Group B streptococci (GBS) cause a range of invasive maternal-fetal diseases during pregnancy and post-partum. However, invasive infections in non-pregnant adults are constantly increasing. These include sepsis and streptococcal toxic shock syndrome, which are often complicated by systemic coagulation and thrombocytopenia. GBS express a hyper-hemolytic ornithine rhamnolipid pigment toxin with cytolytic and coagulatory activity. Here, we investigated the effects of GBS pigment on human platelets. Infections of platelets with pigmented GBS resulted initially in platelet activation, followed by n

Identification of glycan functional epitopes is of paramount importance for rational design of glycoconjugate vaccines. We recently mapped the structural epitope of the capsular polysaccharide from type III Group B Streptococcus (GBSIII), a major cause of invasive disease in newborns, by using a dimer fragment (composed of two pentasaccharide repeating units) obtained by depolymerization complexed with a protective mAb. Although reported data had suggested a highly complex epitope contained in a helical structure composed of more than four repeating units, we showed that such dimer conjugated

Group B Streptococcus (GBS) is the leading cause of neonatal disease worldwide, and invasive disease in adults is becoming more prevalent. Currently, some countries adopt an intrapartum antibiotic prophylaxis regime to help prevent the transmission of GBS from mother to neonate during delivery. This precaution has reduced the incidence of GBS-associated early-onset disease; however, rates of late-onset disease and stillbirths associated with GBS infections remain unchanged. GBS is still recognized as being universally susceptible to beta-lactam antibiotics; however, there have been reports of

During 2015-2016, a total of 3,156 episodes of invasive group B Streptococcus (iGBS) infection in adults (>15 years of age) were recorded in England, corresponding to an annual incidence of 3.48/100,000 population. iGBS incidence was highest in older patients and women of childbearing age. The 493 pregnancy-related iGBS episodes correspond to a rate of 1.34/10,000 live births. In adults up to 60-69 years of age and in pregnant women, iGBS incidence increased with higher levels of socioeconomic deprivation. Hospital admissions associated with iGBS were predominantly emergency admissions (73% [2

Ampicillin is frequently used in neonates for early- and late-onset group B streptococcal (GBS) disease. In 2019, the American Academy of Pediatrics (AAP) published guidelines for GBS which included updated dosing recommendations for ampicillin for bacteremia and provided specific dosing recommendations for meningitis. The dosing recommendations in the guidelines were based off the 2018 Report of the Committee on Infectious Diseases (i.e., Red Book), which differed from the 2015 Red Book. For bacteremia, no dosing changes were recommended for ampicillin dosing in neonates ≤ 7 days of postnatal

A 34-year-old woman presented to the Emergency Room (ER) with an acute presentation of septic shock that required fluid and pressor support in the Intensive Care Unit. History revealed this was her third episode of such a presentation with asymptomatic periods in between. She responded well to medical interventions but reported persistent joint pain. Immunologic workup revealed her diagnosis.

We determined temporary changes in group B Streptococcus antimicrobial susceptibility and serotype distribution from perinatal strains. We examined invasive microbiological isolates from neonates with early-onset group B streptococcal disease (n = 14), and colonized isolates from those born uneventfully (n = 55) and from the genital tracts of pregnant and puerperal women (n = 198), collected between 1999 and 2009. All isolates were susceptible to penicillin. No significant differences were seen in susceptibility of 12 antimicrobial agents examined between invasive and colonized isolates. MIC50

Dr Rubens is a pediatric infectious disease specialist and microbiologist, with a longstanding interest in the molecular pathogenesis of bacterial perinatal infections by Gram-positive bacteria. Group B streptococci (GBS) have been shown to invade eukaryotic cells and this process may be the main mechanism used by the organism to cause disease in neonates. Molecular approaches are used to identify the genetic and biochemical basis of specific virulence traits, such as epithelial/endothelial cell entry and transcytosis, evasion of innate immune mechanisms by inhibiting complement activation and

Group B streptococci are a major cause of serious infections in the newborn, and also cause chorioamnionitis, late abortions, puerperal infections and sepsis. Group B streptococci are harboured in the genital tract of 5-40% of pregnant women, and 50-70% of their infants are colonized. Severe perinatal infections caused by group B streptococci affect 1-5 infants per 1,000 live births. American Academy of Pediatrics (AAP) have issued guidelines for preventing group B streptococcal infections. These are described, and it is suggested that these should be adopted in Norway.

GBS is the leading cause of perinatal bacterial infections in the United States. Selective intrapartum chemoprophylaxis can prevent GBS early-onset neonatal disease and reduce maternal puerperal morbidity. There does not appear to be an effect on late-onset neonatal disease. Protocols for intrapartum antibiotic chemoprophylaxis based on antepartum cultures, testing at parturition, and clinical risk factors have been summarized. While screening of all pregnant women for GBS is an option, screening and selection methods need further development. A highly reliable rapid screening test for intrapa

Eight cases of neonatal gallbladder distension are described. Group B streptococcal sepsis (5 infants) of suspected sepsis (3 infants) was present and probably played an aetiological role in the development of a distended gallbladder. Two infants required surgery because of persistent gallbladder enlargement and rising levels of bilirubin. Five responded to vigorous medical management and one died from sepsis and pneumonia. The need for conservative early management is stressed.

The bactericidal capacity of mononuclear and polymorphonuclear phagocytes obtained from normal newborn infants and from healthy adults was evaluated in vitro, using two group B beta-hemolytic streptococci (GBBHS) serotypes (GBBHS-Ia-SS-615/28 and GBBHS-III-SS-620/50) and uniform opsonic conditions. No intertype differences in bacteriolysis of these two serotypes were observed among leukocytes from newborns or adults. As group, only polymorphonuclear phagocytes from newborns disclosed a significantly lower mean bactericidal capacity than their adult cellular counterpart, and only with respect t
