Role of the Nurse Practitioner in Caring for Heart Transplant Patients: From Hospitalization Through Reintegration Into the Community.

Nurse practitioners are essential members of the multidisciplinary team managing heart transplant candidates and recipients, a patient population with highly specialized, complex care needs. Nurse practitioners contribute to successful patient outcomes by delivering expert care, facilitating smooth transitions, and maintaining continuity of care throughout the patient's journey. This continuum of care progresses from pretransplant evaluation and optimization, to the vulnerable perioperative and recovery period, and throughout the patient's transition back into the community, which entails life

An instrumental variables approach to post-acute care nursing home quality: is there a dime's worth of evidence that continuing care retirement communities provide higher quality?

For the affluent elderly, continuing care retirement communities (CCRCs) have become a popular option for long term care and other health care needs related to aging. While CCRCs have experienced significant growth over the last few decades, very little is known about the quality of care CCRCs provide. This paper is the first to rigorously study CCRCs on a national scale and the only study that focuses on nursing home quality. Using a national sample from 2005, we determine if the quality of post-acute care provided by CCRC nursing homes is superior to traditional nursing homes. To mimic rando

Nursing education: use of clinical simulation in the care of patients with dengue fever.

OBJECTIVE: To analyze how clinical simulation impacts nursing students' proficiency in clinical evaluation, decision-making, therapeutic communication, and reporting of dengue fever with warning signs within a primary health care context. METHODS: Third-term nursing undergraduates participated in the activity, which involved a standardized patient, a 45-year-old female with persistent vomiting, fever, and abdominal pain after a trip to an endemic dengue area. A structured checklist was used to assess their performance in terms of diagnostic suspicion, anamnesis, physical examination, tourniqu

Living Longer after Labor Market Exit: Making Functional Healthspan a Public Health Nursing Priority.

Increasing longevity after labour market exit represents a major social achievement, but the number of years gained does not indicate whether they are lived with preserved mobility, autonomy, cognitive resources, and social participation. Across many countries, retirement and labour market exit precede two or more decades of later life, creating an overlooked opportunity for preventive action. This editorial argues that functional healthspan during these years should become a public health nursing priority. Nurses working in public health, primary care, and community settings are well position

Providing outpatient antibiotic therapy for cellulitis in primary care.

Outpatient parenteral antimicrobial therapy (OPAT) is becoming more widespread. OPAT therapy can be used to treat certain patients who have cellulitis. The decision as to which patients to treat at home must be based on local PCT guidelines--not all patients are suitable for OPAT. OPAT improves patient quality of life by delivering care in the patient's home. This is highly skilled work and the community nurse must have appropriate training and support in order to gain the skills required.

A rare case of fatal rectal perforation and sepsis following traumatic urinary catheterization.

This case report details a fatal rectal perforation and sepsis in a comorbid 96-year-old male after traumatic urinary catheterization, highlighting the risks of IDC management in elderly patients with complex health backgrounds. Despite maximal medical therapy, including escalated antibiotics and ICU care, the patient died from septic shock linked to improper catheter insertion by a non-specialist nurse in the community. This case emphasizes the urgent need for better catheterization practices, specialized nursing education, and clear guidelines to prevent such outcomes.

Anorexia nervosa: identification and management by non-specialist nurses.

Anorexia nervosa - commonly referred to as anorexia - is a significant mental health condition characterised by excessive weight loss and distorted body image. However, although anorexia may be regarded primarily as a mental health issue, nurses from all fields and specialties should have the necessary knowledge and skills to identify and care for individuals with the condition in the community and non-specialist healthcare settings. This article outlines the aetiology of anorexia and considers how individuals with the condition might be identified in primary care or emergency settings. It det

Reduce noise: Enhance health equity-An AAN Consensus Paper.

A growing body of epidemiological studies shows that noise increases the risk for broad-ranging physiological and psychological illnesses. According to the World Health Organization, at least 1.6 million healthy years of life are lost annually in Western Europe due to traffic noise alone. Scholars from selected Expert Panels of the American Academy of Nursing convened to update the information regarding the effects of environmental noise on health disparities, physiological changes in the human body and in animals, and environmental justice issues, and to inform the public and policy makers' e

An Interprofessional Approach to Continuing Education With Mass Casualty Simulation: Planning and Execution.

Many natural and man-made disasters require the assistance from teams of health care professionals. Knowing that continuing education about disaster simulation training is essential to nursing students, nurses, and emergency first responders (e.g., emergency medical technicians, firefighters, police officers), a university in the northeastern United States planned and implemented an interprofessional mass casualty incident (MCI) disaster simulation using the Project Management Body of Knowledge (PMBOK) management framework. The school of nursing and University Volunteer Ambulance Corps (UVAC)

Benefits and challenges of community-based multi-month dispensing of antiretroviral treatment in Zimbabwe: A qualitative study from a cluster randomized trial.

Sub-Saharan Africa carries the highest burden of HIV, with approximately 70% of all people living with HIV (PLWH) globally living in this region. The provision of antiretroviral treatment (ART) significantly affects already overburdened health systems, which need to accommodate large volumes of ART patients while facing a shortage of professional health workers, infrastructure challenges and medical resources. Finding alternative ways to provide routine services to PLWH has become significantly more urgent. Multi-month dispensing (MMD) of ART aims to improve access to treatment for PLWH, while

Improving the uptake of HIV testing in patients with tuberculosis.

The HIV epidemic has increased the global tuberculosis (TB) burden and focused attention on the need to strengthen links between TB and HIV programmes in order to tackle these public health issues more effectively (World Health Organization (WHO), 2004). HIV testing is now recommended in various healthcare settings and guidelines state that it should include all patients with a diagnosis of active TB (British HIV Association, British Association of Sexual Health and HIV, British Infection Society, 2008; Health Protection Agency (HPA), 2009). This has been shown to standardise HIV testing and i

Telephone triage performed by nurses reduces non-urgent ambulance missions: A prospective observational pilot study in Finland.

BACKGROUND: The increased workload in emergency medical services (EMS) is a global phenomenon in welfare states. It has been suggested that telephone triage by nurses may reduce the increasing use of EMS services, by directing patient flow to appropriate care. This study aimed to investigate whether, after an emergency medical communication centre (EMCC) provider assessed risk, a telephone nurse could assess the patient's needs and guide patients to social and health care services in non-urgent cases. METHODS: This prospective observational study was performed in the Kainuu Hospital District
