Retinal tau imaging and histopathology as emerging Alzheimer's disease biomarkers: A systematic review.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by amyloid-β (Aβ) deposition and tau protein aggregation. The retina, as an extension of the central nervous system, has emerged as a promising site for detecting biomarkers of AD. OBJECTIVE: This systematic review synthesizes the current evidence on retinal tau protein as a diagnostic tool for AD and related tauopathies. METHODS: This systematic review adhered to PRISMA guidelines, analyzing studies on retinal tau biomarkers, structural changes, and diagnostic imaging techniques. The included stud

A Homozygous Frameshift Variant in KHDC4 Is Associated With a Syndromic Inherited Retinal Disease in Humans.

Inherited retinal diseases (IRDs) are genetically heterogeneous and often remain unsolved despite advanced sequencing. This study investigated two siblings from a consanguineous Arab-Christian family presenting with a syndromic IRD. Exome sequencing revealed a homozygous frameshift variant (c.1535_1538del:(p.Lys512Argfs*8) in the KHDC4 gene, which segregated with disease in an autosomal recessive pattern. In vitro overexpression of the mutant protein showed aberrant sub-cellular localization, and an in vivo zebrafish knockout model exhibited mild retinal dysfunction. These findings establish K…

Engineering Human Retinal Organoids and Eye-on-a-Chip Models for Degenerative Eye Disease.

Degenerative eye diseases are major causes of irreversible vision loss worldwide, but effective treatments remain limited, partly due to the lack of effective human models. Retinal organoids derived from stem cells can recapitulate key structural and physiological features of the human retina, offering powerful tools to study disease mechanisms and develop new therapies. Here, we review recent progress in engineering retinal organoids and eye-on-a-chip models for modeling degenerative eye diseases, with a focus on engineering innovations. We first describe conventional methods for organoid dif

Retinal thickness and microvascular alterations in the diagnosis of behcet's disease: a case-control study.

BACKGROUND: In the current study optical coherence tomography angiography (OCTA) was used to compare retinal thickness (RT) and superficial vascular density (SVD) in patients with Behcet’s disease (BD) and healthy controls. METHODS: The study included 17 BD patients (34 eyes) and 17 healthy participants (34 eyes). Each eye had an en-face OCTA scan. Macular RT and SVD were assessed in both groups after segmenting each image into 9 subregions. RESULTS: BD patients and controls had significantly different visual acuity (p < 0.001). Compared to the control group, BD patients had increased inner RT

Disease mechanisms revealed in the P23H opsin knock-in mouse model of retinitis pigmentosa.

The molecular mechanisms driving disease progression in autosomal dominant retinitis pigmentosa (adRP) due to a proline-to-histidine substitution (P23H) in opsin are not completely understood. Thus, we undertook to unlock features of the P23H/+ knock-in mouse model. Bisretinoids, the source of short-wavelength fundus autofluorescence (SW-AF), were elevated in albino P23H rhodopsin transgenic rats and in black and albino P23H/+ mice relative to wild type. Conversely, bisretinoid levels were lower in albino P23H/+ relative to black P23H/+ mice. Fluorescence microscopy revealed an inherent and ab

Synergistic retinal UCHL1 dysregulation and synaptic vulnerability reflect Alzheimer's disease severity.

Synaptic failure predicts cognitive decline in Alzheimer's disease (AD), yet its impact and molecular drivers in the human retina remain unclear. Leveraging the retina as an accessible central nervous system (CNS) proxy, we integrated spatially resolved histopathology of retinal cross-sections with ultrastructural, proteomic, and biochemical profiling across independent postmortem cohorts spanning normal cognition, mild cognitive impairment due to AD (MCI), and AD dementia. We uncover early, progressive degeneration of excitatory glutamatergic synapses, evidenced by losses of presynaptic vesic

Organotypic Retinal Explant Culture as a Model for Neuroretinal Degenerative Disease and Future Applications.

Organotypic retinal explant cultures are a valuable experimental tool for investigating neuroretinal diseases and advancing therapeutic strategies. Ex vivo methods preserve the natural architecture and intercellular interactions of the retina while enabling precise manipulations of the microenvironment, a distinct advantage over traditional in vitro two-dimensional cell models. Compared with in vivo animal and human studies, retinal explants offer ethical and cost-effective advantages, serving as bridge models for both animal to human and preclinical to clinical studies. This review explores t…

Stargardt Disease Presenting as Retinitis Pigmentosa: A Case of Monoallelic ABCA4-Associated Retinopathy.

Stargardt disease is an inherited retinal disorder caused by mutations in the ABCA4 gene and encompasses a wide range of phenotypes. Variability in age of onset, visual function, and retinal findings can make diagnosis challenging. We report a 48-year-old Hispanic female patient with a heterozygous ABCA4 variant, c.5603A>T (p.Asn1868Ile), previously classified as benign. The patient had well-preserved best-corrected visual acuity of 20/40 in both eyes and normal color vision testing. Fundus examination revealed optic disc pallor, arteriolar attenuation, and peripheral bone spicule-like pigment

QMix: Quality-Aware Learning With Mixed Noise for Robust Retinal Disease Diagnosis.

Due to the complex nature of medical image acquisition and annotation, medical datasets inevitably contain noise. This adversely affects the robustness and generalization of deep neural networks. Previous noise learning methods mainly considered noise arising from images being mislabeled, i.e., label noise, assuming all mislabeled images were of high quality. However, medical images can also suffer from severe data quality issues, i.e., data noise, where discriminative visual features for disease diagnosis are missing. In this paper, we propose QMix, a noise learning framework that learns a ro

Bacillary layer detachment: Updates on its clinical and prognostic significance in retinal disease.

Bacillary layer detachment (BALAD) refers to the distinctive splitting at the level of the photoreceptor inner segment myoid and accumulation of intraretinal fluid, as seen on optical coherence tomography (OCT). BALAD is an increasingly recognized OCT biomarker of numerous heterogeneous chorioretinal diseases, including posterior uveitis, age-related macular degeneration and macular neovascularization, neoplastic and paraneoplastic retinal disorders, rhegmatogenous retinal detachment, blunt ocular trauma, and miscellaneous conditions. The recognition of BALAD is clinically relevant because, ba…

Rapid clinical assessment of spatial contrast sensitivity changes in retinal disease and ageing.

Evidence suggests best-corrected visual acuity is not sensitive to subtle vision losses in early disease stages, indicating the need for other tests. Here, we quantified contrast sensitivity in 99 people (20-88 years): 52 with eye diseases (10 with early AMD, 9 diabetics without retinopathy, 22 glaucoma suspects, 11 high myopes) and 47 age-matched healthy controls using a new spatial vision chart (0.28-100% Weber contrasts; 3-60 c/° frequencies). In retinal diseases and healthy ageing, the chart detected frequency-dependent losses; sensitivity was reduced at 3 and 8 c/° in AMD, at 6 and 8 c/° …

Retinal and Optic Nerve Lesions Correspond to Amyloid in Autosomal Dominant Alzheimer's Disease.

Autosomal dominant Alzheimer's disease (ADAD) is a rare form of Alzheimer's disease (AD) in which the biology of the disease can be explored during the presymptomatic phase of the illness. The retina is an outgrowth of the central nervous system and therefore provides the opportunity for direct observation of neural tissue and its vasculature during life. Retinal thinning measured in vivo has been previously described in persons carrying ADAD mutations through fundoscopy but its pathologic correlates have not been reported. We describe retinal lesions detected using fundoscopy in vivo in a pat
