The inherent variability of blood pressure means that the accuracy of a small number of clinic readings in estimating the true blood pressure is limited. In many patients, a clinic visit provokes an increase in blood pressure such that they may be misclassified as being hypertensive (white coat hypertension). Ambulatory blood pressure monitoring essentially estimates the average value of systolic and diastolic pressure from readings that fluctuate with mental and physical activity throughout the day. The improved reproducibility of blood pressure measurement achieved in this way increases the precision of clinical trials and/or reduces the number of subjects needed in a trial. Amnioinfusion has become a common intrapartum procedure for the relief of cord compression. We report a case of acutely increased intrauterine pressure during amnioinfusion with maternal but not fetal distress. An important decrease in pressure and symptomatology occurred with withdrawal of only a small amount of fluid. Further study is needed of pressure-volume relationships in the uterus and effects on uteroplacental perfusion. Careful attention must be paid to infusion volumes and intrauterine pressure during amnioinfusion.

Twenty-two hypertensive patients were monitored during two separate drug-free occasions with a Del Mar Avionics ambulatory device. Blood pressure loads (percentage of systolic and diastolic readings more than 140 and 90 mmHg, respectively) and mean BP were measured both to determine their reproducibility and to examine how they correlate with each other. The effect of some anesthetic drugs on intraocular pressure (IOP) was studied in 120 normal healthy patients undergoing non-ophthalmic surgical procedures. IOP rose significantly following the injection of succinylcholine (SCh) alone, or when such injection had been preceded by a pretreatment with a "self-taming" dose of SCh or d-tubocurarine (d-Tc). The authors examined ten seropositive rheumatoid arthritis patients with an EMED gait analysis system in a mean four years after foot surgery and compared that with ten normal subjects who formed a control group. The maximum pressure in the toe regions was almost the same as in the control group. The rheumatoid arthritis group had significantly increased maximum pressure in the first metatarsal and tarsal region.

A 69 year old man presented with headache on arising and relieved with recumbency. His clinical presentation, laboratory data, response to treatment and radiographic evaluation were consistent with spontaneous low cerebrospinal fluid pressure from CSF hyperabsorption. Cisternography showed rapid accumulation in the kidneys and urinary bladder, without evidence of CSF leak. His headache was eventually controlled with two blood patches and oral caffeine.

We report the incidence of pressure sores in patients operated on for hip fracture or with total hip replacement. Pressure sores were found in 30 and 4 per cent, respectively. Half of the sores appeared within 1 week postoperatively, and more than half healed in the hospital. Most of the sores were found in patients over 80 years of age. No relation was found between pressure sores and smoking, diabetes mellitus, low hemoglobin, and poor state of nutrition. Patients with pressure sores stayed in the hospital longer and were often discharged to nursing homes.
