Spasmodic torticollis: the problem of pathophysiology and assessment.

In 17 patients with idiopathic spasmodic torticollis (ST) quantitative indices for both signs (extent and direction of the head deflexions) and complaints (of deflexion, shaking and pain) were collected. In the literature deflexion in the horizontal plane is most frequently considered, but analysis of the data shows that deflexion in the coronal and sagittal planes is also important. Furthermore, it is found that especially the deflexions in the coronal and sagittal plane covariate with the patients' complaints, but not with horizontal deflexion. On the basis of these and related data, it is s

Depression and hypochondriasis in family practice patients with somatization disorder.

The relationships specified in DSM-III between somatization disorder and depression, and somatization disorder and hypochondriasis require further validation and easier methods of detection for use by primary care physicians. The authors investigated hypochondriacal and depressive symptoms in 13 family practice outpatients with somatization disorder. Pain complaints and depressive symptomatology were present in over 75% of this group, while hypochondriacal symptoms were present in 38%. The mean score on the somatization scale of the Hopkins Symptom Check List (HSCL-90) was greater than that re

The clinical utility of the Maslach Burnout Inventory in a clinical population.

This replication study examines the use of the Maslach Burnout Inventory (MBI-GS), a self-report questionnaire on burnout, as a clinical diagnostic instrument for measuring clinical burnout. The MBI and Mini International Neuropsychiatric Interview (MINI), a semistructured interview based on classifications in the Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV; American Psychiatric Association, 1994), were administered to 419 outpatients at a Dutch multicenter institution specializing in the treatment of work-related psychological problems. MBI scores indicative of the

Mental disorders in patients with epilepsy.

OBJECTIVES: The purpose of this study is to assess the feasibility of foetal nasal bone (NB) measurement during the first trimester of pregnancy, and to examine the contribution of this measurement to the prenatal screening for Down syndrome following the definition of NB threshold using ROC curves in an unselected population. METHODS: This prospective study was carried out at our centre SIHCUS-CMCO (reference centre) from January 2002 to December 2004 on a total of 2,044 pregnant outpatients at gestational weeks 11-14. Only 1260 singleton foetuses were used for statistical analysis. In the 7

Do we have an alternative for the Rome III gastroduodenal symptom-based subgroups in functional gastroduodenal disorders? A cluster analysis approach.

BACKGROUND: Functional dyspepsia (FD) is a heterogeneous biopsychosocial disorder. The Rome III consensus proposed a subdivision into epigastric pain syndrome and postprandial distress syndrome, based on gastroduodenal symptom pattern only; nausea/vomiting- and belching disorders were classified as separate functional gastroduodenal disorders (FGD). We aimed to investigate an alternative subdivision of FGD, taking into account gastric sensorimotor function, anxiety & depression and 'somatization', besides gastroduodenal symptoms. METHODS: Gastroduodenal symptom data were available for 857 con

Obsessive-compulsive characteristics in patients with idiopathic spasmodic torticollis.

To explore suggested relationships between involuntary motor disorders and obsessive-compulsive disorder (OCD), we evaluated obsessive-compulsive characteristics in patients with idiopathic spasmodic torticollis (IST). Twenty-two patients with IST had significantly higher scores than 29 age- and sex-matched healthy controls on the Yale-Brown Obsessive-Compulsive Scale and the obsessionality subscale of the Symptom Checklist-90-Revised. The IST patients also had higher (though not significantly so) scores on the Maudsley Obsessive-Compulsive Inventory. Their Beck Depression Inventory scores wer

Vagus Nerve Stimulation in the Treatment of Somatic Stress Disorder: Neurobiological Basis, Evidence Base and Prospects for Enhanced Psychotherapy Vagus nerve stimulation (VNS) has shown mixed but promising results for a number of neuropsychiatric conditions, such as depressive disorder, anxiety disorders, chronic pain and trauma- and stress-related disorders. However, the evidence to date on the use of VNS for somatic stress disorder is limited. Preliminary findings suggest that VNS influences physiological and psychological mechanisms that are also relevant to the pathophysiology of somatic

The 'medically unexplained symptoms' syndrome concept and the cognitive-behavioural treatment model.

The American Psychiatric Association's, 2013 DSM-5 abandoned the use of the term 'medically unexplained symptoms' for non-neurological disorders. In the UK, treatments for various medical illnesses with unexplained aetiology, such as chronic fatigue syndrome, irritable bowel syndrome and fibromyalgia, continue to fall under an MUS umbrella with cognitive behavioural therapy promoted as a primary therapeutic approach. In this editorial, we comment on whether the MUS concept is a viable diagnostic term, the credibility of the cognitive-behavioural MUS treatment model, the necessity of practition

The importance of contextual aspects in the care for patients with functional somatic symptoms.

Functional somatic symptoms refer to physical symptoms that cannot be (bio) medically explained. The pattern or clustering of such symptoms may lead to functional syndromes like chronic fatigue syndrome, fibromyalgia, irritable bowel syndrome, among many others. Since the underlying pathophysiology remains unknown, several explanatory models have been proposed, nearly all including social and psychological parameters. These models have stimulated effectiveness studies of several psychological and psychopharmacological therapies. While the evidence for their effectiveness is steadily growing, e

[Environmental medical syndromes].

Environmental medical syndromes comprise sick building syndrome (SBS), multiple chemical sensitivity (MCS)/idiopathic environmental intolerances (IEI), electromagnetic hypersensitivity, chronic fatigue syndrome (CFS), burnout, fibromyalgia, and the candida syndrome. There is also some overlap described in the literature. There is still no established knowledge of etiology, pathology, pathophysiology, diagnostics, therapy, prevention and prognosis. These syndromes are thought to result from a complex interaction of physical, chemical and/or (micro)biological environmental stresses, individual d

From traumatic attachment to somatization disorder.

OBJECTIVE: This paper emphasizes attachment trauma as an important etiological factor in somatization disorder and psychoanalytic psychotherapy as an effective treatment. METHOD: Weekly psychotherapy with an 18 year old with somatization disorder, using the Conversational Model, is described. CONCLUSION: Early cumulative trauma has far-reaching health consequences. Brief psychotherapy produces symptom relief, and long-term analytic therapy is necessary for sustained improvement.

Update on research and treatment of premenstrual dysphoric disorder.

Many women in their reproductive years experience some mood, behavioral. or physical symptoms in the week prior to menses. Variability exists in the level of symptom burden in that some women experience mild symptoms, whereas a small minority experience severe and debilitating symptoms. For an estimated 5%-8% of premenopausal women, work or social functioning are affected by severe premenstrual syndrome. Many women in this group meet diagnostic criteria for premenstrual dysphoric disorder (PMDD). Among women who suffer from PMDD, mood and behavioral symptoms such as irritability, depressed moo

Dyspepsia as a somatic expression of guilt: a case report.

A 58-year-old woman developed chronic, severe symptoms of heartburn, epigastric pain, and regurgitation that persisted for 2 years. She underwent a thorough evaluation and no organic cause was identified. Therefore, a diagnosis of dyspepsia was made. Her symptoms were refractory to pharmacological treatment. Upon further probing, the patient reported that the onset of her symptoms coincided with the death of her son of cancer 2 years earlier. She blamed herself for the death of her son and admitted to a need for self-punishment. A brief course of treatment using metaphors and hypnosis resulted

Somatization disorder and Briquet's syndrome. An assessment of their diagnostic concordance.

Somatization disorder (SD) as defined by DSM-III is a modification of criteria previously established to define Briquet's disorder (BD). We examined whether the less stringent SD criteria identify the same patient population as the more stringent BD criteria. All psychiatric female outpatients who reported having multiple unexplained physical problems prior to the age of 30 years were included in the study. Eighty-five (10.7%) of 794 patients fulfilled these screening criteria. Of the 85 patients, 41 had both SD and BD. Thirty-six patients did not have either disorder. The results indicate tha
