INTRODUCTION: Infections with carbapenem-resistant enterobacteria are an emerging threat. This study reports the microbiologic, clinical, and epidemiologic features and the therapeutic outcomes of the infections caused by carbapenem- and pandrug-resistant Klebsiella emerged in our hospital. OBJECTIVE: We present our experience with the neuronavigation system used for surgery of distal anterior cerebral artery (DACA) aneurysms.
METHODS: Between 2001 and 2004, 12 patients with a DACA aneurysm were consecutively treated with direct clipping assisted by the neuronavigation system. We used the BrainLAB Vector Vision2 neuronavigation system (BrainLAB, Heimstetten, Germany). Seven out of 12 patients presented with subarachnoid hemorrhage. Aneurysms were located at distal A2 in 10 patients and distal A3 in two patients. The size of the aneurysms ranged from 3 to 10 mm.

A 47-year-old man presented with generalized pruritic lesions that had persisted for 2 years and shown no response to therapy with topical corticosteroids. The identification of numerous mites in the patient's scales led to the diagnosis of crusted scabies. The lesions responded to systemic therapy with Ivermectin (Stromectol) and topical application of Permethrin (InfectoScab). As underlying factors we suspect the immunosuppressive therapy attempts with topical corticosteroids as well as alcohol abuse.

Comprehensive genomic profiling (CGP) of a metastatic liver tumor biopsy specimen suggested that the patient, who was initially diagnosed with cholangiocarcinoma, had colorectal cancer. The identification of both FBXW7 and APC mutations is deemed characteristic of colorectal cancer. Indeed, subsequent colonoscopy revealed sigmoid colon carcinoma that led to tumor resection followed by systemic chemotherapy. CGP is principally used to identify agents that might potentially benefit the patient. However, results must be interpreted carefully to ensure consistency with the initial diagnosis.
