Treatments for refractory glaucoma include trabeculectomy, in which a filtering bleb is created to reduce aqueous pressure. Mitomycin C (MMC) is often used as an adjuvant to reduce post-trabeculectomy bleb scarring and consequent failure. However, scarring sometimes still occurs. Thus, we searched for more effective trabeculectomy adjuvants with high-throughput screening (HTS) of a library of 1,165 off-patent drug compounds. This revealed that amsacrine (AMSA), a DNA topoisomerase II (TOP2) inhibitor, was the top candidate. Compared to MMC, rabbits that underwent trabeculectomy with 10% AMSA h…

We compared the effect of seton (White pump shunt) surgery (16 eyes) with that of trabeculectomy and 5-fluorouracil (31 eyes) in treating 38 Asian patients with medically uncontrollable neovascular glaucoma. We found the probability of long-term success (intraocular pressure < or = 26 > or = 5 mm Hg) of seton surgery (53.0% at 3 years) to be similar to that obtained following trabeculectomy with adjunctive 5-fluorouracil (45.4% at 3 years). However, 3 years postoperatively, the probability of the preservation of visual acuity was significantly greater following trabeculectomy than seton surger…

Sturge-Weber syndrome (SWS) is a rare congenital neurocutaneous disorder that causes congenital glaucoma. Previous experiences have shown that drainage procedures are often required to control associated glaucoma. The conventional surgical approach in trabeculectomy carries a significant risk of intraoperative expulsive hemorrhage. Here, we describe a modified approach of the conventional trabeculectomy technique, which may lower the risk of expulsive hemorrhage. A viscoelastic device was employed to maintain a steady intraocular pressure throughout the procedure. Details of the surgical techn…

The Collaborative Initial Glaucoma Treatment Study (CIGTS) was initiated to evaluate whether local medical therapy or trabeculectomy is the better initial treatment for patients with newly diagnosed open-angle glaucoma. Visual field progression, intraocular pressure, and visual acuity were to be monitored for a period of 5 years. A total of 607 patients were randomly distributed among the two groups and controlled at 6-month intervals. At the same time the investigators tried to record quality of life using a questionnaire. Both trabeculectomy and medical therapy were able to lower intraocular…

We document a case of unilateral hemiarcus lipoides, which developed de novo 10 months after trabeculectomy in the left eye of a 54-year-old woman. The involved eye showed heterochromia complicata Fuchs and had undergone intracapsular cataract extraction 11 years and trabeculectomy one year earlier. Except for intermittent hypercholesterolemia, the patient did not present any signs of systemic disease. The possible roles of relative ocular hypotony and modified aqueous circulation after trabeculectomy in the development of unilateral hemiarcus lipoides are discussed.
