Assessing the risks and benefits of benzodiazepines for anxiety disorders in patients with a history of substance abuse or dependence.

In this article, the authors reevaluate the traditional position that benzodiazepines should be avoided in anxiety disorder patients with a history of substance abuse or dependence. The efficacy of benzodiazepines in each of the anxiety disorders is reviewed, as are their side effects and toxicity. The definitions of benzodiazepine abuse and dependence are discussed, and relevant animal, experimental, and clinical data are reviewed and analyzed. A manual and computerized (MEDLINE) search was performed from 1966 to the present to examine the English-language literature published on benzodiazepi

"A state bordering on insanity"?: identifying drug addiction in nineteenth-century Canadian asylums.

This article examines the growing awareness of drug addiction as form of mental illness in several Canadian lunatic asylums in the last half of the nineteenth century and the beginning of the twentieth. Whereas in the 1870s and 1880s, medical and reform associations formed to cure and treat addiction and inebriety, asylum evidence suggests that it was not until the turn of the century that drug habituation was considered a condition which merited admission to asylum. Prior to the turn of the century, drug use appeared in the psychological profile of asylum entrants only as an attendant conditi

Substance use disorders in an adolescent inpatient psychiatric population.

This study examined the comorbidity of substance use disorders and other psychiatric disorders in adolescent populations. The study population was comprised of 100 consecutive admissions, ages 13 to 17, to an acute care adolescent psychiatric inpatient unit for substance use disorders. Patients were assessed using the Personal Experience Screening Questionnaire (PESQ) and the substance-use disorder portion of the Structured Clinical Interview for DSM III-R (SCID-R). Thirty-three (33%) patients were identified as having a substance abuse or dependence diagnosis. There was no significant differe

Anxiety, depression, and substance use disorders: diagnosis, treatment, and prescribing practices.

Attending physicians routinely encounter patients with signs and symptoms of anxiety and mood disorders as well as psychoactive substance use and psychoactive substance-induced organic mental disorders. These symptoms may represent either primary disorders or pathology that is secondary to other disorders. This article describes some of the relationships between substance use disorders and symptoms of anxiety and depressive disorders. In addition, some patients with these disorders may have a concurrent substance use disorder or be at high risk for developing one. Routine treatment of anxiety

Discrepancies between patient report, clinical assessment, and urine analysis in psychiatric patients during inpatient admission.

Self-report and clinical assessment of substance use were compared with urine analysis results in 56 male patients consecutively admitted for inpatient psychiatric treatment. All subjects received DSM-III-R Axis I diagnosis and were classified into diagnostic groups. Urine samples were tested for cocaine, marijuana, opiates, phencyclidine (PCP), amphetamines, and barbiturates. Thirty-five of the 56 patients (62%) produced urine samples that were positive for at least 1 substance of abuse. Of this group, 15 patients (27% of total sample) denied substance use during the week prior to admission.

Clinical relevance of endorphins in psychiatry.

1. 13 years after the discovery of endorphins, the relationships between these peptides and mental illness remain unclear. 2. Current concepts are controversial and convincing hypotheses have not been elaborated yet. 3. Endorphin measurements in CSF or serum of psychiatric patients did not strongly support that schizophrenia or depression is related to a deficit or an excess of these peptides. Moreover, treatment with endorphins or opiate antagonists did not result in a distinct change of psychiatric symptoms. Recently, research focused on the relationship between endorphins and addictive beha

Corticosteroid abuse--an unusual manifestation of drug dependence.

A depressed 44-year-old man presented with a 2 1/2-year history of use of a synthetic glucocorticoid (prednisone) as the only psychoactive drug. The pattern of use strongly suggested drug dependence; secondary Cushing's syndrome was noted. We call attention to the potential abuse of neuroendocrine agents and, more specifically, corticosteroid preparations.

Drug detection in traffic-related fatalities in Connecticut (2020-2024).

Motor vehicle fatalities continue to be a leading cause of preventable death in the United States. Beginning in 2019, there was a steady uptick in traffic fatalities nationally, which peaked in 2021. The state of Connecticut has experienced similar trends with increases in traffic fatalities over the last several years. The focus of this review was to evaluate drug positivity in traffic-related fatalities that occurred in Connecticut between January 1, 2020 and December 31, 2024. Cases related to traffic fatalities were identified by the state's Office of the Chief Medical Examiner. Deaths of

Validation and application of a method for the quantification of 137 drugs of abuse and new psychoactive substances in hair.

INTRODUCTION: In the dynamic universe of new psychoactive substances (NPS), the identification of multiple and chemically diverse compounds remains a challenge for forensic laboratories. Since hair analysis represents a gold-standard to assess the prevalence of NPS, which are commonly detected together with classical drugs of abuse (DoA), our study aimed at developing a wide-screen method to detect and quantify 127 NPS and 15 DoA on hair. MATERIALS AND METHODS: A multi-analyte ultra-high performance liquid chromatography mass spectrometry method for the identification and quantification of 12

[Detoxification after use of new psychoactive substances purchased online].

BACKGROUND: The knowledge base on new psychoactive substances (NPS) is generally limited. This introduces new challenges and increased unpredictability in substance abuse treatment. CASE PRESENTATION: A man in his thirties was submitted to detoxification after reportedly using flubromazolam, a high potency designer benzodiazepine, which he had purchased on the dark web. Extensive drug testing of serum, urine and hair, and the remains in a dropper bottle delivered by the patient, did not reveal flubromazolam or possible metabolites, but did reveal several common drugs of abuse, and 8-aminoclon

Drug Addiction and Incarceration: A Call for Research and Transparency Among Prison-Based Substance Abuse Treatment Programs.

As drug-related offense and illicit drug overdose rates continue to grow in the United States, criminologists have begun to pay more attention to factors influencing illicit drug use as well as effective methods of promoting drug abstinence in treatment programs across the nation. Although much scholarly attention is given to community-based substance abuse treatment programs, a considerably smaller focus of research is devoted to substance abuse treatment programs that are prison-based. Moreover, some of the most effective methods of treating inmates who are addicted to an illicit drug (such

GHB: a life-threatening drug complications and outcome of GHB detoxification treatment-an observational clinical study.

BACKGROUND: GHB (gammahydroxybutyrate) and its precursors are popular recreational drugs due to their sedative, anxiolytic and sexually stimulating effects. Their use has been steadily increasing in recent years. The detoxification process is complex and prone to high rates of complications while little is known about the pathophysiology. This study aims to elucidate the characteristics of GHB-addicted patients and to evaluate the risks and complications of GHB withdrawal treatment. METHODS: This observational study describes prospectively the socioeconomic status, clinical history and course

Seizure Occurring During Baclofen Monotherapy for Phenibut Withdrawal.

OBJECTIVE: Phenibut is a widely available gamma-aminobutyric acid B receptor agonist. When taken chronically, phenibut causes dependence and subsequent withdrawal if stopped. Baclofen, a gamma-aminobutyric acid B receptor agonist structurally related to phenibut, has been used to manage phenibut withdrawal (PW), although baclofen doses for PW are not well defined and may exceed Food and Drug Administration-approved doses. Little data described outcomes from baclofen use. METHODS: This case details a patient who experienced a seizure while on baclofen 10 mg thrice daily as monotherapy for PW a
