Sleep apnea in acromegaly.

In patients with acromegaly obstruction of the upper airway may develop due to enlargement of the tongue and thickening of the tissues of the larynx. The sleep apnea syndrome may develop in patients with upper airway obstruction from other causes. We studied a somnolent patient with acromegaly in whom we documented the sleep apnea syndrome. Endoscopy, while the patient was sleeping, revealed that periodic obstruction during sleep occurred because the large tongue bulged into the pharynx and obliterated the pharyngeal airway. Tracheostomy was followed by the disappearance of the somnolence. The

Congenital central hypoventilation syndrome: a case report.

Congenital central hypoventilation syndrome (CCHS) is a relatively rare, life-threatening, and lifelong multisystem disorder characterized b … before the diagnosis of CCHS was made. The diagnosis was confirmed by a PHOX2B sequence analysis. A tracheotomy was performed and the infant was discharged home on a home ventilator with outpatient follow-up. The clinical presentation of CCHS, as well as diagnosis and treatment strategies, is reviewed.

[Relapsing polychondritis with laryngotracheal extension: therapeutic approach in one patient].

OBJECTIVE: We report the case of a 32-years-old woman suffering from relapsing polychondritis in a laryngeal localization leading to dyspnea due to stenosis or collapse. METHODS: Incomplete efficiency of medical treatment required endoscopic insertion of a laryngotracheal silicone prosthesis (Trachobronxane ST, Novatech) which avoided tracheotomy and allowed better upper airway patency. Clinical and endoscopic follow-up was continued for several months. RESULTS: The prosthesis restored laryngotracheal patency and allowed moderate physical activity. The follow-up endoscopic explorations showe…

Phenytoin-induced lingual tonsil hyperplasia causing laryngeal obstruction.

The effects of phenytoin sodium on lymphoid tissue have been known for some time. However, its effect on the lymphoid tissue of Waldeyer's ring is unreported in the otolaryngology literature. We present the case of a 78-year-old woman who was treated with phenytoin for generalized tonic-clonic seizures for 4 months and who subsequently developed lingual hyperplasia that caused laryngeal obstruction eventually requiring a tracheotomy. Of note, no history of airway compromise or difficulty with intubation from a prior operation 20 years ago was reported; these findings suggest that the lingual t…

Diffuse idiopathic skeletal hyperostosis causing obstructing laryngeal edema.

Diffuse idiopathic skeletal hyperostosis (DISH), or ankylosing hyperostosis ("Forestier's disease"), is an ossifying diathesis of unknown et … eophytes led to edema of the laryngeal inlet and consequent severe dyspnea, necessitating emergency tracheotomy. Surgical excision of the osteophytic masses resulted in relief of symptoms. Symptomatology, radiographic features and individual treatments are discussed, with the latter dependent on clinical symptoms.

Supracricoid laryngectomy with cricohyoidoepiglottopexy: a partial laryngeal procedure for glottic carcinoma.

The partial horizontal supracricoid laryngectomy with cricohyoidoepiglottopexy consists of resection of the whole thyroid cartilage and paraglottic space. The cricoid cartilage, the hyoid bone, most of the epiglottis, and at least one arytenoid cartilage are conserved. Thirty-six patients with squamous cell carcinoma of the glottis who underwent this procedure from 1974 through 1986 are presented. All 36 recovered physiologic deglutition and phonation. None required a permanent tracheotomy. The 3-year actuarial survival rate was 86.5%. The local recurrence rate was 5.5%. The indications for th…
