[Evaluation of the curative effect of functional endoscopic sinus surgery by acoustic rhinometry].

OBJECTIVE: To evaluate the curative effect of Functional endoscopic sinus surgery (FESS) who suffered with chronic rhinosinusitis by acoustic rhinometry. METHOD: We collected 60 patients who accepted FESS for chronic rhinosinusitis, and calculated the mean minimal cross-sectional area (MCSA), nasal volume (NV), nasal airway resistance (NAR) by acoustic rhinometry within endoscopy to estimate the curative effect of FESS. RESULT: Four weeks after FESS, the patients' ethmoid sinus, maxillary sinus, frontal sinus and sphenoid sinus were clear by endoscopy. Meanwhile, the mean MCSA and NV by acoust

Restricted diffusion in the superior ophthalmic vein and cavernous sinus in a case of cavernous sinus thrombosis.

A previously healthy 14-year-old boy developed headache, stiff neck, fever, diplopia, right proptosis, and right complete sixth and partial third cranial nerve palsies. Orbital CT showed features of pansinusitis and orbital fat stranding. An initial diagnosis of orbital cellulitis was made. However, closer inspection of the CT disclosed nonfilling of the right superior ophthalmic vein (SOV) and both cavernous sinuses, suggesting cavernous sinus thrombosis (CST). CT venography (CTV) confirmed these features and disclosed nonobstructing thrombus within the left sigmoid sinus and proximal segment

Isolated central nervous system aspergillosis in the acquired immunodeficiency syndrome.

Aspergillus infection involving the central nervous system are unusual, but should be included in the differential diagnosis in patients with the acquired immunodeficiency syndrome and neurologic signs and symptoms. Of the few reported AIDS cases with central nervous system aspergillosis, the majority have had focal brain abscesses. We report an atypical case that presented as a basal meningitis with pontine infarction secondary to invasive Aspergillus sinusitis.

Acute severe irreversible visual loss with sphenoethmoiditis-'posterior' orbital cellulitis.

Orbital cellulitis secondary to adjacent paranasal sinusitis presents with marked proptosis, ophthalmoplegia, eyelid edema, chemosis, and/or conjunctival hyperemia. These conditions often precede visual dysfunction. "Posterior" orbital cellulitis secondary to sphenoethmoidal sinusitis may be defined as the clinical syndrome in which early severe visual loss overshadows or precedes accompanying inflammatory orbital signs. The visual loss may be attributed to involvement of the intracanalicular or orbital apical segment of the optic nerve. Total irreversible unilateral visual loss developed in t

Coil migration after endovascular coil occlusion of internal carotid artery pseudoaneurysms within the sphenoid sinus.

INTRODUCTION: We report two cases of coil migration after endovascular treatment of pseudoaneurysm of the internal carotid artery within the sphenoid sinus with coils and noncovered stents. CASE REPORTS: Two patients underwent sphenoid sinus exposure for pituitary adenoma and chronic infection, respectively. As a complication pseudoaneurysms of the internal carotid artery within the sphenoid sinus developed. One patient was treated with stent and coils, the second with coils alone. Both patients experienced coil migration after 9 and 26 months, respectively, with the necessity for further tre

Retrospective Study on the Prevalence of Haller Cells Using Cone Beam Computed Tomography.

BACKGROUND AND OBJECTIVE: The paranasal sinuses are cavities filled with air located within specific bones, namely the frontal, ethmoidal, maxillary, and sphenoidal sinuses. The ethmoidal sinus is composed of three groups: anterior, middle, and posterior, which are found on both sides. Haller cells (HC), also known as infraorbital ethmoid cells, are projections of the anterior ethmoid sinus that extend into the floor of the orbit and the upper part of the maxillary sinus. Infraorbital ethmoid cells have been associated with several disease conditions and symptoms, such as sinusitis, headaches,

[Complications of endonasal paranasal sinus surgery--diagnostic and therapeutic consequences].

BACKGROUND: Functional endoscopic sinus surgery has been proven the therapeutic method of choice in surgical therapy of chronic sinusitis. On the other hand, endonasal sinus surgery may cause severe complications even when performed by a skilled surgeon. This is easily explained by the close vicinity of many functionally important structures to the operative site. CASE REPORTS: Three histories are reported that involve possible complications even in apparently simple cases. Diagnostic and therapeutic consequences are discussed. In a case previously diagnosed histologically as chronic unspecif

A case report: Diagnosis and treatment of idiopathic hypertrophic pachymeningitis.

A 57-year-old man who suffered from a headache for 1 year, accompanied by blurred vision for 7 months and numbness in his left face for 1 week was admitted to the Affiliated Hospital of Zunyi Medical University on May 7, 2022. One year ago, the patient had no obvious precipitating factor of paroxysmal stabbing pain in the whole skull with dizziness, which could be relieved by oneself after lasting for 1-2 min each time, with about 20 episodes per day. The cranial magnetic resonance imaging revealed changes in bilateral frontal lobe ischemic foci, bilateral frontal, ethmoid, sphenoid and maxill

IgG4-Related Chronic Sinonasal Pseudotumor with Refractory Nasal Bleeding: A Case Report.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic fibro-inflammatory and idiopathic autoimmune disorder. IgG4-RD can be characterized by the presence of pseudotumors. Inflammatory pseudotumors may involve any part of a human organ. There are few reports of sinus lesions in IgG4-RD. An 82-year-old man has a history of chronic sinusitis for the last several years and no remarkable family history. Two years before disease presentation, the patient experienced intermittent nasal bleeding, stuffy nose, dizziness, and fatigue. Blood test revealed positive (160X) antinuclear antibody with a m

Skull base reconstruction in the extended endoscopic transsphenoidal approach for suprasellar lesions.

OBJECT: The extended transsphenoidal approach to the suprasellar region has the advantages of minimal invasiveness and brain manipulation in the surgical treatment of small to medium lesions. At the same time, however, it carries a higher risk of postoperative cerebrospinal fluid (CSF) leakage and related complications than those for the standard transsphenoidal approach. Effective reconstruction of large skull base defects is a major concern in such extended approaches and remains challenging. METHODS: Between January 2004 and April 2006, 21 patients affected by different suprasellar lesions

[Pituitary abscess: report of two cases and a review].

Two cases of pituitary abscesses are reported. The first case was a 22-year-old female who was admitted with 14 months history of intermittent fever and headache followed by amenorrhea and polyuria. Plain craniograms showed no remarkable changes on the sella turcica. Direct coronal CT demonstrated a cystic lesion with a ring-like enhanced capsule in the pituitary complex with the apparent bulging of the pituitary top surface. On dynamic CT, the cystic lesion showed no remarkable increase in density. Transsphenoidal surgery was performed and a purulent materials was drained by cutting the under

Anatomical Features and Morphometric Characteristics of the Sphenoidal Sinus in MRI Studies.

Differential access to pathological sellar processes and adjacent regions is determined by the anatomic structures identified through diagnostic imaging. Both direct endonasal access (microscopic or endoscopic) and sublabial access utilize the sphenoid sinus (SS) as the primary surgical pathway. Critical factors include the pneumatization of the sinus, its intermediate septa, and the presence of a double wall, consisting of a connective tissue membrane along the dorsal wall of the SS. The present study aims to demonstrate the significance of the size and type of the SS based on MRI measurement

Direct carotid-cavernous fistula after functional endoscopic sinus surgery - A case report.

INTRODUCTION AND IMPORTANCE: Direct carotid-cavernous fistulas (CCF) are primarily caused by head trauma. Some cases have also been attributed to iatrogenic injuries during endovascular procedures. However, the reports of functional endoscopic sinus surgery (FESS) associated with direct CCFs are extremely rare. PRESENTATION OF CASE: A 52-year-old male worker, who suffered from chronic sinusitis and underwent functional endoscopic sinus surgery (FESS) performed by an otolaryngologist. Intra-operative finding indicated a left sphenoid sinus wall injury without internal carotid artery bleeding,

Wooden Foreign Body in the Skull Base: How Did We Miss It?

BACKGROUND: Timely detection of intraorbital and skull base wooden foreign bodies is crucial. Wooden foreign bodies are difficult to detect on imaging. The radiologist may fail to identify wooden foreign bodies on two thirds of initial scans and can miss them in almost one third of total cases. CASE DESCRIPTION: A 66-year-old woman sustained a penetrating injury through the left upper eyelid with a small tree branch. The branch was immediately removed in the field, and she was provided with prompt medical care at a local hospital. Initial computed tomography (CT) scan diagnosis was "posttraum

Anatomical considerations in acromegalic patients: A multicentric cohort study.

BACKGROUND: Changes in craniofacial morphology due to elevated levels of growth hormone (GH) vary according to ethnic groups studied in the international literature. We aimed to evaluate the radiologic and endoscopic anatomical variations in acromegalic patients, in comparison with non-functioning pituitary adenomas (NFPA) in the Mexican population. METHODS: One hundred and fifteen patients with endocrinologically and histologically confirmed diagnosis of GH-secreting pituitary adenoma (PA) and NFPA, treated only by endoscopic endonasal approach, in ten different institutions in Mexico, were
