A systematic clinical examination of the shoulder joint, including a structured medical history, is essential for the diagnosis of shoulder pathologies. Complex clinical situations that are accompanied by pain, restriction of movement, loss of strength, or instability have to be considered in accordance with the functional interaction between the cervical spine, the shoulder girdle, and the glenohumeral joint. Only accurate diagnosis allows us to apply successful therapeutic interventions. In order to achieve this, the physician needs to use standardized clinical tests and signs combined with

Various shoulder injuries can give rise to axillary artery complications. Two such cases were admitted to Tan Tock Seng Hospital late last year. Both were young adult males, who had developed axillary artery thrombosis secondary to shoulder injuries. The following is a short discussion on the common types of shoulder injuries, how they result in axillary artery compromise and a short section on their management and treatment.

OBJECTIVES: To characterize the literature on operative interventions for proximal humerus nonunions in adults. Second, to identify prognostic factors associated with outcomes for locked plate open reduction and internal fixation (ORIF).
DATA SOURCES: PubMed, EMBASE, MEDLINE, Scopus, and Cochrane databases were searched for articles from 1990 to 2020.
STUDY SELECTION: Studies reporting outcomes of proximal humerus nonunions managed with ORIF, hemiarthroplasty (HA), total shoulder arthroplasty (TSA), or reverse TSA (RTSA) were included. Studies failing to stratify outcomes by treatment or fra

Stress lesions of the shoulder in athletic and active adolescents are most commonly associated with overhead sports. One of the most uncommon stress lesions of the shoulder in adolescents is an avulsion of the lesser tuberosity of the proximal humerus. To our knowledge, only 2 other cases of lesser tuberosity avulsions from repetitive motion have been reported, both of which were secondary to baseball pitching. This article describes a case of an isolated partial avulsion of the lesser tuberosity of the humerus in an adolescent as a result of repetitive stress from fly fishing. The patient had

Clavicle fractures are a common injury resulting from a high-energy force, such as a fall onto the shoulder, motor vehicle accident, or sporting activity. Although some clavicle fractures may be treated nonoperatively, operative treatment results in higher union rates and faster return to activity. Here we discuss the operative treatment options for plating of clavicle fractures; specifically, a single plate placed either superiorly or anteriorly or two plates placed orthogonally. Because both techniques provide adequate stability, fracture and patient characteristics should guide the surgical

Subchondral insufficiency fracture (SIF) of the humeral head is rare compared to SIF of the femoral head, and most of the reported cases have been of elderly women with osteoporosis. We report a rare case of SIF of the shoulder in a man aged 32 years. Following a consultation with a local physician, the patient was referred to our hospital with worsening right shoulder pain and suspected humeral head necrosis based on magnetic resonance imaging (MRI) findings. The patient had no history of steroid or excessive alcohol consumption. The patient had a limited range of motion in the shoulder joint

Posterior shoulder dislocation occurs rarely and is challenging to manage. The mechanisms of trauma are varied, which complicates diagnosis. Missed or delayed diagnosis and treatment can have serious deleterious effects on shoulder function. All cases of suspected posterior shoulder dislocation require a high level of suspicion and appropriate imaging. Identification of associated injuries, such as fractures and rotator cuff tears, is important to guide treatment. In the acute setting, most patients are treated with closed or open reduction with additional soft-tissue or bony procedures. Patie

Healing of clavicular fractures with shortening of 15 mm or more has been associated with poor function and open reduction and internal fixation has been recommended as the appropriate initial treatment. We studied 41 patients at least three months after clavicular fracture. Clavicular shortening was calculated and shoulder function was assessed using the Constant Scoring method. Although three patients with shortening of 1.5 cm or more had low scores, this could be attributed to other factors. We could not demonstrate any relationship between clavicular shortening and shoulder function.

Complications of shoulder dystocia are divided into fetal and maternal. Fetal brachial plexus injury (BPI) is the most common fetal complication occurring in 4-40% of cases. BPI has also been reported in abdominal deliveries and in deliveries not complicated by shoulder dystocia. Fractures of the fetal humerus and clavicle occur in about 10.6% of cases of shoulder dystocia and usually heal with no sequel. Hypoxic ischemic brain injury is reported in 0.5-23% of cases of shoulder dystocia. The risk correlates with the duration of head-to-body delivery and is especially increased when the duratio

There is substantial variability in the treatment of proximal humeral fractures because of classification systems with poor interobserver reliability, rapid advances in technology (e.g., anatomically designed locking plates and reverse shoulder arthroplasty), and limited Level-I and II evidence for best treatments based on fracture patterns and physiological age. Almost three-quarters of proximal humeral fractures can be treated nonoperatively with good functional results as nearly 50% are nondisplaced or Neer one-part fractures. Another quarter occur in infirm patients or are characterized by

OBJECTIVE: We supposed difference of rotation alignment of postoperative humeral shaft fracutres between open reduction and internal fixation (ORIF) and intramedullay nailing (IMN) could be identified and the difference might influence the involved shoulder. This study evaluated and compared the extent of malrotation and shoulder function after humeral shaft fractures treated operatively with IMN or ORIF.
MATERIALS AND METHODS: Fifty humeral shaft fractures were randomly allocated into 2 groups. Group I underwent antegrade IMN and group II underwent ORIF. Malrotation was measured postoperativ

AIMS: The aim of this study was to define the complications and long-term outcome following adolescent mid-shaft clavicular fracture.
METHODS: We retrospectively reviewed a consecutive series of 677 adolescent fractures in 671 patients presenting to our region (age 13 to 17 years) over a ten-year period (2009 to 2019). Long-term patient-reported outcomes (abbreviated version of the Disabilities of the Arm, Shoulder and Hand (QuickDASH) score and EuroQol five-dimension three-level (EQ-5D-3L) quality of life score) were undertaken at a mean of 6.4 years (1.2 to 11.3) following injury in severel

Stress fractures in children are rare compared with the incidence in adults. This report describes an 11-year-old girl with stress fractures of the acromion, clavicle, and first rib on the left and contralateral fractures of the first and second ribs. It was eventually discovered that these fractures were caused by a nervous tic consisting of repetitive, vigorous shrugging and translation of the shoulders.

CONCLUSIONS: Pelvic, acetabular, femur, and shoulder fractures cluster with hip fractures within individuals at greater than expected frequency. If we observe similar treatment risk reductions within that cluster, subsequent trials could consider the use of a composite endpoint to better estimate hip fracture r …
