Soft-tissue ankle equinus contracture is an important component of numerous foot and ankle deformities. In high-risk patients who have multiple co-morbidities, procedure selection and careful surgical technique are paramount to increase the likelihood of postoperative success. Scaphoid nonunions can exist with or without avascular necrosis of the proximal pole, and waist fractures may have an associated humpback deformity. CT best shows the deformity and bone loss, whereas MRI will show avascular necrosis. Operative treatment should be directed at correcting the deformity with open reduction and internal fixation and bone grafting. Vascularized bone grafts should be used in cases of avascular necrosis.

Osteoporotic vertebral compression fractures have previously been treated nonoperatively given the tremendous morbidity associated with open fixation in elderly patients who often have multiple medical comorbidities. Multiple surgical procedures have been described for the correction of hallux limitus deformity. We describe a new modification of the Youngswick procedure for the surgical treatment of hallux limitus. Other procedures for hallux limitus correction are also discussed. This modified Youngswick procedure will provide a new approach to treating hallux limitus secondary to metatarsus primus elevatus when shortening of the first metatarsal is not indicated.

Unreduced posterior dislocations of the shoulder are rare and difficult to treat. Treatment depends of defect of the humeral head. Since 1997 to 2002 eleven patients were referred to us for treatment unreduced posterior dislocations of shoulder with impressed fracture of the humeral head. Agenesia of the anterior cruciate ligament is a rare entity which is exceptionally symptomatic. High-grade spondylolisthesis is defined as Meyerding grade 3, 4, or 5 with an increased L5-SI kyphosis/slip angle. Modern-day treatment of high-grade spondylolisthesis usually involves some reduction of the spondylolisthesis along with pedicle screw instrumentation and circumferential fusion at L5-S1, achieved entirely through a posterior approach or through separate anterior and posterior approaches. After reviewing our experience with pollicization in congenital cases of thumb hypoplasia or aplasia, we found that classic techniques have several weak points concerning function and appearance. Abduction is frequently inadequate, and adduction is quite weak. Esthetically the thumb has a slender aspect, the web fold is absent, and the commissure looks more like a cleft. We tried to prioritize the issues to propose some technical modifications for improvement of function and appearance.
