Pattern of Closure of Skull Base Synchondroses in Crouzon Syndrome.

BACKGROUND: The age of closure of skull base synchondroses has never been analyzed in a homogenous population of children with Crouzon syndrome. METHODS: A retrospective case-control study was performed on 30 Crouzon children (17 male, 13 female) aged 1 month to 12.48 years with Fibroblast Growth Factor Receptor type 2 mutation. Eleven synchondroses were analyzed on millimetric computed tomodensitometric slices before surgery. Syndromic patients were compared with a series of 235 healthy children previously published. RESULTS: Synchondrosis closure follows a global pattern that occurs earlie…

Collagen matrix as an inlay in endoscopic skull base reconstruction.

BACKGROUND: Multi-layer reconstruction has become standard in endoscopic skull base surgery. The inlay component used can vary among autografts, allografts, xenografts and synthetics, primarily based on surgeon preference. The short- and long-term outcomes of collagen matrix in skull base reconstruction are described. METHODS: A case series of patients who underwent endoscopic skull base reconstruction with collagen matrix inlay were assessed. Immediate peri-operative outcomes (cerebrospinal fluid leak, meningitis, ventriculitis, intracranial bleeding, epistaxis, seizures) and delayed complic…

Lesion in Scalp and Skull as the First Manifestation of Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common primary tumor of the liver and the fifth most common cancer in the world. The lungs, bone, and lymph nodes are frequent sites of metastasis of HCC. The purpose of the present study is show that metastases, although rare, must be among the differential diagnosis of skin lesions and that a diagnostic research based on these findings can be conducted. The authors report a rare case of metastatic hepatocellular injury to the scalp and skull treated by a radical surgical approach. Excision of the lesion in the scalp was performed "en bloc." The tumo…

[Primary lymphoma of the skull: Case report and literature review].

Primitive lymphomas of the bone are exceptional tumors, representing 4% of all non-Hodgkin lymphomas. The location at the skull remains the rarest. We report the case of a 56 year old patient with lytic lesions in the skull of a small cell lymphoma B, treated with primary chemotherapy and intensity-modulated radiotherapy in arctherapy with a dose of 30Gy in 15 fractions. With a follow-up time of 18 months after the end of treatment, the patient has no sign of disease evolution.

Infiltrative mass of the skull base and nasopharynx: A diagnostic conundrum.

Inflammatory skull base masses are enigmatic and often behaviourally unpredictable. We present a case of idiopathic hypertrophic pachymeningitis (IHP) forming a central skull base mass to illustrate the process required when one investigates such skull base lesions. This is the first description of mass forming or tumefactive IHP extending into the nasopharynx. A 32-year old woman presented with frontal headaches and nasal discharge. She then deteriorated and was admitted with worsening headaches, serosanguinous nasal discharge and bilateral ophthalmoplegia. Multimodality imaging confirmed a d…

PURLs: helmets for positional skull deformities: a good idea, or not?

Probably not. Helmets appear to be no more effective than waiting for natural skull growth to correct the shape of an infant's head.

Transorbital neuroendoscopic surgery for the treatment of skull base lesions.

Transorbital neuroendoscopic surgery (TONES) is a relatively new technique that not only allows access to the contents of the orbit but also the intracranial compartment, including the anterior cranial fossa, middle fossa and lateral cavernous sinus. In this study, we aimed to retrospectively review the largest experience to our knowledge with regards to surgical outcomes of skull base pathologies treated with a TONES procedure. Forty patients (aged 3-89 years) underwent 45 TONES procedures between the years of 2006-2013. Pathologies were cerebrospinal fluid leak repair (n=16), traumatic fract…

Endoscopic anatomy of the skull base explored through the nose.

OBJECTIVE: Different surgical approaches have been used over the years in order to access skull base. The endoscopic endonasal approach represents a direct and minimally invasive approach to the suprasellar, retrosellar, and retroclival space, with the advantage of avoid brain retraction and visualize safely and effectively the surgical target. The present contribution aims to provide anatomical details of the skull base as seen from below (i.e., via an endoscopic endonasal approach). METHODS: Five human cadaver heads were dissected. The anatomical neurovascular structures within the skull ba…

[On the formation of open skull fractures inflicted by cutting objects].

The objective of the present work was to study the properties of thrusting weapons, the conditions of their action, and the influence on the formation of cranial vault fractures. The lacerated wounds were simulated using an ordinary axe, native skull and isolated cranial vaults, specially designed multilayer polymeric models of bone and other tissues. This approach made it possible to obtain the objective characteristics of a thrusting blow as a medium-speed thrust (7- 20 m/s) with the impact mass commensurate with or exceeding the weight of the affected object. In indispensable condition for …

Parents' decision for helmet therapy in infants with skull deformation.

PURPOSE: Helmet therapy is regularly prescribed in infants with positional skull deformation. Evidence on the effectiveness is lacking, which complicates decision making. This study aims to assess the relation between parents' decision for treatment of skull deformation in their infant and their level of anxiety, decisional conflict, expectations of treatment effect, perceived severity of deformation and perceived side effects. METHODS: Parents of 5-month-old infants with skull deformation were invited to participate in a survey. Data collection included background characteristics, anthropome…

The pioneering contribution of italian surgeons to skull base surgery.

The origin of neurosurgery as a modern, successful, and separate branch of surgery could be dated back to the end of the 19th century. The most important development of surgery occurred in Europe, particularly in Italy, where there was a unique environment, allowing brilliant open-minded surgeons to perform, with success, neurosurgical operations. Neurosurgery began at the skull base. In everyday practice, we still pay tribute to early Italian neuroanatomists and pioneer neurosurgeons who represented a starting point in a new, obscure, and still challenging field of medicine and surgery during…

Natural head position and growth of the facial part of the skull.

The aim of this study was to determine any correlation between natural head position and cranio-cervical growth direction and if natural head position influences facial growth direction. One hundred sixty (160) cephalometric radiographs were examined and cranio-cervical inclinations determined (angles: NS-Ver, NS-OPT, NS-CVT). On the basis of the NS-ML angle, radiographs were divided into two groups: mandibular anteriorotation and posteriorotation. On the basis of the SGo/NMe index, two groups were formed: short-faced and long-faced subjects. The angles NS-Ver, NS-OPT, and NS-CVT describe cran

Stereotactic radiosurgery for the treatment and palliation of base of skull metastases.

OBJECTIVES: Patients with skullbase metastases often present with evolving cranial nerve deficits, pain and advanced systemic disease. These factors along with declining performance status limit invasive interventions; yet, a safe, efficient treatment modality that augments palliative efforts is desirable. We herein report the role of stereotactic radiosurgery (SRS) in the management of base of skull metastases. METHODS: This retrospective institutional series reviewed 18 consecutive patients (12 male, 6 female) with of a total of 21 skullbase metastases. Seventy-five percent of patients pres…

Clinical anatomy and imaging of the cranial nerves and skull base.

Evaluation of patients with cranial neuropathies requires an understanding of brainstem anatomy and nerve pathways. Advances in neuroimaging, particularly high spatial resolution magnetic resonance imaging (MRI), have enabled visualization of these tiny structures and their related pathology. This review provides an approach toward using imaging in the evaluation of cranial nerve (CN) and skull base anatomy and pathology. Because brainstem nuclei are inextricably linked to the information contained within CNs, they are briefly mentioned whenever relevant; however, a comprehensive discussion of…
