The case of a 37-year-old man with hematogenous osteomyelitis associated with the human immunodeficiency virus (HIV) is presented, with a review of the literature. Hematogenous osteomyelitis is a relatively rare entity in the patella; most cases have involved adolescents and immunocompromised patients. There have been no previous reports of hematogenous osteomyelitis in HIV-positive patients. The diagnosis requires clinical suspicion and roentgenographic evidence. Point tenderness over the patella and a painful, swollen knee joint are signs that should alert a physician to the possibility of h…

Forty-one histologically documented instances of focal sclerosing osteomyelitis and focal periapical osteopetrosis have been evaluated for radiomorphologic characteristics. These reactive lesions exhibit five distinct patterns that include periapical focal opacifications, target lesions, radiolucencies, multiconfluent configurations, and opacifications with concomitant root resorption. While an irritational origin can usually be detected in chronic sclerosing osteomyelitis, the cause of focal periapical osteopetrosis remains enigmatic.

A rare case of actinomycotic osteomyelitis with an intracranial granuloma is presented. The computerized tomography findings are probably the first of their kind. Both the site of osteomyelitis and clinical presentation are unusual. The pertinent literature is reviewed.

Smallpox osteomyelitis (osteomyelitis variolosa) with gross clinical and radiologic deformities of the elbows, hands and feet occurred in a 53-year-old Vietnamese refugee. This disease, once common in Asia and Africa, may cause permanent damage to joints and growth centers in the course of the acute infection in childhood. The roentgenographic features are sufficiently diagnostic to exclude congenital dysplasia of the skeletal system, leprosy, or sequelae of thermal injury, and other joint disorders.

Osseous infections have a special importance among the various polymorphic osteoarticular troubles of drepanocytemia. From 92 cases, the authors emphasize 4 points: --Osteitis and osteomyelitis are frequent, specially in the homozygotic form. --Salmonellae are the prevailing pathogenic bacterias. --Osteomyelitis caused by staphylococcus have, in drepanocytic patients, a more serious and more expanding evolution than in other patients. --Anatomic and radiologic features as well as evolution are different in the staphylococcal forms and in the salmonellal ones in which, most often, surgery may b…

Chronic nonbacterial osteomyelitis (CNO) is a rare, autoinflammatory bone disorder that predominantly affects children and adolescents. The disease covers a broad clinical spectrum ranging from isolated bone lesions to its severe multifocal form, chronic recurrent multifocal osteomyelitis (CRMO). Although its exact pathogenesis remains elusive, recent advances highlight a pivotal role of innate immune dysregulation, particularly involving imbalanced cytokine signaling. These abnormalities drive sterile bone inflammation and osteoclast activation, leading to bone pain and lytic or sclerotic les…

RATIONALE: Denosumab is a commonly used agent for the management of osteoporosis, particularly in elderly populations. The adverse reaction associated with its use is osteomyelitis of the jaw, which poses significant clinical challenges, especially in patients with compromised overall health.
PATIENT CONCERNS: The patient was an elderly individual with osteoporosis and a documented history of denosumab treatment. At the time of consultation, the primary manifestations included pain, swelling, and purulent discharge at the extraction socket, which impaired normal eating and quality of life. Th…

Clinical presentation and microbial culture among osteomyelitis patients is required for proper diagnosis and management. Therefore, it is of interest to evaluate the clinical presentation and microbial culture among osteomyelitis patients. Hence, 200 patients with osteomyelitis having clinical symptoms and radiological findings were qualified for participation. Specimens such as synovial fluid, bone sequestrum, pus swabs and pus were collected aseptically and examined for microbial growth. Clinical assessment of osteomyelitis patient showed that most commonly affected bone was tibia with trau…

Vertebral osteomyelitis is a disc and vertebral infection that causes nonspecific symptoms such as back pain, fever, and weakness. The most common causative pathogen is Staphylococcus aureus, and Escherichia coli (E. coli) is an uncommon cause. An 88-year-old man presented to the emergency department with a fever and lower back pain. His blood cultures were persistently positive for E. coli on days one, three, and five, and a diagnosis of vertebral osteomyelitis was made after an MRI of the lumbar spine. It has been reported that infectious dissemination to the vertebrae may occur through Bats…

Cutibacteria are gram-positive, non-sporulating, anaerobic, or microaerophilic bacilli that are increasingly recognized in the setting of indolent post-operative infection. Clinically significant infection with Cutibacterium avidum in the pediatric population is rarely encountered. Herein, we report our experience with two pediatric cases of osteomyelitis and soft tissue abscess after femoral derotational osteotomy for congenital hip dysplasia.

Rib osteomyelitis can be caused by a contiguous infection after a traumatic fracture. Post traumatic osteomyelitis can present as chronic six or more weeks after bone infection. However, this patient developed first rib osteomyelitis 17 years after trauma, following the initiation of anticoagulation therapy. 17 years ago, a 55-year-old male patient was in a motor vehicle collision. He was diagnosed with a left first rib fracture and an internal carotid dissection. He subsequently underwent a left subclavian central venous catheter placement. His rib fracture was managed nonoperatively and the …

Acute pyogenic vertebral osteomyelitis is mainly attributed to haematogenous spread of aerobic bacteria, while anaerobic osteomyelitis results from contiguous spread of polymicrobial infections through breaks in the gut mucosal barrier and involves the vertebral bodies in only about 2%-5%. Herein, we report two cases of vertebral osteomyelitis due to Bacteroides fragilis. It is noteworthy that cases of vertebral osteomyelitis due to Bacteroides fragilis have been attributed to the extension of intra-abdominal or pelvic floor infections. However, in the two cases described, there was no history…

CASE: We present an interesting case of femoral emphysematous osteomyelitis (EO) in a 67-year-old patient with the offending pathogen to be Parvimonas micra. Two knee arthrotomies with lateral thigh exploration were performed to debride and irrigate the bone and compartments. Although the patient initially improved, definitive treatment with an antibiotic intramedullary rod was necessary and resulted in complete resolution.
CONCLUSION: We present the first case of EO secondary to P. micra described in the literature and recommend aggressive surgical management for complete resolution and pres…

Chronic recurrent multifocal osteomyelitis (CRMO) is an auto-inflammatory disease characterised by sterile bone lesions. We report a case of a seven-year-old female patient who presented at a university hospital in 2010 and 2018 with CRMO. While the most promising results have been observed in patients under treatment with bisphosphonates (BPs), the initial decision to treat the current patient with a dose of zoledronic acid every six months was recalled as the patient developed tonic-clonic seizures immediately following the second dose BP administration. Following recall, the patient maintai…

Chronic recurrent multifocal osteomyelitis (CRMO) or also called chronic non-bacterial osteomyelitis (CNO) is the most common autoinflammatory bone disease. It is characterized by the presence of symptomatic and non-symptomatic bone lesions, mono or multifocal. The main sites involved are the metaphyses of the long bones, the bones of the pelvis, the vertebrae, the clavicle, the mandible. Local symptoms include pain, swelling, and warmth in the absence or presence of fever. The inflammatory process can involve the skin (palmoplantar pustulosis or acne) and the intestine. Diagnosis is complex a…
