Multifocal Motor Neuropathy with Persistent Conduction Block: The Seminal Case.

Although multifocal motor neuropathy (MMN) is now recognized as a distinct, albeit rare, neurological condition, the path to its recognition was long and winding. This article provides an insight into the medical history of MMN "patient zero" and the first scientific publication that led to the recognition of MMN by the medical community. Multifocal motor neuropathy is nowadays recognized as a disease that produces asymmetric muscle weakness and cramping, with spontaneous motor unit activity (fasciculations and myokymia) but without sensory disorder. From an electrophysiological point of view,

Evaluation and Management of Unsuccessful Carpal Tunnel Release.

Carpal tunnel release (CTR) is a routine and effective surgical treatment for carpal tunnel syndrome. Dissatisfied patients after CTR are uncommon but present diagnostic and therapeutic challenges. This article discusses the most common reasons for unsuccessful CTR, the appropriate ensuing workup, and current evidence regarding revision surgery. The authors' preferred treatment algorithm for the surgical management of the unsuccessful CTR is presented.

A Comparison of 6 Diagnostic Tests for Carpal Tunnel Syndrome Using Latent Class Analysis.

Background: Nerve conduction studies (NCS), ultrasonography (US), Carpal Tunnel Syndrome 6 (CTS-6), Wainner, Lo, and Kamath are clinical diagnostic tools that can be used to diagnose carpal tunnel syndrome (CTS). Latent class analysis (LCA) is a proven statistical technique that can be used to evaluate diagnostic tests in a lack of a reference standard. Given that there is no accepted reference standard, we elected to perform an LCA to evaluate the 6 clinical diagnostic tests. Methods: One hundred eighty-seven wrists were prospectively evaluated by a board-certified hand surgeon using US, the

Carpal tunnel syndrome among milking parlor workers in Northern Italy: a comparison of screening approaches.

BACKGROUND: Occupational tasks characterized by repetitive, awkward and forceful movements of the hand and wrist may heighten the risk of carpal tunnel syndrome (CTS) among dairy parlor workers. Median nerve impairment can be investigated with ultrasonography (US) and nerve conduction studies (NCS) but a structured questionnaire may help identify early symptoms. OBJECTIVES: Our objectives were to: a) compare the sensitivity of US investigations and NCS to detect early signs of CTS; b) explore the correlation of the results of these two tests with CTS symptoms obtained from the administration

Feasibility of kilohertz frequency alternating current neuromodulation of carotid sinus nerve activity in the pig.

Recent research supports that over-activation of the carotid body plays a key role in metabolic diseases like type 2 diabetes. Supressing carotid body signalling through carotid sinus nerve (CSN) modulation may offer a therapeutic approach for treating such diseases. Here we anatomically and histologically characterised the CSN in the farm pig as a recommended path to translational medicine. We developed an acute in vivo porcine model to assess the application of kilohertz frequency alternating current (KHFAC) to the CSN of evoked chemo-afferent CSN responses. Our results demonstrate the feasi

Clinicopathological characteristics of subtypes of chronic inflammatory demyelinating polyradiculoneuropathy.

OBJECTIVE: To evaluate the clinical and pathological correlations characterising each clinical subtype of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). METHODS: We assessed 106 consecutive patients who had CIDP fulfilling the European Federation of Neurological Societies and Peripheral Nerve Society criteria and had been referred for sural nerve biopsy. Patients with anti-neurofascin 155, anti-contactin 1 and anti-LM1 antibodies were excluded. RESULTS: 55 patients were classified as having typical CIDP. Regarding atypical CIDP, the multifocal acquired demyelinating sensor

Visualization of the electrical activity of the cauda equina using a magnetospinography system in healthy subjects.

OBJECTIVE: To establish a method to measure cauda equina action fields (CEAFs) and visualize the electrical activities of the cauda equina in a broadly aged group of healthy adults. METHODS: Using a 124-channel magnetospinography (MSG) system with superconducting interference devices, the CEAFs of 43 healthy volunteers (22-64 years of age) were measured after stimulation of the peroneal nerve at the knee. Reconstructed currents were obtained from the CEAFs and superimposed on the X-ray image. Conduction velocities were also calculated from the waveform of the reconstructed currents. RESULTS:

Optimizing electrodiagnosis for Guillain-Barré syndrome: Clues from clinical practice.

INTRODUCTION: The most efficient method of performing electrophysiology for Guillain-Barré syndrome (GBS) is unknown. METHODS: We retrospectively analyzed electrophysiological data of 97 consecutive GBS patients from Birmingham, UK (2001-2012), studied ≤ 3 weeks post-onset. RESULTS: The sensitivity of electrophysiology for each GBS subtype was dependent on the upper and lower limb nerves tested. In acute inflammatory demyelinating polyneuropathy (AIDP), abnormalities were predominant in the arms, whereas leg abnormalities predominated in axonal GBS. In AIDP, the most common abnormal paramete

Diagnostic Accuracy of Monofilament Tests for Detecting Diabetic Peripheral Neuropathy: A Systematic Review and Meta-Analysis.

OBJECTIVE: To systematically evaluate the diagnostic accuracy of monofilament tests for detecting diabetic peripheral neuropathy. METHODS: We searched EMBASE (OvidSP), MEDLINE (OvidSP), the Cochrane Library, and Web of Science to identify diagnostic accuracy trials of monofilament tests for detecting diabetic peripheral neuropathy. We used a hierarchical summary receiver operating characteristics (HSROC) model to conduct the meta-analysis of diagnostic accuracy of monofilament tests for detecting diabetic peripheral neuropathy. RESULTS: A total of 19 comparative trials met the inclusion crit

History of electromyography and nerve conduction studies: A tribute to the founding fathers.

The early development of nerve conduction studies (NCS) and electromyography (EMG) was linked to the discovery of electricity. This relationship had been concluded by observing the effect of applying electricity to the body of an animal and discovering that nerves and muscles themselves could produce electricity. We attempt to review the historical evolution of NCS and EMG over the last three centuries by reviewing the landmark publications of Galvani, Adrian, Denny-Brown, Larrabee, and Lambert. In 1771, Galvani showed that electrical stimulation of animal muscle tissue produced contraction an

Nerve ultrasound in Miller Fisher variant of Guillain-Barré syndrome.

INTRODUCTION: Focal enlargement of the peripheral and spinal nerves, visualized using high-resolution ultrasound (HRUS), has been reported in early Guillain-Barré syndrome, but not in the Miller Fisher variant. We report the use of HRUS in 2 patients who presented with acute ataxic neuropathy, areflexia, and ophthalmoparesis. METHODS: Ultrasound and/or nerve conduction studies (NCS) of peripheral nerves, the vagus, and spinal nerves C5/6 were performed at onset and 2 weeks after immunoglobulin therapy. RESULTS: Both patients fulfilled criteria for diagnosis of Miller Fisher syndrome (MFS). L

Practical rules for electrodiagnosis in suspected multifocal motor neuropathy.

Multifocal motor neuropathy (MMN) with conduction block (CB) is a rare chronic immune-mediated neuropathy, but important to diagnose as it is treatable. The key features in prototypic MMN are electrodiagnostic demonstration of focal CB away from common sites of entrapment and normal sensory conduction across these sites. However, there are challenges in distinguishing CB from the effects of abnormal temporal dispersion. Consensus electrodiagnostic criteria, reinforced by modeling studies, are available to support definite or probable CB. In addition, consideration of technical issues can guard

Harlequin syndrome in a patient with putative autoimmune autonomic ganglionopathy.

The author presents a patient with Harlequin and Horner syndromes as part of an autoimmune autonomic ganglionopathy and suggests implication for work-up and management. In general, Harlequin and Horner syndromes are reported to be caused by either a structural lesion of the sympathetic pathway or, when no structural lesion is found, are presumed to be idiopathic. In this paper, a 76 year old man developed a Harlequin and Horner syndromes in the setting of subacute autonomic failure and other systemic features. The patient's symptoms improved with a short course of intravenous methylprednisolon

[Carpal tunnel syndrome and Down's syndrome].

BACKGROUND: Carpal tunnel syndrome is caused by entrapment of the median nerve in the wrist, leading to pain, paraesthesia and numbness in the hand, especially at night. It is relatively common in adults but rare in children. CASE DESCRIPTION: We describe a 13-year-old girl with Down's syndrome who was experiencing increased tiredness. Her sleep was disrupted by pain and tingling in her hands. Electrophysiological examination revealed a bilateral absence of median nerve conduction, confirming the diagnosis of carpal tunnel syndrome. The diagnosis had not been made before, probably because the
