Ultrasound-Guided Peripheral Nerve Blocks for Shoulder Dislocation in the Emergency Department: A Systemic Review.

BACKGROUND: Shoulder dislocations are among the most common orthopedic emergencies encountered in the emergency department (ED). Ultrasound- … udies met inclusion criteria, all of which were randomized controlled trials comparing USG-PNB with procedural sedation and analgesia. Pooled patient satisfaction scores were similar for both analgesia methods (3.5 ± 0.6 vs. 3.9 ± 0.6 out of 5; p = 0.76). Patients managed with procedural sedation and analgesia achieved higher rates of overall shoulder reduction (100% …

A thin cryobiopsy device compatible with transnasal endoscopy for the gastrointestinal tract.

Luminal organ biopsies are critical for disease diagnosis and are obtained using single-bite forceps inserted through the working channel of large endoscopes. Procedures using these endoscopes frequently require patient sedation or anesthesia and may not be feasible for use in pediatric patients. Additionally, forceps-derived biopsies can suffer from difficulty maintaining tissue orientation, crush artifacts, and lack of precise control of biopsy depth. The high cost and risks of anesthesia and sedation have driven the development of smaller endoscopes for unsedated procedures. However, reduce

Quality of conscious sedation using dexmedetomidine during full-endoscopic transforaminal discectomy for sciatica: a prospective case series.

INTRODUCTION: Percutaneous transforaminal endoscopic discectomy (PTED) is increasing in popularity as a minimally invasive procedure to treat sciatica caused by lumbar disc herniation. The objective of the current study is to evaluate safety of and satisfaction with the use of local anesthesia and conscious sedation during PTED. METHODS: During a 12-month inclusion period, patients were prospectively included in this single center case series. Inclusion criteria consisted of sciatica lasting for at least 6 weeks, which was not responsive to conservative treatment. PTED was performed using dex

Propofol target-controlled infusion for sedated gastrointestinal endoscopy: A comparison of propofol alone versus propofol-fentanyl-midazolam.

Gastrointestinal (GI) endoscopy is the major technique for diagnosis of GI disease and treatment. Various sedation and analgesia regimens such as midazolam, fentanyl, and propofol can be used during GI endoscopy. The purpose of the study was to compare propofol alone and propofol combination with midazolam and fentanyl in moderate sedation for GI endoscopy. One hundred patients undergoing GI endoscopy were enrolled in this study. All patients received a propofol target-controlled infusion (TCI) to maintain sedation during the procedure. Patients were randomly allocated into either Group P (pro

Using intranasal midazolam spray to prevent claustrophobia induced by MR imaging.

OBJECTIVE: Up to 37% of patients undergoing MR imaging examinations experience moderate to severe levels of anxiety that necessitate the termination of the procedure in 5-10% of patients. Although the clinical use of MR imaging has increased, effective procedures to handle claustrophobia are lacking. We evaluated the effectiveness of intranasally administered midazolam spray in preventing claustrophobic responses of patients undergoing MR imaging. SUBJECTS AND METHODS: Fifty-four patients scheduled for MR imaging were included in this prospective study. Anxiety and sedation of patients were e

Bilateral Costoclavicular Brachial Plexus Block for Closed Reduction of Bilateral Elbow Dislocation: A Case Report and Review of the Literature.

Bilateral brachial plexus block is approached cautiously because of concerns regarding local anesthetic systemic toxicity, respiratory compromise, and bilateral upper-limb motor block. We describe a 60-year-old man with bilateral posterolateral elbow dislocation after a road traffic accident who underwent closed reduction and cast application under bilateral ultrasound-guided costoclavicular brachial plexus block using 10 mL of local anesthetic on each side. The procedure was completed without general anesthesia or deep sedation. This case highlights that low-volume bilateral costoclavicular b

Code Malignant Hyperthermia: Malignant Hyperthermia in the Emergency Department-Implications for Nursing.

Malignant hyperthermia is a rare, life-threatening hypermetabolic reaction that can strike unexpectedly in the emergency department, demanding swift recognition and intervention to save lives. This syndrome can be fatal if not treated and occurrs in 1 in 100,000 adults and 1 in 30,000 children, with a 3% to 5% mortality rate. Although often associated with perioperative environments, malignant hyperthermia should remain on the radar for emergency nurses, especially when patients exhibit sudden hyperthermia, muscle rigidity, and tachycardia after intubation or procedural sedation. This article …

Gastric rupture following nasopharyngeal catheter oxygen delivery-a report of two cases.

Iatrogenic gastric distension and subsequent rupture following nasal or nasopharyngeal catheter oxygen delivery is a rare but life-threatening condition that requires urgent laparotomy. We report two cases recently encountered at our institution. Both patients exhibited symptoms of abdominal pain and distension following oxygen delivery involving a nasopharyngeal catheter during procedural sedation. Oxygen flow rates were 4 l/minute in both cases. The diagnosis was made by urgent imaging. Both patients survived following laparotomy and repair of gastric rupture. Seventeen cases have been repor…

Design considerations of a randomized controlled trial of sedation level during hip fracture repair surgery: a strategy to reduce the incidence of postoperative delirium in elderly patients.

Background Delirium is an acute change in mental status characterized by sudden onset, fluctuating course, inattention, disorganized thinking, and abnormal level of consciousness. The objective of the randomized controlled trial "A STrategy to Reduce the Incidence of Postoperative Delirium in Elderly Patients" (STRIDE) is to assess the effectiveness of light versus heavy sedation on delirium and other outcomes in elderly patients undergoing hip fracture repair surgery. Our goal is to describe the design considerations and lessons learned in planning and implementing the STRIDE trial. Methods D

When Safety Promotion Impairs Access to Care: Lessons From Endoscopy Across Canada.

An informal review of existing conscious sedation policies across Canada shows that, following endoscopic procedures, many sites require that a patient be accompanied home and cared for by a support person. Though well-intended, these policies may act as a barrier to care for vulnerable persons or result in unnecessary hospital admissions. We argue that equity requires more flexibility, creativity and risk tolerance in the application or interpretation of safety-related rules in particular situations. Unexamined common practices can sometimes pull against our responsibility to promote greater

[Cardiopulmonary resuscitation-induced consciousness (CPRIC) : Consciousness in cardiopulmonary resuscitation: intra-arrest sedation and immediate extubation with spontaneous circulation].

The term cardiopulmonary resuscitation-induced consciousness (CPRIC) describes the presence of "awareness" or "consciousness" during CPR, i.e. the at least partially conscious (co)experience of one's own resuscitation measures. Depending on the circumstances surrounding the resuscitation, including the medication administered, this may or may not be accompanied by a later memory of the event (= awareness of CPR). In the literature it is unanimously criticized that this is a little-noticed phenomenon and that there are as yet no generally accepted guidelines for dealing with it with or without

Airway foreign body caused by pepper inhalation 7 years previously retrieved under conscious sedation with spontaneous respiration: a case report.

A 26-year-old man who had inhaled a dried pepper 7 years previously was admitted to our hospital for repeated coughing with yellow sputum and occasional hemoptysis. A thoracic high-resolution computed tomography scan revealed a foreign body at the proximal end of the right lower bronchus. We attempted to remove the foreign body by flexible bronchoscopy, but this was unsuccessful because the foreign body fell deeper into the bronchus. After a multidisciplinary team meeting, the foreign body was successfully extracted by bronchoscope suction and forceps under conscious sedation with spontaneous
