We report on two patients diagnosed as having active pulmonary tuberculosis who later developed lung cancer. In both cases, the lung cancer was detected during the treatment of pulmonary tuberculosis. Both patients were initially considered to be experiencing exacerbation of pulmonary tuberculosis. Case 1 was seen in a 74-year-old man. His chest roentgenogram revealed microscopic cavitary lesions with infiltration into both lung fields. His sputum tested positive for acid-fast bacilli. Although he was treated with isoniazid (INH), rifampicin (RFP), ethambutol (EB) and pyrazinamide (PZA), his g…

OBJECTIVE: To report our therapeutic experience with tuberculosis of the lower cervical spine with neurological complications in 9 adults in Gabon.
PATIENTS AND METHODS: A retrospective study of these 9 adults treated for progressive Pott's tetraplegia or paraplegia from 1982 to 1998 was conducted. The medical treatment consisted of the early administration in the first 3 patients of rifampin, isoniazid and ethambutol for 18 months; and, in the remaining 6 patients, of rifampin, isoniazid, ethambutol and pyrazinamide for 6 months. This medical treatment was systematically started 3 weeks befo…

BACKGROUND: Tuberculosis of the endometrium is usually associated with infertility. Once a patient is diagnosed with endometrial tuberculosi … psy. Antituberculosis chemotherapy was commenced at 14 weeks' gestation with rifampicin, isoniazid, pyrazinamide and pyridoxine. The pregnancy remained uneventful, and the patient delivered a normal female infant, 3,380 g at term. Pathologic examination of the placenta was performed, but there were no granulomas or features of chronic inflammation.

289 patients with TB, presented to a single tertiary care unit in Saudi Arabia between 1989 and 1994. The isolate from each patient was tested for in vitro susceptibility to rifampicin, isoniazid, ethambutol, pyrazinamide and streptomycin. 25 patients (8.7%) had isolates resistant to at least 1 anti-tuberculous drug. Single drug resistance (SDR)-mainly isoniazid-occurred in 14, and resistance to at least 2 drugs (multi-drug resistance-MDR) in 11, of which 8 were due to both isoniazid and rifampicin. Previous drug treatment occurred significantly more often in patients with MDR (8/11), than SDR…

To examine the effect of HIV on response to treatment and recurrence rate in patients with tuberculosis (TB), we have followed 239 previously untreated, adult, TB patients in a prospective cohort study in Lusaka, Zambia. One hundred and seventy-four (73%) were HIV-1 antibody positive. Patients with sputum smear positive, miliary, or meningeal TB were prescribed 2 months daily streptomycin, thiacetazone, isoniazid, rifampicin, pyrazinamide followed by 6 months thiacetazone and isoniazid; others, 2 months streptomycin, thiacetazone and isoniazid followed by 10 months thiacetazone and isoniazid. …

BACKGROUND: In 2020, WHO guidelines prioritised the use of a standard fully oral short treatment regimen (STR) consisting of bedaquiline, levofloxacin or moxifloxacin, ethionamide, ethambutol, high-dose isoniazid, pyrazinamide, and clofazimine for the management of rifampicin-resistant tuberculosis. A high prevalence of resistance to constituent drugs precluded its widespread use by countries in the WHO European region. We evaluated three 9-month fully oral modified STRs (mSTRs) in which ethionamide, ethambutol, isoniazid, and pyrazinamide were replaced by linezolid, cycloserine, or delamanid …

BACKGROUND: Efflux-mediated export of pyrazinoic acid (POA) has been associated with pyrazinamide (PZA) resistance, yet the specific transport components remain incompletely defined. Mycobacterium smegmatis , which exhibits intrinsically high POA efflux, provides a quantitative model to study PZA/POA transport mechanisms.
METHODS: Using clustered regularly interspaced short palindromic repeats interference, we silenced three efflux pump orthologs of Mycobacterium tuberculosis in M. smegmatis : MSMEG_5046 ( Rv1250c ), MSMEG_0241 ( Rv0202c /MmpL11), and MSMEG_0232 ( Rv0191c ). Gene knockdown wa…

Tuberculosis (TB) is a global health challenge caused by Mycobacterium tuberculosis, with drug resistance, treatment toxicity, and treatment adherence challenges continuing to impede control efforts. The objective of this review is to explore current advancements in TB treatment, for both drug-sensitive and drug-resistant TB, focusing on pharmacologic regimens, diagnostics, and adjunctive therapies. For drug-sensitive TB, a 4-month rifapentine-moxifloxacin regimen has been proven to be non-inferior to the traditional 6-month standard, while optimized pyrazinamide dosing or faropenem substituti…

Setting Standard anti-tubercular therapy (ATT) typically involves a regimen of 2 months of HRZE intensive phase, followed by 4 months of HRE continuation phase - 2HRZE/4HRE (H-isoniazid, R-rifampicin, Z-pyrazinamide, E-ethambutol). However, adverse effects and comorbidities often necessitate alternative non-standardized regimens. Objective The aim of the study was to identify and evaluate the use, rationale, and outcome of non-standardized ATT regimens in patients with drug-sensitive tuberculosis. Method Our prospective observational study included 148 patients at a tertiary care hospital who …

Ocular tuberculosis (TB) is an underrecognized extrapulmonary manifestation of Mycobacterium tuberculosis (M. tuberculosis) infection that can result in irreversible vision loss. Current systemic therapies, including isoniazid, rifampicin, pyrazinamide, and ethambutol, are often inadequate in achieving therapeutic intraocular concentrations and may pose ocular toxicity risks. The eye's unique anatomical and physiological barriers, including the cornea, blood-aqueous, and blood-retinal barriers, limit drug penetration, particularly to the posterior segment. This paper explores the potential of …

BACKGROUND: In 2010, the World Health Organization (WHO) revised the paediatric dosages of anti-tuberculosis drugs, increasing rifampicin to 15 mg/kg, isoniazid to 10 mg/kg and pyrazinamide to 35 mg/kg. We assessed treatment outcomes, safety and adherence among children treated with the new recommended dosages.
