In-game monitoring of adolescent handball players: a preliminary examination of associations between external load parameters and objective and subjective fatigue markers.

BACKGROUND: Advances in local positioning systems (LPS) have enabled detailed monitoring of external load in handball. However, the relationship between external load and internal fatigue under real match conditions remains insufficiently understood, particularly in adolescent players. This study examined associations between external load parameters and both objective and subjective fatigue markers during an official match and evaluated agreement between players 'ratings of perceived exertion (RPE) and coaches' ratings of observed exertion (ROE). METHODS: Highly trained adolescent male handb

A Clinical Practice Framework for Ultrasound-Informed Multimodal Management of Post-Stroke Spasticity: A Proof-of-Concept Case.

BACKGROUND: Stroke remains a leading cause of long-term disability worldwide. Post-stroke spasticity (PSS) is increasingly recognized as a complex complication resulting from the interaction between neural hyperexcitability and non-neural muscular adaptations, including fibrosis, altered muscle architecture, and increased stiffness. Consequently, effective management requires a comprehensive approach targeting both components. Musculoskeletal ultrasound (MSK US) has emerged as a practical tool for muscle characterization, treatment targeting, and longitudinal monitoring within neurorehabilitat

Utility of Serial Repetitive Nerve Stimulation in Differentiating Pharmacological From Immunological Exacerbation of Myasthenia Gravis: A Case Report.

Myasthenia gravis (MG) can be exacerbated by certain medications. However, the contribution of many suspected medications remains uncertain, often based on case reports and confounded by variable drug responses and the fluctuating nature of MG. Objective drug evaluations are needed to establish causation. This report examines cibenzoline, an antiarrhythmic suspected of exacerbating MG, and proposes a method for assessing the causal role of medications in MG exacerbations through daily repetitive nerve stimulation (RNS) correlated with drug levels. We present a case of a 78-year-old man with st

Feasibility study of respiratory function home monitoring for patients with Myasthenia Gravis.

BACKGROUND: Myasthenia gravis (MG) is a rare and chronic autoimmune neuromuscular disease that causes weakness among skeletal muscles responsible for mobility and breathing. Individuals with MG who have difficulty breathing may need to schedule an emergent provider visit where a negative inspiratory force (NIF) measurement test is performed to measure respiratory strength. PURPOSE: Examine the feasibility of individuals with MG to perform NIF respiratory function monitoring in the home setting and determine its impact on their perceived quality of life. METHODS: This is a quantitative one-gr

A case report: use of cerebral oximetry in the early detection of cerebral hypoperfusion in a post-cardiac arrest patient during targeted temperature management.

BACKGROUND: We present a patient who received cerebral oximetry monitoring during targeted temperature management (TTM) post-cardiac arrest and discuss its potential in the early detection of cerebral hypoperfusion and implications on haemodynamics and ventilatory management. CASE SUMMARY: A 60-year-old Chinese male was admitted for acute pulmonary oedema with Type 2 respiratory failure. He failed an initial trial of non-invasive ventilation and was planned for intubation and mechanical ventilation. However, the patient suffered a pulseless electrical activity cardiac arrest peri-intubation.

Guillain-Barré syndrome: a comprehensive review.

Guillain-Barré syndrome (GBS) is a potentially devastating yet treatable disorder. A classically postinfectious, immune-mediated, monophasic polyradiculoneuropathy, it is the leading global cause of acquired neuromuscular paralysis. In most cases, the immunopathological process driving nerve injury is ill-defined. Diagnosis of GBS relies on clinical features, supported by laboratory findings and electrophysiology. Although previously divided into primary demyelinating or axonal variants, this dichotomy is increasingly challenged, and is not endorsed by the recent European Academy of Neurology

Open thoracoabdominal aortic aneurysm repair in a patient with myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune neuromuscular junction disorder and rarely coexists with aortic aneurysms requiring open repair. A 66-year-old patient with MG underwent extended thoraco-abdominal aortic aneurysm (TAAA) repair 16 years after onset of type-B acute aortic dissection. At 62 years, the patient was diagnosed with MG (MGFA class IIIa) from positive anti-acetylcholine receptor antibody without thymoma. Preoperatively, MG was well-controlled by prednisolone, cyclosporin and pyridostigmine. Extent II TAAA repair was performed under general anaesthesia maintained by total intrave

Imaging immunological processes from blood to brain in amyotrophic lateral sclerosis.

Neuropathology studies of amyotrophic lateral sclerosis (ALS) and animal models of ALS reveal a strong association between aberrant protein accumulation and motor neurone damage, as well as activated microglia and astrocytes. While the role of neuroinflammation in the pathology of ALS is unclear, imaging studies of the central nervous system (CNS) support the idea that innate immune activation occurs early in disease in both humans and rodent models of ALS. In addition, emerging studies also reveal changes in monocytes, macrophages and lymphocytes in peripheral blood as well as at the neuromus

Anesthetic management of comprehensive dental restoration in a child with glutaric aciduria type 1 using volatile sevoflurane.

Glutaric aciduria type 1 (GA1) is a rare, inherited mitochondrial disorder that results from deficiency of mitochondrial glutaryl-CoA dehydrogenase. Most patients develop neurological dysfunction early in life, which leads to severe disabilities. We present a 37-month-old girl with GA1 manifested as macrocephaly and hypotonia who received comprehensive dental restoration surgery under general anesthesia with sevoflurane. She was placed on specialized fluid management during a preoperative fasting period and anesthesia was administered without complications. All the physiological parameters, in

[Treatment of chronic hepatitis B: adherence and safety].

Second generation nucleos (t) idic analogues result in a complete viral suppression after 48 to 96 weeks of therapy in most patients, regardless of the virus (HBV genotype, wild type or pre-C mutant), the underlying liver disease (cirrhosis or not) or the immune status (mono- or HIV/HBV co-infection). This antiviral efficacy may result in HBe or HBs seroconversion. Its clinical impact is important since inactivation of necroinflammation allows, in the absence of liver comorbidities, a stabilisation then a reversal of fibrosis and cirrhosis, and consequently a decrease in the occurrence of carc

Beyond the jaw: vocal, oropharyngeal and cognitive-linguistic biomarkers for precision pain phenotyping in TMD.

Temporomandibular disorders (TMDs) represent a group of conditions affecting the temporomandibular joint and its associated musculature. TMD is the leading cause of chronic orofacial pain. Current assessment relies heavily on patient-reported outcomes, which capture pain intensity and its impact but provide limited mechanistic or functional insight. This narrative conceptual review proposes a framework for investigating vocal, oropharyngeal, and cognitive-linguistic (VOCL) features as candidate behavioral biomarkers for precision pain phenotyping in TMD. We synthesize the literature from TMD a

Respiratory insufficiency and sleep impairment in facioscapulohumeral muscular dystrophy.

Respiratory involvement in facioscapulohumeral muscular dystrophy is a clinically relevant yet frequently underdiagnosed. Manifestations range from subclinical restrictive ventilatory impairment to sleep-disordered breathing and nocturnal hypoventilation, and contribute to reduced quality of life as well as increased morbidity and mortality. Restrictive patterns occur in approximately 10-45 % of patients and are primarily associated with early disease onset, greater clinical severity, spinal deformities, and loss of ambulation. Pathophysiology is multifactorial, involving respiratory muscle we

Paediatric Long-Term Home Respiratory Support: Recommendations of the Swiss Society of Paediatric Pulmonology.

The number of paediatric patients evaluated for long-term home respiratory support has risen sharply in recent years, reflecting both improved survival of children with complex chronic conditions and expanded therapeutic indications. This population is characterized by substantial clinical heterogeneity, encompassing obstructive, restrictive, or central ventilatory disorders. Contributing comorbidities - such as severe obesity, chronic aspiration with secondary lung disease, neuromuscular weakness, impaired central respiratory drive, and dynamic upper airway obstruction - further complicate di
