Complications in the use of the halo fixation device.

The medical records of 179 patients were reviewed to identify complications related to the use of the halo external skeletal-fixation device. The complications that were identified included pin-loosening in 36 per cent of the patients, pin-site infection in 20 per cent, pressure sores under either a plastic vest or a plaster cast in 11 per cent, nerve injury in 2 per cent, dural penetration in 1 per cent, dysphagia in 2 per cent, cosmetically disfiguring scars in 9 per cent, and severe pin discomfort in 18 per cent. One hundred and eighty (25 per cent) of the 716 pins used had become loose at

Open fractures of the tibia treated by the Hoffmann external fixator.

In a prospective study 40 consecutive patients with open tibial fractures were treated by the Hoffmann external fixator; the follow-up period was 18 months. All fractures were irrigated, debrided, and fixed rigidly with the Vidal-Adrey modification of the Hoffmann device as soon as the patients' condition allowed. The wounds were left open, to be closed secondarily with skin grafts, muscle flaps, and other plastic surgical procedures. The 40 wounds were all classified as Gustilo's Type II or III in relation to soft tissue damage; 36 injuries were due to motor vehicle accidents and four to guns

Do Nuss bars compromise the blood flow of the internal mammary arteries?

OBJECTIVES: Minimally invasive repair of pectus excavatum, the so-called Nuss procedure, has become a popular technique in recent years. The internal mammary arteries (IMAs) lie on the posterolateral surface of the sternum, and the Nuss bar is likely to obstruct the blood flow in these arteries. This obstruction could become important in the later stages of the lives of these young people if they were to require coronary artery bypass grafting. The goal of this study is to investigate the extent of obstruction of the IMAs caused by Nuss bars. METHODS: Data were collected prospectively on all

Titanium wire internal fixation for stabilization of injury of the cervical spine: clinical results and postoperative magnetic resonance imaging of the spinal cord.

Nine patients with dislocation of the cervical spinal with posterior ligamentous damage were treated with posterior internal fixation using a twisted pair of 22-gauge titanium wires and iliac crest bone fusion. Fixation using the titanium wire was compared with fixation using stainless steel wire for differences in surgical insertion, long term stability of bony fusion, and postoperative magnetic resonance imaging (MRI) artifacts near the implanted wire. MRI of the cervical spine is valuable for diagnosing the acute and chronic consequences of traumatic cervical spinal injury by providing anat

Management of compound injuries of the lower extremity.

The reconstructive surgeon experienced in the use of rotated or free muscle and myocutaneous flaps, skilled in microvascular surgery, and interested in lower extremity trauma, may favorably alter the course of management of these historically difficult problems. It can be of great advantage to the patient if this surgeon is involved in the primary evaluation, so that all initial therapy can be performed with the future reconstructive goals in mind.

Effect of different artifact reduction methods for quantitative computed tomography measurements of bone mineral density: a pilot study.

BACKGROUND: The increasing use of metal internal fixation devices has heightened clinical concerns about peri-implant complications, such as osteoporosis, fracture risk, and hardware loosening. Quantitative computed tomography (QCT) is employed for the quantitative assessment of bone mineral density (BMD) at specific anatomical sites in clinical settings, while conventional CT is used to visualize the structures surrounding the implant. However, significant metal artifacts present in the images can impede the observation and diagnosis of adjacent bone structures. This study systematically eval

Quality of Life in Children With Achondroplasia Undergoing Paired Limb Lengthening With an External Fixator and Modified Distraction Control: Observational Nonrandomized Study.

BACKGROUND: Transosseous distraction osteosynthesis is prioritized in orthopedic care for children with achondroplasia. However, difficulties encountered during treatment and rehabilitation directly impact patients' quality of life. Using rod external fixators within a semicircular frame for osteosynthesis is less traumatic compared to spoke circular devices. Their straightforward assembly and mounting on the limb segment can help significantly reduce treatment duration, thereby improving children's quality of life during treatment and rehabilitation. OBJECTIVE: This study aimed to conduct a

Comparison of the outcome following the fixation of osteotomies or fractures associated with total hip replacement using cables or wires: the results at five years.

There are no recent studies comparing cable with wire for the fixation of osteotomies or fractures in total hip replacement (THR). Our objective was to evaluate the five-year clinical and radiological outcomes and complication rates of the two techniques. We undertook a review including all primary and revision THRs performed in one hospital between 1996 and 2005 using cable or wire fixation. Clinical and radiological evaluation was performed five years post-operatively. Cables were used in 51 THRs and wires in 126, and of these, 36 THRs with cable (71%) and 101 with wire (80%) were evaluated

Stress distribution on the thorax after the Nuss procedure for pectus excavatum results in different patterns between adult and child patients.

OBJECTIVE: In the Nuss procedure, in which the deformed thorax is forcibly corrected by insertion of correction bars, considerable stresses occur on the patient's thorax. We performed the present study to elucidate how stress patterns on the thorax after this procedure differ between child and adult patients. METHODS: Eighteen patients with pectus excavatum, constituting a child group (n = 10) and an adult group (n = 8), were included in the study. After a 3-dimensional computer-assisted design model was produced with computed tomographic data from each patient, simulation of the Nuss procedu

Endovascular embolization of iatrogenic vertebral artery injury during anterior cervical spine surgery: report of two cases and review of the literature.

STUDY DESIGN: Case description. OBJECTIVES: To report 2 cases of vertebral artery injury (VAI) during anterior cervical decompression surgery and review the pertinent medical literature. SUMMARY OF BACKGROUND DATA: The incidence of VAI during an anterior surgical approach to the cervical spine is rare but potentially lethal. The spine surgeon should be aware of the detailed surgical anatomy and be prepared to manage an inadvertent injury to the vertebral artery. METHODS: In the first case presented, infection was the cause of VAI. During aggressive irrigation and pus drainage, massive bleed

Surgical correction of orbital and periorbital deformities using lamella and complex osteotomies in both orbital rim and wall.

PURPOSE: For dealing with the problems in orbital and periorbital deformities. METHODS: In orbital deformities such as hypertelorism, orbital dystopia, radiated orbit, and secondary deformities of traumatic orbit, the authors used lamella osteotomies of either orbital rim and wall or complex osteotomies in both adjacent orbital rims and walls with bicoronal incision and galea aponeurotica dissection. In most of the cases, the fragments of complex osteotomies were repositioned with rigid fixation. RESULTS: Best results with nearly normal orbital volumes, better contour of orbit and midface, a

Simultaneous somatosensory evoked potential and electromyographic recordings during lumbosacral decompression and instrumentation.

OBJECTIVE: Both motor and sensory neurological deficits have been reported after decompression and instrumentation of the lumbosacral spine. In this report, we describe a simple and effective method by which sensory and motor functions can be monitored simultaneously, using somatosensory evoked potentials (SSEP), spontaneous electromyographic (EMG) activity, and compound muscle action potential monitoring. The concomitant use of these monitoring techniques allows ongoing functional evaluation of the cauda equina and spinal cord during patient positioning, surgery, wound closure, and anesthetic

Atlas fractures: treatment and long-term results.

Fractures of the first cervical vertebra (C1) represent approximately 7% of all acute cervical spine fractures. Frequently, the atlas will be fractured in combination with the axis (44% of cases). Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute traumatic atlas fractures are reviewed. The long-term outcome from an atlas fracture is good (median follow-up, 40 months).
