Twenty-nine patients previously diagnosed as having suffered from farmer's lung in or before 1970 were asked whether the condition had recurred and what measures they had taken to avoid such recurrence. Those who had retired from farming had been least affected by recurrence, while those who continued to farm had been protected by making silage instead of hay or by wearing protective respirators. To be effective, a respirator should be worn on every occasion that farm dust is encountered and must be properly maintained.

A 69-year-old retired miner with stage 4 non-small-cell lung cancer presented with a 2-month history of obstructive liver function tests following nivolumab immunotherapy. His case had not responded to high dose prednisolone or mycophenolate and he was admitted for investigation. MR cholangiopancreatography demonstrated areas of intrahepatic biliary tree beading and stricturing, in keeping with sclerosing cholangitis. Prednisolone and mycophenolate were stopped and ursodeoxycholic acid commenced with subsequent partial improvement of the patient's liver function tests.

Ruxolitinib is widely in use for treatment of myeloproliferative disorders. It causes inhibition of the Janus kinase (JAK) signal transducer and activation of transcription (STAT) pathway, which plays a key role in the underlying pathophysiology of myeloproliferative diseases. The authors present a case report of a retired furniture worker who initially presented with a mucin-secreting adenocarcinoma of the right ethmoid sinus. He underwent surgical resection, leaving the nasal septum intact, and topical treatment with 5-fluorouracil. He remained disease free. Three years after his initial presentation he was found to have a left nasal polyp on routine examination. This subsequently proved to be a second primary adenocarcinoma. A review of the literature has not shown any other cases of a second primary adenocarcinoma of the ethmoid sinuses.

The respiratory tract is often the site of injury from occupational exposures because it has direct contact with the ambient environment. Inhalation of potentially toxic materials in the workplace can lead to all major lung diseases with the exception of pulmonary vascular ones. New materials are being introduced into the workplace at a rate faster than their potential toxicities can be evaluated, and some are found to cause lung disease. Therefore, the possibility of occupational lung disease should be considered whenever a working or retired person has unexplained respiratory illness.

The purpose of this study was to examine sociodemographic and health-related risks for loneliness among older adults using Health and Retirement Study Data. Overall prevalence of loneliness was 19.3%. Marital status, self-report of health, number of chronic illnesses, gross motor impairment, fine motor impairment, and living alone were predictors of loneliness. Age, female gender, use of home care, and frequency of healthcare visits were not predictive. An 86-year-old man who was a retired tunnel construction worker with pleural plaques and chronic pleural effusion was referred for an evaluation of newly developed pulmonary infiltrates. A transbronchial lung biopsy revealed organizing pneumonia with transthyretin amyloid deposition in the alveolar septa. Genetic testing confirmed wild-type transthyretin amyloidosis. BACKGROUND: Donepezil, an acetylcholinesterase inhibitor commonly used to treat Alzheimer's disease (AD), is generally well tolerated. There have been no previous reports on donepezil-induced obsessive-compulsive disorder (OCD).
CASE PRESENTATION: The patient, a retired man in his 70s diagnosed with AD, displayed OCD symptoms following donepezil initiation, exacerbating post-stroke-specifically, a cerebral infarction in the right posterior limb of the internal capsule. Remarkably, the symptoms abated upon discontinuation of donepezil.
CONCLUSION: Caution should be exercised when using donepe

OBJECTIVE: To present a successfully treated case of hepatic artery aneurysm.
CLINICAL FEATURES: A 76-year-old, retired male Caucasian presented with acute abdominal pain which was diagnosed as acute cholecystitis. An aneurysm of the hepatic artery was found at operation for cholecystectomy, and an arteriogram confirmed the position of the aneurysm, distal to the origin of the gastroduodenal artery.
