Neurological manifestations of xeroderma pigmentosum, a rare autosomal recessive neurocutaneous syndrome, are variable. The association with progressive mental retardation, usually with onset in childhood, is well known. We present a case of x.p. with progressive presenile dementia. This combination has, to our knowledge, not yet been reported in the literature. Although no hints on another aetiology have been found, the coincidental combination of x.p. with M. Alzheimer has to be taken into consideration. CT scan and MRI showed a marked cerebral atrophy.

Poor upper extremity function is often recorded in meningomyelocele patients. Only 2 of the 25 patients we assessed, 5 to 19 years old, showed normal upper extremity function in the clinical neurological examination and a timed hand function test simulating daily activities. Slow performance with unsystematic variability was typical. Poor hand function correlated strongly with hydrocephalus. A trend towards better performance with increasing age may indicate that the difficulties are overcome in some patients. While patients without hydrocephalus showed a near-normal distribution in the seven

We have described the important clinical features and aspects of the Chiari-I malformations, with particular emphasis on Chiari-I malformation. Previously thought to be a rare finding with only minor significance, Chiari-I malformation is an important cause of a variety of symptoms, and will be diagnosed even more frequently as the use of MRI increases. The clinician must consider Chiari-I malformation in any patient with unexplained sensorineural hearing loss, headache, vertigo, ataxia, dysequilibrium, dysphagia or other cranial nerve symptom, especially if accompanied by more classic symptom

1052 patients with facial pain have been examined and followed up by the author for an 18-year period. The patients are classified according to type of attack into: Typical Trigeminal Neuralgia (brief pain paroxysms with pain-free intervals). Atypical Trigeminal Neuralgia (pain paroxysms with intervals of pain or paroxysms lasting for minutes). Non-neuralgiform Facial Pain (pain lasting or occurring for long periods). The material was equally distributed between patients with Neuralgia and Non-neuralgiform Facial Pain. In the majority of cases Trigeminal Neuralgia occurred after the age of 50.

As many as 61 patients with neurogenic syncopes were examined. Of these, 46 demonstrated well-defined signs of the hyperventilation syndrome within the structure of the clinical manifestations. The remaining 15 patients had no disorders indicated. The control group comprised 18 healthy persons. Thorough analysis of factors provoking syncopal, pre- and postsyncopal conditions, studies into the psychic and vegetative spheres including the respiratory system--all this made it possible to define a number of features characteristic of patients entering the groups under study. It has been shown that

The clinical presentation and radiological features were analyzed in 30 cases of syringomyelia associated with Chiari malformation. None of the patients had spinal dysraphism. The age on admission ranged from 6 to 59 years with a mean of 27 years. Syringomyelia was diagnosed by CT myelography and or MRI from 1982 to 1988. The initial symptoms were skeletal abnormality (43%) such as scoliosis (12 cases) or pes cavus (one case), unilateral pain or numbness (40%) and unilateral motor weakness (17%). Frequently seen signs on admission were sensory deficit (100%), scoliosis (57%), muscle weakness (

We examined 7 adults with brachial plexus injury following liver transplantation; they were among 120 transplants performed over a 10-month period (incidence, 5.8%). The lesion was right-sided in 5 patients, left-sided in 1, and bilateral in 1. The affected side correlated with the axillary veno-venous shunt in 3 lesions only. All noted weakness and sensory loss shortly after recovery from general anesthesia. The EMG examination indicated axonal lesions, and recovery was protracted.

Till today the meaning of neuromuscular factors in etiology and treatment of congenital dislocation of the hip is not quite clear. Our observation in 62 children with 78 dislocated hips show that neurophysiological mechanisms play of very important role in the development and upkeep of CDH. The neurological examination in 17 children pointed to the typical asymmetries of the children which correlated with the side of the dislocated hip. The knowledge of neurophysiological disturbances on the dislocated hip is of great value for the diagnosis of CDH and even more for the exercise treatment in h

The disability outcome related to the presenting signs and symptoms was studied retrospectively in 193 patients with definite multiple sclerosis (MS). Patients whose first sign was motor weakness were found to be more severely disabled, at all stages throughout the course of the disease, than patients with other presenting signs or symptoms. Patients whose disease manifested initially with sensory disturbances or paraesthesiae proved to be less seriously disabled than other patients at all stages of the disease. A similar tendency was found in patients presenting with optic neuritis (ON) for t

Four situations are envisaged for which a Doppler "unravelling" examination in a patient with arteritis, conducted on a routine basis, led to the detection of stenotic or occlusive lesions of neck vessels. The first case concerns a patient with a recent or long-standing permanent neurological disorder related to an established cerebral lesion. Once the latter has been confirmed as being due to the cervical lesion then angiography is not always essential, since treatment of an established vascular accident is usually ineffective. The second case involves asymptomatic patients with a history ind

In patients with transient ischemic attack (TIA), the risk of stroke increases greatly, especially in the months immediately following the initial attack. Diagnosis of TIA is based primarily on the patient's cerebrovascular history, since results of neurovascular examination are usually normal. TIA is often related to atherosclerotic arterial disease but can have numerous causes. Migraine, focal seizures, and other neurologic conditions can closely mimic TIA. Surgical and medical therapies help minimize the risk of stroke. The choice of therapy depends on the vascular territory of ischemia, th

During a survey of neuronal ceroid lipofuscinosis (Batten's disease), two younger siblings of patients were found to have the characteristic vacuolation in 11 to 17 per cent of their lymphocytes. The children were four to five years of age and did not have any overt signs or symptoms of the disease. However, they both had markedly reduced auditory memory-span before decreased vision and funduscopic changes developed. The subsequent course of the disease confirmed the diagnosis. It is concluded that neuronal ceroid lipofuscinosis may affect mental functions much earlier than previously believed

The clinical and pathological features of two unrelated sporadic cases of neuroaxonal dystrophy occurring in young adults are described. Initial clinical manifestations in both patients were those of primary psychiatric disorders. They subsequently developed extrapyramidal symptoms, dementia, cerebellar ataxia, and corticospinal dysfunction, but neither demonstrated myoclonic seizures. In one patient the diagnosis was made before death by brain biopsy. On pathological examination both patients showed a generalized distribution of spheroids within the central nervous system and, in one, in the

Basal ganglia side-effects caused by neuroleptic drugs are frequently ignored, misdiagnosed, or mistreated by the patient or physician. Such side-effects must be anticipated and appropriate management instituted so that patient compliance with the drug regimen is increased and the therapeutic relationship is maintained. Side-effects can be detected in frequent and complete examinations of the head and neck, limbs, and chest and back. The various medications for drug-induced parkinsonism are reviewed, with an emphasis on the indications for selection and the effects of each drug. Common errors

Clinical responses to peppermint odor were observed in 100 healthy term and preterm infants during active sleep on two successive days. Thirty-six asphyxiated term infants and three infants with holoprosencephaly also were tested. Olfactory reflexes were regularly elicited in normal infants after 32 weeks' gestation, but some premature infants as young as 28 weeks also responded. Responses included initiation of sucking and arousal-withdrawal. Habituation occurred after two or three trials. A Moro response was seen only in a few asphyxiated infants; loss of olfactory reflexes appeared to paral
