Cecal perforation presenting as abdominal-wall necrotizing fasciitis.

The preoperative diagnosis of a cecal perforation associated with Salmonella infection as a cause of abdominal-wall necrotizing fasciitis (AWNF) is clinically difficult. Computed tomography of the abdomen is helpful, and can detect the combined presence of a pneumoscrotum and pneumoperitoneum. Its presence indicates a patent processus vaginalis, which acts as the primary route for the spread of the intra-abdominal infectious process into the abdominal wall. An exploratory laparotomy should be done to confirm the presence of intra-abdominal pathology in order to avoid delayed treatment.

Laparoscopic-assisted colectomy in heart transplant recipients.

Reports of laparoscopy in heart graft recipients are scarce and, to our knowledge, laparoscopic colectomy has not yet been reported in heart transplant patients. The magnitude and the tolerance of the hemodynamic changes induced by pneumoperitoneum are unknown in heart graft recipients, who have a denervated heart and are "preload-dependent". The authors report the clinical courses of 2 heart graft recipients who developed acute diverticulitis without perforation or peritonitis and who underwent laparoscopic-assisted colectomy without complications.

Hyperkalaemia associated with prolonged insufflation of carbon dioxide into the peritoneal cavity.

We measured plasma potassium concentrations in five anaesthetized pigs over a 3.5 h period during which a carbon dioxide pneumoperitoneum was maintained. During this period the abdominal pressure was kept at 1.96 kPa and normocapnia was maintained by intermittent positive pressure ventilation. The potassium concentration was found to increase with time to a mean of 5.63 (SD 0.44) mmol litre-1. Consideration should be given to monitoring plasma potassium concentration in patients undergoing prolonged laparoscopic surgery.

Spontaneous gastric perforation in preterm low birthweight infants. Possible traumatic aetiology. A report of two cases.

Two cases of spontaneous gastric perforation in preterm low birthweight newborn infants are reported. The patients were referred within a short period from the same maternity hospital. They presented with gross abdominal distension of sudden onset. The abdominal X-rays showed typical saddle-bag configuration with massive pneumoperitoneum. The liver and spleen were displaced downwards and towards the midline. The nasogastric tubes used in the resuscitation may have played an important role in causing the perforations.

Gastrointestinal perforation by chicken bones.

Four cases of gastrointestinal perforation by chicken bones are presented. Variability in the clinical manifestations generally precludes a correct preoperative diagnosis of intraabdominal chicken bone abscess. However, in 2 patients, a diagnosis was possible with plain abdominal radiographs. Although the abnormalities seen were nonspecific, the identification of a chicken bone with an associated mass or extraluminal gas collection in a patient with signs of peritonitis, mechanical bowel obstruction, or pneumoperitoneum strongly suggests the diagnosis. A history of alcoholism or wearing dentur…

Effects of Low-Pressure CO2 Insufflation on Cerebral and Splanchnic Oxygenation in Neonates Undergoing Laparoscopic Pyloromyotomy.

Pathophysiological effects of abdominal CO2 insufflation on cerebral and splanchnic oxygenation in neonates and infants undergoing laparoscopy remain poorly investigated. We investigated laparoscopic pyloromyotomy as a paradigm to determine such changes in this specific population.Single-center, prospective cohort of 28 infants undergoing laparoscopic pyloromyotomy at the mean age of 30.9 ± 10.6 days. The pneumoperitoneum was set at 6 to 8 mmHg. Regional cerebral oxygen saturation (cSO2) and splanchnic oxygen saturation (sSO2) were measured by near-infrared spectroscopy. End-tidal carbon dioxi…

Effects of Peak Inspiratory Pressure-Guided Setting of Intracuff Pressure for Laryngeal Mask Airway Supreme™ Use during Laparoscopic Cholecystectomy: A Randomized Controlled Trial.

PURPOSE: To determine the effects of peak inspiratory pressure (PIP)-guided intracuff pressure (ICP) modulation of laryngeal mask airway (LMA) Supreme™ during laparoscopic cholecystectomy. METHODS: Totally 120 patients were randomly divided using computer-generated numbers into a control group (n = 60; ICP, 60 cmH2O) and a PIP group (n = 60), in which ICP was increased with 5 cmH2O each time from PIP level until no air leaks from the oropharynx. PIP, ICP, cuff volume (CV), oropharyngeal leak pressure (OLP) and leak fraction (LF) were recorded before and after pneumoperitoneum establishment. P…

Patient-engaged preoperative optimization for a complex incisional hernia: a case report.

Complex hernias with loss of domain present significant surgical challenges. Preoperative strategies such as botulinum toxin A (BTA) injections and progressive preoperative pneumoperitoneum (PPP) combined with structured prehabilitation and active patient participation may facilitate abdominal wall reconstruction. We report the case of a 65-year-old female with a complex incisional hernia. The patient underwent preoperative BTA injections followed by PPP before elective repair. This case highlights the value of a multimodal, patient-centered optimization pathway in facilitating successful abdo…

ECG changes with perforated duodenal ulcer mimicking acute cardiac ischemia.

A 38-year-old man presented to the emergency department with severe epigastric pain. An electrocardiogram (ECG) showed T wave inversions in leads V2 through V4 consistent with myocardial ischemia. The patient had undergone stress thallium testing and cardiac angiography several months prior for recurrent chest pain, and results from both studies were normal. Pneumoperitoneum seen on a chest X-ray prompted emergency laparotomy. A perforated duodenal ulcer was found and repaired. The ECG changes reverted to normal after surgery.

Gastric rupture during fiberoptic bronchoscopy, a rare complication of oxygen administration by nasopharyngeal cannula: a case report.

Concomitant use of a nasopharyngeal catheter is frequently used for oxygen supply during fiberoptic bronchoscopy (FOB). This is a procedure that presents possible complications that are not negligible. We demonstrate the case of a 61-year-old woman who underwent FOB due to a history of hemoptoic sputum. During the procedure, gastric rupture occurred with a large pneumoperitoneum and bilateral pneumothorax requiring immediate drainage of the air and an emergent laparotomy. This was probably a complication of the nasopharyngeal catheter. The knowledge of these complications is essential for thei…

A Case of Pneumatic Rectal Perforation Caused by Compressed Air.

Rectal perforation is a rare cause of acute abdomen. The most common cause of rectal perforation is trauma. Barotrauma due to the injection of compressed air in the rectum is an extremely rare cause of rectal perforation. We report a case of extensive pneumoperitoneum with abdominal compartment syndrome caused by rectal perforation secondary to the forceful injection of compressed air through the perineum. The patient was successfully managed by immediate relief of abdominal compartment syndrome by needle decompression followed by surgical repair of rectal perforation.

Hazards of laparoscopic adrenalectomy for Conn's adenoma. When enthusiasm turns to tragedy.

A 74-year-old man with primary aldosteronism had a small tumor (27 x 23 mm) of his right adrenal gland successfully removed by a transperitoneal laparoscopy. Despite absence of malignancy in the resected tumor and complete relief of all symptoms in the immediate postoperative period, recurrence occurred 6 months later. The tumor behaved as a carcinoma spread in the peritoneal cavity, and the patient eventually died with peritoneal carcinomatosis. We suggest that the laparoscopic technique coupled with pneumoperitoneum may have favored this recurrence.

Small bowel ischemia following laparoscopic cholecystectomy.

BACKGROUND: Among a myriad of physiological adverse affects of pneumoperitoneum-associated intra-abdominal hypertension, compromise of the mesenteric circulation is well documented. METHODS: After experiencing a case of fatal small bowel ischemia in the aftermath of laparoscopic cholecystectomy, the literature was reviewed. RESULTS: A Medline and Index Medicus search revealed at least 6 cases of small bowel ischemia following laparoscopic cholecystectomy. CONCLUSIONS: Mesenteric ischemia should be considered in the differential diagnosis of patients developing nonspecific abdominal symptoms…

Primarily Gasless Robot-Assisted Cholecystectomy in Dogs: A Cadaveric Feasibility Study.

Gallbladder disease is a common cause of morbidity in dogs, and cholecystectomy remains the definitive treatment in many cases. Although minimally invasive approaches offer recognized advantages, their adoption is limited by technical complexity and by the physiological effects of carbon dioxide (CO₂) pneumoperitoneum. This cadaveric study evaluated the feasibility of performing primarily gasless robot-assisted cholecystectomy under abdominal wall suspension in five canine cadavers. A normobaric operative field was established using a suspension device combined with the VersiusTM robotic platf…

Modification of the modified Graham patch repair for duodenal perforation using the gastrocolic ligament: Two case reports.

INTRODUCTION: Gastroduodenal perforations are relatively common surgical emergencies with mortality rates as high as 40%. The Graham patch repair is one surgical approach but may need to be modified when the patient lacks viable omentum. The gastrocolic ligament can be utilized to repair these perforations for coverage. PRESENTATION OF CASES: Case 1: A 77-year-old female with a complex history presented with severe abdominal pain and was found to have pneumoperitoneum on CT scan. She was found to have a first segment duodenal perforation. We employed a modified Graham patch omentopexy utilizi…

Review of robot-assisted laparoscopic surgery in management of infant congenital urology: Advances and limitations in utilization and learning.

As robotic-assisted (RAL) surgery expanded to treat pediatric congenital disease, infant anatomy and physiology posed unique challenges that … vely. Furthermore, end-tidal CO2 (EtCO2 ) is important for monitoring and predicting the effects of pneumoperitoneum on caridiovascular physiology, incranial pressure, and risk of acidosis and hypercarbia. Positioning can further exacerbate these effects and affect intra-abdominal working space. For infant robotic pyeloplasty and heminephrectomy, a "beanbag" is co …

Diagnosing penetrating diaphragmatic injuries: CT scan is valuable but not reliable.

BACKGROUND: The diagnosis of penetrating isolated diaphragmatic injuries can be challenging because they are usually asymptomatic. Diagnosis by chest X-ray (CXR) is unreliable, while CT scan is reported to be more valuable. This study evaluated the diagnostic ability of CXR and CT in patients with proven DI. METHODS: Single center retrospective study (2009-2019), including all patients with penetrating diaphragmatic injuries (pDI) documented at laparotomy or laparoscopy with preoperative CXR and/or CT evaluation. Imaging findings included hemo/pneumothorax, hemoperitoneum, pneumoperitoneum, e…

First Report of Two Cases of Acute Gastric Ischemia after Robot-Assisted Radical Cystectomy.

Gastrointestinal ischemia is rare after small pelvis surgery. Minimal invasive robotic surgery requires adaptation of the surgical approach for cystectomy and derivation construction such as the use of pneumoperitoneum and Trendelenburg positioning of the patient. Two cases with gastric ischemic complications after robot-assisted radical cystectomy are described. The first case was a 68-year-old female who had prolonged gastroparalysis and blood in a replaced gastric tube at day 10 after robotic cystectomy and Bricker urinary derivation. Gastroscopy revealed ischemia of gastric and proximal du…
