Ceftobiprole: a potential empirical post-operative monotherapy in prosthetic joint infections.

BACKGROUND: This study aimed to evaluate in vitro susceptibility to ceftobiprole of clinical strains identified from prosthetic joint infections (PJIs) compared to that of the associations currently recommended for post-operative empirical antibiotic therapy (PEAT) (vancomycin with either cefepime, third-generation cephalosporin or piperacillin-tazobactam). METHODS: We performed a 1-year retrospective study on all the surgical procedures performed in our hospital for PJI. Susceptibility profiles of all the strains cultured from surgical samples were reviewed to compare ceftobiprole to current

[Implantation of bipolar prosthesis for treatment of medial femoral neck fractures in the elderly--clinical and radiographic outcome].

In our study, we retrospectively evaluated 203 patients with medial or lateral femoral neck fractures who received 210 bipolar prosthesis (7 bilateral implantations) at our institution. 204 patients underwent cemented bipolar hemiarthroplasty. These included 149 females and 54 males with a median age of 82 (46-97) years. At the time of surgery 144 patients (71%) suffered from more than 3 medical conditions. We conducted our follow-up examination at a median of 27.9 months after the operation, at which time 97 patients (48%) had already died. On examination, 17% of our 76 patients had a Harris

Intra-abdominal aortic graft infection: prognostic factors associated with in-hospital mortality.

BACKGROUND: Mortality associated with aortic graft infection is considerable. The gold standard for surgical treatment remains explantation of the graft. However, prognostic factors associated with early mortality due to this surgical procedure are not well-known. METHODS: Retrospective analysis of patients admitted in our center between January 2006 and October 2011 for aortic graft infection. The primary endpoint was in-hospital mortality. A bivariate analysis of characteristics of patients associated with in-hospital outcome was performed. RESULTS: Twenty five evaluable patients were stud

Dacron carotid patch infection: a report of eight cases.

From 1995 through 1998, we encountered eight patients with infected Dacron patches after previous carotid endarterectomy. Two of the original operations had been done elsewhere, but the six patients who were collected from our own series represented 0.5% of the 1258 carotid endarterectomies we performed and 1.8% of the 340 synthetic carotid patches we applied without any comparable infections among another 918 patients who received either vein patch angioplasty (n = 843) or primary arteriotomy closure (n = 74) during the same 4-year study period. With a single exception ("no growth"), bacteria

Revolutionizing care: Recent innovations and the future of treating implant-induced infections and inflammation.

Medical implants are crucial for restoring function and regenerating injured organs and tissues. However, beyond design, material properties and placement, the risk of a foreign body reaction (FBR) upon interaction between the host immune system and the biomaterial, as well as the risk of infection at the implant site significantly influence treatment success. FBR has been confirmed to severely affect the integration of medical implants into the host tissue, often leading to therapy failure. This has led to the engineering of several novel biomaterials to improve the biocompatibility and integ

Macrophage activation syndrome triggered by Candida parapsilosis-related cochlear implant infection.

BACKGROUND: Cochlear implant-related infections may rarely lead to life-threatening systemic complications. Macrophage Activation Syndrome (MAS) is a severe and potentially fatal hyperinflammatory condition. MAS associated with CI infection has not been previously reported in pediatric patients. CASE PRESENTATION: We present a 13-year-old female patient who developed MAS secondary to Candida parapsilosis-related sepsis associated with cochlear implant (CI). Despite broad-spectrum antibacterial and antifungal therapy, the patient's clinical course deteriorated with pancytopenia, hyperferritine

Infections of Selected Nonvalvular Cardiovascular Devices: A Science Advisory From the American Heart Association.

Nonvalvular cardiovascular device infections, although rare in most cases, represent substantial clinical challenges due to their life-threatening potential and diagnostic complexity. Although infections associated with prosthetic valves, cardiovascular implantable electronic devices, vascular grafts, and mechanical circulatory support devices have garnered considerable attention and specific guidance, infections of other nonvalvular cardiovascular devices have not. Therefore, this Science Advisory focuses on a contemporary profile of other nonvalvular device infections and includes an algorit

Efficacy of Commercially Available Irrigation Solutions on Removal of Biofilms Grown on Porous Titanium Implants: An In Vitro Study.

BACKGROUND: Periprosthetic joint infection (PJI) remains a major complication in orthopaedic surgery. A challenge in treating PJI is bacterial biofilm formation on implants, which confers a barrier to conventional antibiotic therapies and often necessitates implant removal and revision surgery. The goal of this study was to compare the efficacy of six commercially available irrigation solutions in decreasing bacterial load and disrupting biofilm formed on three dimensionally printed porous titanium discs that are similar to the surface characteristics of TJA implants. METHODS: An in vitro mod

Safety and effectiveness of intraosseous regional prophylactic antibiotics in total knee arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Intraosseous regional administration (IORA) as a widely applicable and clinically valuable route of administration has gained significant attention in the context of total knee arthroplasty (TKA) for the prophylactic administration of antibiotics. However, there is still controversy regarding its effectiveness and safety. The latest meta-analysis reports that the use of IORA for antibiotics in TKA is as safe and effective as IV administration in preventing prosthetic joint infection (PJI), but they did not separate the statistics for primary TKA and revision TKA, which may be inapp

[Is there an indication for endocarditis prophylaxis in patients with cardiac devices?].

Cardiac implantable electronic devices (CIED) are essential in treating cardiovascular disease, but their infections are serious complications. The number of patients with CIEDs is increasing, as are CIED infection rates and related endocarditis. These infections arise either from contamination during implantation or from secondary infections. Current guidelines do not support prophylactic antibiotics for CIED patients before dental or other invasive procedures, unless the patient has other risk factors (E.g. an artificial heart valve). Patients with fever or signs of a pocket infection should

Effect of Antibiotic Spacer Dosing on Treatment Success in Two-Stage Exchange for Periprosthetic Joint Infection.

INTRODUCTION: In two-stage exchange for periprosthetic joint infection (PJI), adding antibiotics to cement spacers is the standard of care; however, little is known about optimal dosage. There is emphasis on using >3.6 g of total antibiotic, including ≥2.0 g of vancomycin, per 40 g of cement, but these recommendations lack clinical evidence. We examined whether recommended antibiotic spacer doses affect treatment success. METHODS: This was a retrospective review of 202 patients who underwent two-stage exchange for PJI from 2004 to 2020 with at least 1-year follow-up. Patients were separated i

Prosthetic knee joint infection caused by α-hemolytic Streptococcus species: a case report.

BACKGROUND: Knee arthroplasty is an orthopedic surgical procedure in which a damaged joint is replaced with an artificial one. It is estimated that 1-2% of knee arthroplasties will encounter infection over their lifetime. Although α-hemolytic Streptococcus species play an important role in prosthetic joint infection, they are less common than staphylococcal species. CASE PRESENTATION: In this report, a 50-year-old Iranian woman was diagnosed with prosthetic knee joint infection based on clinical, radiological, and laboratory findings. She was diabetic and had undergone a left total knee arthr

Diagnostic and Invasive Colonoscopy Are Not Risk Factors for Revision Surgery Due to Periprosthetic Joint Infection.

BACKGROUND: Colonoscopy is routinely performed for colorectal cancer screening in patients who have a preexisting unicompartmental knee arthroplasty (UKA), total knee arthroplasty (TKA), or total hip arthroplasty (THA) prostheses. However, colonoscopy is theorized to provoke transient bacteremia, providing a potential nidus for periprosthetic joint infection. This study aimed to investigate the risk of aseptic and septic revision surgery in patients who underwent diagnostic colonoscopy or invasive colonoscopy within one year following UKA, TKA, or THA. METHODS: A retrospective cohort analysis
