Injectable platelet-rich fibrin as a valid therapeutic option for inactive linear morphea (en coup de saber): report of cases.

OBJECTIVES: Localized scleroderma, also called morphea, is an inflammatory cutaneous disorder characterized by sclerosis of the skin and underlying tissues. Morphea is currently classified into 4 subtypes: limited, generalized, linear and mixed. Linear morphea in an en coup de saber pattern (LScs) manifests as band-like, atrophic lesions predominantly affecting the frontoparietal area, leading to facial contour defects and skin pigmentation. However, pharmacological treatments for LScs are limited in meeting the aesthetic expectations of patients. Recently, autologous platelet concentrates hav

Is it really post-irradiation morphea or oleoma of the breast?-A case report and literature review.

We report the case of a 65-year-old woman who underwent breast-conserving surgery (BCS) and radiotherapy for breast cancer with hyperpigmentation and skin thickening of the treated breast site 10 years after the surgery. The patient was injected with a liquid foreign body in both breasts 30 years ago. Theses clinical features were considered scleroderma, post-irradiation morphea (PIM), and recurrent breast cancer for differential diagnosis. We performed breast magnetic resonance imaging (MRI), however, the patient had no abnormal findings. Owing to the pain, increased hyperpigmentation, and po

Scleredema of Buschke associated with lichen sclerosus: Three cases.

Scleredema adultorum of Buschke is a rare fibromucinous, scleroderma-like connective tissue disease most commonly found in a post-infectious setting or linked to hematological disorders or diabetes. Lichen sclerosus et atrophicus is an autoimmune condition only in 2.5% of cases localized exclusively at an extragenital site, occurring in up to 34% of patients in association with other autoimmune diseases such as vitiligo, thyroid disorders, alopecia areata, lichen planus, morphea, pernicious anemia and systemic lupus erythematosus. In particular, a stronger link with an autoimmune background in

[Plasma cell panniculitis: a histopathological variant of morphea profunda].

BACKGROUND: Plasma cell panniculitis is a rare histopathological variant of morphea profunda. Its occurrence in two siblings has been reported once. We report a new case in which a brother and a sister with a family history of consanguinity presented plasma cell panniculitis of long duration. CASE REPORTS: A man and his sister, 38 and 28 years old respectively, presented thick hyperpigmented indurations of the trunk and thighs since puberty. Previous investigations did not lead to any definitive diagnosis. Deep cutaneous specimens taken from both patients exhibited indistinct histological fea

Cryoablation of drug-resistant ventricular tachycardia in a patient with a variant of scleroderma.

A 37-year-old man with a benign variant of scleroderma (CRST syndrome: calcinosis circumscripta, Raynaud's phenomenon, sclerodactyly, and telangiectasia) presented with recurrent ventricular tachycardia. Preoperative electrophysiologic study suggested that the mechanism of tachycardia was an ectopic pacemaker focus in the right ventricle. Right ventricular dilatation, tricuspid insufficiency, normal pulmonary pressures, and normal coronary arteries were also demonstrated. At surgery, epicardial mapping localized the site of origin of ventricular tachycardia to the anterior right ventricle near

Clinical and Epidemiological Features of Juvenile-Onset Systemic Sclerosis From a Nationwide Survey in Japan.

OBJECTIVE: To evaluate clinical and epidemiological features of juvenile-onset systemic sclerosis (jSSc) in Japan and to identify racial and generational differences. METHODS: We surveyed patients with jSSc (developed < 18 years of age) who visited selected facilities in Japan between January 2016 and December 2020. We estimated the number of patients with jSSc and the annual incidence rate in Japan. Thereafter, differences in clinical characteristics by disease subtype, autoantibody, and age at investigation were analyzed and compared with previous cohorts. RESULTS: Of the 3005 institutions

Acral manifestations of paraneoplastic and collagen vascular diseases.

The skin often signals a number of systemic disease, making skin findings of paramount significance. Paraneoplastic diseases and collagen vascular diseases are vitally important illnesses. Paraneoplastic diseases and collagen vascular diseases may also occur with many different acral skin findings. Paraneoplastic skin diseases, associated with some cancers, are by definition nonmalignant skin disorders. These diseases can occur before, simultaneously, or after the diagnosis of cancer. Acral paraneoplastic diseases include acanthosis nigricans maligna, acquired pachydermatoglyphia, acrokeratosi

Gastrointestinal hemorrhage in patients with systemic sclerosis and CREST syndrome.

OBJECTIVES: Systemic sclerosis (SSc) and calcinosis, Raynaud's phenomenon, esophageal disease, sclerodactyly, telangiectasia (CREST) syndrome present distinctive microvasculature lesions that are thought to be responsible for tissue damage and disease progression. Involvement of the gastrointestinal tract may lead to the occurrence of profuse hemorrhage. We performed a study to assess the incidence and characteristics of gastrointestinal hemorrhage in a large group of patients with SSc and CREST syndrome. METHODS: We reviewed the medical records of 144 patients with SSc/CREST seen at our inst

Successful treatment of scleroderma with PUVA therapy.

PUVA therapy was carried out on four patients with scleroderma; three of them had cutaneous manifestations of progressive systemic sclerosis and one other exhibited generalized morphea. PUVA therapy was given with daily doses of 0.25J/cm2 or 0.4J/cm2 for 3-8 weeks, resulting in total doses between 3.5J/cm2 and 9.6J/cm2. All four patients responded well to this treatment; improvements of hand closure, skin sclerosis index, and flexion of fingers or knee joints were obtained. Thus, PUVA appeared to be beneficial for treating scleroderma.

Comparative esophageal and anorectal motility in scleroderma.

Esophageal and anorectal pressures were recorded in 26 patients (4 men and 22 women) with scleroderma. Eleven patients suffered from a localized form of the disease and 15 from progressive systemic sclerosis. The latter only had marked functional abnormalities in esophageal and anorectal motility. Mean resting pressure at the lower esophageal sphincter of patients with progressive system sclerosis and controls was, respectively, 6 +/- 2 and 25 +/- 1 mmHg (p less than 0.001); mean closing pressure was 5 +/- 5 and 48 +/- 3 mmHg (p less than 0.001); coordination of opening the lower esophageal sp

Serological changes in progressive systemic sclerosis.

Sera from 37 patients with scleroderma were studied for the presence of auto-antibodies. Rheumatoid factors and antibodies to smooth muscle and thyroid were seen in some patients. Ten of 31 patients (32%) had antibodies to nuclear antigens. They were not related to the extent or severity of disease but were often associated with other complications or coincidental diseases. These serological changes only occurred in the patients with progressive systemic sclerosis and did not occur in morphoea. Antibodies to double-stranded DNA were found in only one case, and neither clinical nor serological

A comprehensive framework for navigating patient care in systemic sclerosis: A global response to the need for improving the practice of diagnostic and preventive strategies in SSc.

Systemic sclerosis (SSc), the most lethal of rheumatologic conditions, is the cause of death in >50% of SSc cases, led by pulmonary fibrosis followed by pulmonary hypertension and then scleroderma renal crisis (SRC). Multiple other preventable and treatable SSc-related vascular, cardiac, gastrointestinal, nutritional and musculoskeletal complications can lead to disability and death. Vascular injury with subsequent inflammation transforming to irreversible fibrosis and permanent damage characterizes SSc. Organ involvement is often present early in the disease course of SSc, but requires carefu

Outcome measures and monitoring framework in juvenile systemic sclerosis: bridging routine care and clinical trials.

INTRODUCTION: Juvenile systemic sclerosis (jSSc) is an orphan multisystem autoimmune disease that can cause significant morbidity during childhood and can lead to significant worsening of quality of life. The pathophysiological features are identical with adult-onset systemic sclerosis (SSc), jSSc exhibits a distinct clinical phenotype characterized by a significantly higher prevalence of diffuse cutaneous involvement, frequent overlap syndromes, distinct autoantibody profiles, and different long-term survival trajectories. Historically, the evaluation of jSSc has relied heavily on the extrapo

Capillaroscopy in the daily clinic of the pediatric rheumatologist.

In the last decade, nailfold capillaroscopy is finding its way to the daily clinic of (pediatric) rheumatologist. This review will provide the necessary knowledge for the clinician performing this easy and non-invasive examination in children. In the first part, background information on type of capillaroscopy device and standardized (internationally validated) interpretations for the different capillary variables compared to healthy pediatric controls will be provided. The second part focusses on capillary changes that are observed in Raynaud's phenomenon with follow-up recommendations. This

Prevalence of erectile dysfunction in Thai scleroderma patients and associated factors.

BACKGROUND: Erectile dysfunction (ED) has been reported among patients with systemic sclerosis (SSc) and primarily limited cutaneous SSc in Caucasians. While there is no data on ED among Thais in whom the diffuse cutaneous SSc subset is common. OBJECTIVES: We aimed to estimate the prevalence of ED among Thais with SSc, evaluate its severity, and determine the associated factors. METHODS: We did a cross-sectional study among adult Thai male SSc patients. All eligible patients: a) completed the IIEF-15 questionnaire by themselves; b) underwent a genital examination by an experienced urologist

Acquired disorders with depigmentation: A systematic approach to vitiliginoid conditions.

Acquired disorders with depigmentation are commonly encountered by dermatologists and present with a wide differential diagnosis. Vitiligo, the most common disorder of acquired depigmentation, is characterized by well-defined depigmented macules and patches. Other conditions, such as chemical leukoderma, can present with similar findings, and are often easily mistaken for vitiligo. Key clinical features can help differentiate between acquired disorders of depigmentation. The first article in this continuing medical education series focuses on conditions with a vitiligo-like phenotype. Early re
