Central venous cannulation in neonates.

A modified technique for central venous cannulation for parenteral nutrition in neonates has been developed. The internal or external jugular veins are cannulated and the catheter is tunneled out onto the anterior part of the chest wall through a long subcutaneous tunnel. Fixation of the catheter is accomplished by gluing two small pieces of Silastic sheeting to the catheter and then suturing this sandwich to the skin. Over 150 central venous catheters for total parenteral nutrition have been placed using this method. The reduced rate of catheter-related complications and improved ease of nurs

Alignment of Midwifery Education in Nepal With Global Standards and Essential Competencies From the International Confederation of Midwives: A Mixed-Methods Study.

INTRODUCTION: Maternal and neonatal mortality rates in Nepal remain high, reflecting persistent gaps in the quality of maternal and newborn care. To address this issue, a national midwifery education program was initiated in 2016. This study assessed Nepal's preservice midwifery curricula against global standards and competencies defined by the International Confederation of Midwives (ICM). METHODS: A mixed-methods study was conducted, comprising a desk review of national standards and curricula against ICM education standards (2021) and competencies (2024), 15 stakeholder interviews, 3 focus

Moisture-associated skin damage and its management in neonatal and infant populations: a retrospective study in Italy.

OBJECTIVE: Moisture-associated skin damage (MASD) is a significant concern in the paediatric population, particularly among neonates, who exhibit the highest incidence due to their highly sensitive and fragile skin. The aim of the study was to identify the most effective treatment. METHOD: A retrospective observational study was conducted at Meyer Children's Hospital IRCCS, Florence, Italy. Data from infants admitted to the neonatal intensive care unit of the hospital were collected from electronic health records. Demographic, clinical and nursing data were analysed to identify the key clinic

Remote Mentorship to Improve Continuous Positive Airway Pressure Use in the Ethiopian Neonatal Network.

BACKGROUND: Continuous positive airway pressure (CPAP) is an evidence-based therapy for respiratory distress syndrome (RDS), the leading cause of death for preterm infants globally. Ethiopian Neonatal Network (ENN) teams identified a quality gap in CPAP use for preterm infants with RDS. We sought to use remote education and mentorship without additional resources to improve CPAP use in this population. METHODS: Nineteen public ENN hospitals participated in this quality improvement (QI) collaborative (September 2021 to September 2022). The primary intervention was implementation of a telemento

A qualitative analysis of the Three Good Things intervention in healthcare workers.

BACKGROUND: Intensive care unit (ICU) personnel have an elevated prevalence of job-related burn-out and post-traumatic stress disorder, which can ultimately impact patient care. To strengthen healthcare workers' skills to deal with stressful events, it is important to focus not only on minimising suffering but also on increasing happiness, as this entails many more benefits than simply feeling good. Thus, the purpose of this study was to explore the content of the 'good things' reported by healthcare workers participating in the 'Three Good Things' intervention. METHODS: In a tertiary care me

Brugia pahangi: circulating antibodies to adult worm antigens in uninfected progeny of homologously infected female jirds.

Serum IgG antibody levels to adult Brugia pahangi antigens were measured in uninfected offspring from uninfected and B. pahangi-infected female jirds. Antibody titers to B. pahangi antigens in sera of offspring from infected females mimicked the maternal titer during the suckling period. Neonate titers peaked at 2 weeks of age at levels as high as 1:4100, then decreased to levels well below maternal titers by 8-12 weeks of age. Concurrent maternal and 2-week-old neonate sera recognized identical B. pahangi antigens in Western blots. Spleen cells from 2-week-old filariae-exposed and unexposed o

Perinatal Care for Individuals With a History of Sexual Trauma.

Many women and individuals assigned female at birth experience sexual assault or abuse in their lives leading to sexual trauma. In this article, we review the effects of sexual trauma and resulting chronic stress on the body and during the perinatal period. Maternal, fetal, and neonatal health implications are discussed. Routine screening for sexual assault and violence can assist with early identification and intervention. A variety of modalities and methods for managing the effects of sexual trauma have been identified, including pharmacologic treatment, psychotherapy, complementary and alte

Society for Maternal-Fetal Medicine Special Statement: A maternal transport briefing form and checklist.

When performing a maternal transport between two facilities, numerous pieces of information must be communicated between physicians, nurses, and transport personnel, including the health status of 2 patients (mother and fetus), availability of bed space and personnel in 2 units at the receiving facility (labor and delivery unit and neonatal intensive care unit), arrangements for transportation, and inpatient and outpatient records. The amount and complexity of information creates a risk of medical error due to communication lapses. A cognitive aid such as a standardized form can help the team

Management of communication and swallowing in intensive care: the role of the speech pathologist.

Patients in adult, pediatric, and neonatal intensive care settings often require the services of a speech-language pathologist. It is common practice to consult a speech pathologist to determine whether a patient is ready to initiate oral intake or help a patient with an artificial airway communicate. Assessments for dysphagia are initially clinical and conducted at bedside. Results from the clinical evaluation help determine if and when an instrumental examination should be completed. Patients who are tracheostomized, or had been, and those who were intubated for a prolonged period are at ris

Biological agents and pregnancy.

Pregnant women are exposed to many biological, eg microbial, agents, which are potentially harmful to the fetus. The reported rates of vertical transmission of hepatitis B and human immunodeficiency virus vary between 3 to 90% and 0 to 65%, respectively. The susceptibility to hepatitis B and human immunodeficiency infection is increased in pregnant physicians, midwives, and nurses because of the bloodborne nature of these viruses. Also, TORCH (toxoplasmosis-rubella-cytomegalovirus-herpes) infections, acquired during pregnancy, may result in congenital infection, and serious sequelae in the neo

Nosocomial bacteremia in neonates related to poor standards of care.

BACKGROUND: In developing countries, intravenous liquids are mixed and administered by nurses, sometimes under suboptimal infection control conditions. We hypothesized that outbreaks of infusate-associated neonatal bacteremias are common, and we evaluated whether they can be detected by vigilant microbiologic surveillance of infusates. METHODS: We studied intravenous infusates administered to neonates in a Mexican hospital where mixtures of infusates were prepared in hospital wards. The study was performed in 3 stages: stage 1, initial culturing of in-use infusates under basal conditions; sta

Role of simulation in preparation for the care of conjoined twins-prenatal preparation to separation.

The rarity and complexity of conjoined twins creates a challenge for prenatal planning, delivery resuscitation, and postnatal management. The modality of simulation offers a safe practice environment for a multidisciplinary group consisting of neonatal providers, nurses, respiratory therapists, and surgeons in which to identify and address clinical decision making, procedural, and behavioral plans related to routine and emergency care of these patients. Simulation-based clinical rehearsals (SbCR) provide a unique opportunity to prepare for rare, complex, and patient specific clinical procedure

The Effect of Route of Administration and Vehicle on the Pharmacokinetics of THC and CBD in Adult, Neonate, and Breastfed Sprague-Dawley Rats.

Introduction: Basic pharmacokinetic (PK) and pharmacodynamic models of the phytocannabinoids Δ-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) are critical for developing translational models of exposure and toxicity. The neonatal period is a particularly important time to study the effects of cannabinoids, yet there are few studies of cannabinoid PKs by different routes such as direct injection or breast milk ingestion. To study this question, we have developed a translationally relevant rodent model of perinatal cannabinoid administration by measuring plasma levels of THC and CBD after di

Impact of Adherence to a Standardized Oxytocin Induction Protocol on Obstetric and Neonatal Outcomes.

BACKGROUND: Oxytocin protocols are employed to induce uterine contractions and progressive cervical changes, but they are associated with adverse maternal and neonatal outcomes. The aim of this study was to determine whether compliance with a checklist-based protocol for oxytocin administration was associated with changes in neonatal and maternal outcomes. METHODS: A retrospective cohort study of 86,786 pregnant women undergoing term (> 37 weeks) induction of labor between January 2015 and December 2017 was performed. Systemwide training in the use of an oxytocin administration protocol was p

Analysis of noise levels in the neonatal intensive care unit: the impact of clinical microsystems.

Reorganization of neonatal intensive care by introducing clinical microsystems may help to allocate nursing time more appropriately to the needs of patients. However, there is concern that cohorting infants according to acuity may enhance noise levels. This single-center study investigated the impact of reorganization of neonatal intensive care unit by implementing clinical microsystems in a Level III NICU on environmental noise. This prospective study measured 24-h noise levels over a period of 6 months during pre- and post-implementation of microsystems cohorting infants of similar acuity. C

Nurse-Led Interventions to Mitigate High-Alert Medication Errors in Critical Care: A Systematic Review of Effectiveness and Implementation Strategies.

BACKGROUND: Intensive care units (ICUs) are high-risk environments where nurses administer the majority of high-alert medications, making them the final checkpoint in the medication-use process. However, evidence on nurse-led interventions specifically targeting high-alert medication errors has not been systematically synthesized across all ICU populations. AIM: This systematic review evaluated the effectiveness of nurse-led interventions in reducing high-alert medication errors in neonatal, paediatric and adult intensive care units and examined implementation strategies that enhance patient
