Uncovering the "skeleton in the closet": the issue of bone and joint disorders in the Maldives and the opportunities for primary prevention and health promotion.

Disorders of bones and joints are the most common cause of severe long-term pain and disability around the world yet receive inadequate attention in many countries. Bone/joint health is absent from current health policy and the primary prevention/health promotion agenda in Maldives and diagnostic and curative infrastructure is very limited. In 2004, tentative evidence emerged indicating that degenerative bone and joint conditions may impose a large burden on community health. A study was undertaken amongst a representative community sample of women aged 15-50 to further investigate the issue. …

Secondary analysis of primary and preventive services accessed and perceived service barriers by children with developmental disabilities and their families.

Children and youths with developmental disabilities (DD) have needs for more community-based services to address ongoing health, educational, employment, housing, transportation, and recreational concerns. Secondary data analysis was conducted to examine the array of services accessed and service obstacles to primary, preventive and special health care services of 102 children and youths with developmental disabilities. The two services most frequently reported as being used by families were SSI (29.4%) and MediCal (California's Medicaid program) (27.8%). Speech therapy was the most frequently

Pain prediction for preventive non-operative management of dentinal caries in primary teeth in general dental practice.

OBJECTIVES: To provide a pain-predictive model for the non-operative management of carious deciduous teeth from the analysis of data from a retrospective analysis of clinical case notes of children regularly attending two general dental practices and receiving preventive care. DESIGN: A clearly defined protocol was used to determine the fate of deciduous teeth diagnosed as carious into dentine but symptomless and left unrestored from the sequential examination of the clinical records of 480 children attending at least annually. RESULTS: The age of the children at the first visit when carious

Poxvirus-encoded serpins do not prevent cytolytic T cell-mediated recovery from primary infections.

Previous observations that the highly conserved poxvirus-encoded serpins inhibit cytotoxic activities of alloreactive CTL via granule and/or Fas-mediated pathways was taken to indicate their involvement in immune evasion by poxviruses. We now show that interference with 51Cr release from target cells by ectromelia and cowpoxvirus is limited to alloreactive but not MHC-restricted CTL. The data are in support of the paramount importance of CTL and its effector molecule perforin in the recovery from primary ectromelia virus infection and question the role of serpins in the evasion of poxviruses f

Identification of children with fetal alcohol syndrome and opportunity for referral of their mothers for primary prevention--Washington, 1993-1997.

Heavy maternal use of alcohol during pregnancy can cause permanent birth defects, including fetal alcohol syndrome (FAS). Although these alcohol-related defects are entirely preventable, the factors associated with maternal use of alcohol during pregnancy are complex and often resistant to change. In addition, not all women who drink heavily will produce children with FAS. Although targeting primary prevention efforts to all women at risk for drinking during pregnancy is ideal, limited resources require targeting women at the highest risk for producing children affected by prenatal alcohol exp…

Prevention, early detection and team management of skin cancer in primary care: contribution to The health of the nation objectives.

The incidence of all skin cancers is increasing. If The health of the nation targets are to be addressed, incidence figures need to be more accurate. Solar damage is the major causal factor in all skin cancers. Certain individual risk factors also play an important part, especially in the development of malignant melanoma. Prevention and early detection are crucial in reducing morbidity and mortality from skin cancer. This paper considers the role of primary care skin screening clinics and cutaneous surgery facilities in the early detection and management of skin cancer. It also illustrates th

Genetic risk stratification and preventive strategies for double primary HNSCC and ESCC: a single-center cohort study.

BACKGROUND: Head and neck squamous cell carcinoma (HNSCC) and esophageal SCC (ESCC) share risk factors, such as alcohol consumption, smoking, and aldehyde dehydrogenase 2 (ALDH2) gene polymorphisms. However, the involvement of these factors in the occurrence of double primary cancers remains unclear. This study aimed to examine the risk factors for double cancers. METHODS: This single-center study analyzed 113 patients with HNSCC and ESCC diagnosed between 2014 and 2022, classified into four stage-based groups: Group A (early ESCC + early HNSCC), Group B (early ESCC + advanced HNSCC), Group C

Preventing maternal deaths in Somalia requires stronger primary health care and functional emergency referral systems.

Maternal mortality remains a critical public health crisis in Somalia, driven by structural vulnerabilities, low utilization of essential maternity care, and fragmented health service delivery. The vast majority of maternal deaths result from preventable obstetric complications such as hemorrhage, hypertensive disorders, sepsis, and obstructed labor. Current national surveys demonstrate severe gaps across the continuum of maternal care, particularly among rural, pastoralist, and internally displaced populations. Reducing preventable maternal deaths requires moving away from isolated emergency

Association of primary care telehealth with preventable healthcare utilization and care continuity among patients with chronic conditions.

Telehealth may help maintain access to care for individuals with chronic conditions, particularly during periods of disruption. This study examined the association between telehealth use and health care utilization, continuity, and quality in primary care. We used a difference-in-differences design (2019-2021) to compare outcomes for patients attributed to practices with high versus low telehealth uptake before and after the onset of the COVID-19 pandemic across three clinical research networks. Outcomes included evaluation and management visits, preventable emergency department visits, avoida

Spatiotemporal Analysis of the Primary Factors and Risk Quantification of Human Brucellosis Prevention and Control in Zhejiang Province of China.

INTRODUCTION: Brucellosis is a major zoonotic disease that poses significant public health and socio-economic threats. In China, human brucellosis incidence has continued to rise, including in highly developed regions such as Zhejiang Province, despite relatively low levels of livestock husbandry. This study investigates the recent surge in human brucellosis in Zhejiang, aiming to identify key risk factors and inform prevention strategies. METHODS: We collected case data from 11 municipal divisions of Zhejiang Province (2018-2023). To overcome limitations of conventional clustering methods, w

Pollen-Based Artificial Enzyme With Efficient Biocatalysis and Immune-Priming to Prevent Primary and Secondary Wound Infection.

Bacterial infection poses a significant threat to clinical treatment due to the emergence of drug resistance and the high risk of recurrence. Here, we report the de novo design of a spiky pollen-based vanadium artificial enzyme particle (VAE-Pollen) that integrates potent reactive oxygen species (ROS)-catalytic activity with immune priming to prevent both primary and secondary bacterial infections. Experimental and theoretical analyses confirm that bacteria are efficiently captured by the micro-structured surface of VAE-Pollen, and the introduction of oxygen vacancies modulates the electronic

Association of lipoprotein(a) and diabetes in primary prevention of coronary heart disease: The Multi-Ethnic Study of Atherosclerosis (MESA).

BACKGROUND AND AIMS: Lipoprotein(a) [Lp(a)] and diabetes are both independently associated with cardiovascular disease. Studies demonstrate that elevated Lp(a) is associated with increased incidence of cardiovascular disease in those with and without diabetes. However, it is unclear if Lp(a) modifies the association of diabetes and coronary heart disease (CHD) in primary prevention. METHODS: The present analysis included 6668 participants from the Multi-Ethnic Study of Atherosclerosis, a community-based cohort without clinical cardiovascular disease at baseline. Participants were categorized …

Efficacy of Intensive Lifestyle Education and Forest-Based Exercise for the Primary Prevention of the Risk of Coronary Artery Disease.

INTRODUCTION: Despite its efficacy, conventional center-based cardiac rehabilitation has several limitations which have led to the emergence of home-based programs and intensive cardiac rehabilitation as alternative methods for overcoming these limitations. Alternative methods for primary prevention have been recommended for similar reasons. Lifestyle modification is considered key to success in both primary and secondary prevention. Therefore, this primary prevention study aimed to investigate the efficiency of intensive lifestyle education and home-based programs involving unstructured exerc…

Preventive Antibiotic Use and Complications After Endoscopic Retrograde Cholangiopancreatography in Patients Hospitalized for Primary Sclerosing Cholangitis.

BACKGROUND: The American Society for Gastrointestinal Endoscopy recommends prophylactic antibiotics before endoscopic retrograde cholangiopancreatography (ERCP) in primary sclerosing cholangitis (PSC). We assessed the impact of this approach on the incidence of post-ERCP outcomes using nationwide data. METHODS: Using 2015-2021 Nationwide Inpatient Sample data and relevant ICD-10 codes, we analyzed adult hospitalizations for PSC who underwent ERCP, with and without antibiotic prophylaxis. Hierarchical multivariate logistic regression analysis was used to assess the association between prophyla

The Current State of Alcohol Screening and Management in Virginia Primary Care Practices: An Evaluation of Preventive Service Use.

The US Preventive Services Task Force (USPSTF) recommends screening and behavioral counseling for adults over 18 years for unhealthy alcohol use. Recommended screening instruments include the Alcohol Use Disorders Identification Test-Concise and or Single Alcohol Screening Question. Behavioral counseling is feasible in primary care, taking on average 30 minutes. Baseline data for a practice facilitation trial demonstrated clinicians appropriately screened only 10.8% of patients and only identified 9.6% as having risky drinking. Yet, 24% of patients reported risky drinking on a survey, demonstr
