Evolution in reimbursement for sleep studies and sleep centers.

Because of the rapid increase in the volume and costs of polysomnography and other sleep medicine diagnostic services, the Centers for Medicare & Medicaid Services (CMS) recently commissioned the Office of Inspector General (OIG) to review claims submitted for these services. The OIG found numerous cases of inappropriate payment for submitted claims and recommended significant changes in the CMS auditing process for polysomnography claims review. Additionally, a local Medicare Administrative Contractor released the most specific rules and regulations to date regarding billing and payment for s…

The relationship between excessive daytime sleepiness and metabolic syndrome in severe obstructive sleep apnea syndrome.

BACKGROUND: Excessive daytime sleepiness (EDS), which is commonly considered a cardinal sign of obstructive sleep apnea (OSA), may lead to an increased rate of metabolic syndrome (MetS), and be an independent risk factor for cardiovascular morbidity and mortality. The aim of this cross-sectional study was to examine the role of EDS in MetS and its components by researching severe OSA patients. METHODS: The records of 175 consecutive patients who underwent standard polysomnography and diagnosed severe OSA were included. Subjective daytime sleepiness was assessed using the Epworth sleepiness sc…

Correlation of daytime sleepiness with urine metabolites in patients with obstructive sleep apnea.

PURPOSE: The apnea-hypopnea index (AHI) is closely related with the severity of daytime sleepiness, but excessive daytime sleepiness (EDS) is not presented on all patients with obstructive sleep apnea (OSA). It is unclear why daytime sleepiness is not always present in OSA patients even if their sleep is disrupted from the perspective of polysomnographic findings. This study aimed to analyze the correlation between sleepiness and urine metabolites of neurotransmitters involved in the arousal system. METHODS: On the basis of AHI in polysomnography, 49 consecutive OSA patients were included. Ac…

Late-onset congenital central hypoventilation syndrome and a rare PHOX2B gene mutation.

INTRODUCTION: Congenital central hypoventilation syndrome (CCHS) is a rare lifelong disorder characterized by an abnormal ventilatory respon … 1 of the PHOX2B gene). The parents' DNA showed no PHOX2B mutations. Hypoventilation was observed by polysomnography, with no autonomic response to declining oxygen or increasing carbon dioxide values. A subsequent sleep study showed less hypoxia and hypercapnia. The patient has been on non-invasive ventilation during sleep, showing good growth and neurocognitive …

Correlation between sleep parameters, physical activity and quality of life in somnolent moderate to severe obstructive sleep apnea adult patients.

PURPOSE: Several symptoms are described in obstructive sleep apnea (OSA), including excessive daytime sleepiness, depressive mood and neurocognitive dysfunction. However, few studies examined the relationship between objective physical activity, quality of life and sleep parameters. The purpose of this study was to determine sleep parameters (evaluated by actigraphy and polysomnography) implied in the occurrence of impaired physical activity and reduced quality of life in somnolent moderate to severe OSA patients. METHODS: We reviewed retrospectively data of 75 somnolent moderate to severe (A…

Asleep home blood pressure monitoring in obstructive sleep apnea: a pilot study.

BACKGROUND: Obstructive sleep apnea (OSA) is known to be related to nocturnal blood pressure (BP) and nondipping hypertension. This pilot study evaluated night-time BP assessed using a home monitor in patients with OSA. PARTICIPANTS AND METHODS: Patients referred to a sleep clinic were subjected to polysomnography, clinic BP measurements, and home BP monitoring using a device that allows daytime (3 days, two duplicate readings per day) and automated night-time BP measurement (3 nights, three readings per night). RESULTS: Thirty-nine patients were included [72% men, mean age 48.7±10.8 years, …

Feasibility of noninvasive single-channel automated differentiation of obstructive and central hypopneas with nasal airflow.

BACKGROUND: The identification of obstructive and central hypopneas is considered challenging in clinical practice. Presently, obstructive and central hypopneas are usually not differentiated or scores lack reliability due to the technical limitations of standard polysomnography. Esophageal pressure measurement is the gold-standard for identifying these events but its invasiveness deters its usage in daily practice. OBJECTIVES: To determine the feasibility and efficacy of an automatic noninvasive analysis method for the differentiation of obstructive and central hypopneas based solely on a si…

Obstructive sleep apnea among express bus drivers in Malaysia: important indicators for screening.

OBJECTIVES: Obstructive sleep apnea (OSA) has been identified as one of the significant risk factors for motor vehicle crashes (MVCs). In the interest of public safety, this study was conducted to determine the prevalence of OSA and its associated factors among express bus drivers in Malaysia. Identifying factors or conditions related with OSA is very important because they can be used as indicators to subject a person to a confirmatory diagnosis using polysomnography testing. METHODS: Two hundred eighty-nine randomly selected express bus drivers from 5 express bus companies participated in t…

Impact of kidney transplantation on sleep apnoea in patients with end-stage renal disease.

BACKGROUND: Sleep apnoea is common in patients with end-stage renal disease. Although individual case reports have described an improvement in sleep apnoea following kidney transplantation, there have been no longitudinal studies of a case series to determine what proportion of patients with sleep apnoea improve. METHODS: Dialysis-dependent patients awaiting kidney transplantation and pre-dialysis patients with an identified living donor kidney had overnight polysomnography, which was repeated several months after successful kidney transplantation. Patients were divided into apnoeic and non-a…

Sleep related rhythmic movement disorder revisited.

To classify sleep related rhythmic movement disorder (SRMD) based on clinical, polysomnographic and videometric evaluation in a predominantly adult population, twenty-four patients (four females) aged 11-67 years identified by polysomnography and videometry were classified for type of SRMD, its duration and frequency during wakefulness and in the different sleep stages. SRMD persisted unto child- and adulthood in all patients. SRMD is not restricted to sleep-wake transition, occurs most frequently in wake, stages NREM 1 and 2, but also in REM and slow wave sleep. Most patients have one form of…
