Can patients with schizophrenia undergo renal transplantation with success?

We report a case of a 41-year-old man suffering from paranoid schizophrenia. The patient has been consulting in our psychiatric hospital since he was 29 years old. Eight years later, he developed kidney failure and required peritoneal dialysis. After more than two years, the nephrology team indicated a renal transplantation and his brother suggested giving his kidney. There were no obstacles for transplantation in the immune and histological compatibilities; the psychiatric staff decided to check the patient's compliance with medication. The patient was compliant to all his medications and to …

Oxidized Albumin: An Emerging Biomarker Bridging Oxidative Stress and Renal Pathophysiology.

Oxidized albumin has emerged as a promising biomarker in blood for assessing oxidative stress and multiple organ dysfunction. Recent advances suggest its potential detectability in urine, offering a noninvasive avenue for early diagnosis and monitoring of kidney disease. This review synthesizes current knowledge on oxidized albumin, its pathophysiological relevance, and the evolving methodologies for its detection, highlighting its translational potential in clinical nephrology.

Mutations in Na(K)Cl transporters in Gitelman's and Bartter's syndromes.

The successful merging of modern molecular genetics with basic renal physiology is exemplified by the recent description of the molecular basis of two classic diseases of clinical nephrology; Bartter's and Gitelman's syndromes of inherited hypokalemic alkalosis. Mutations in four different genes have been identified, each of which causes hypokalemic alkalosis, salt wasting and hypotension. These genetic studies have greatly advanced our understanding of renal physiology.

Renography using the gamma camera and the intertechnique cinescintigraphy system. A review of the current applications.

The application and use of radio-isotopes in nephrology have been neglected by clinicians. The indications and techniques used in the renogram and renal scan in the light of our experience over 2 1/2 years are presented. Although these methods are finding ever-increasing applications, the main uses at this stage are the investigation of possible renovascular hypertension, acute obstructive renal failure and postrenal transplant complications.

ESRD from autosomal dominant polycystic kidney disease in the United States, 2001-2010.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is amenable to early detection and specialty care. Thus, while important to patients with the condition, end-stage renal disease (ESRD) from ADPKD also may be an indicator of the overall state of nephrology care. STUDY DESIGN: Retrospective cohort study of temporal trends in ESRD from ADPKD and pre-renal replacement therapy (RRT) nephrologist care, 2001-2010 (n = 23,772). SETTING & PARTICIPANTS: US patients who initiated maintenance RRT from 2001 through 2010 (n = 1,069,343) from US Renal Data System data. PREDICTOR: ESRD from…

Executive summary of the KDIGO 2025 Clinical Practice Guideline for the Management of Immunoglobulin A Nephropathy (IgAN) and Immunoglobulin A Vasculitis (IgAV).

The Immunoglobulin A nephropathy (IgAN) and Immunoglobulin A vasculitis (IgAV) management guideline was last updated and published as part of the Kidney Disease: Improving Global Outcomes (KDIGO) 2021 Clinical Practice Guideline for the Management of Glomerular Diseases. Based on new developments in IgAN assessment and therapy, a major update of the guideline was necessary in 2024. Compared with the last version, the KDIGO 2025 IgAN guideline now encourages a more liberal kidney biopsy policy and suggests aiming for stricter proteinuria control, with a goal of <0.5 g/d, ideally <0.3 g/d, and a

Addressing the Need for Clinical Trial End Points in Autosomal Dominant Polycystic Kidney Disease: A Report From the Polycystic Kidney Disease Outcomes Consortium (PKDOC).

Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disease. Expansion of multiple cysts throughout both kidneys is thought to lead to progressive loss of kidney function and kidney failure in some patients. In recent years, much has been learned about the pathophysiology of ADPKD. However, to date, only one therapy has been approved in the United States and in other regions for the treatment of ADPKD. Feasible end points and a clear regulatory pathway may stimulate further development in this area and ultimately lead to more treatments for ADPKD successfu

Ten years of laparoscopic living donor nephrectomy: retrospect and prospect from the nephrologist's point of view.

The laparoscopic living kidney donor nephrectomy introduced in 1995 has become an accepted method of kidney harvest for transplantation. The method has proven its usefulness as well as its superiority compared to open donor nephrectomy. Based on the results of a decade, an overview from a nephrologist's point of view is presented here in; a view that is known to be quite different from (and sometimes contrary to) the surgeon's approach. While urologists and surgeons focus more on the technique and complication rates, the nephrologist tends to estimate the new procedure with regard to his dialy
