Improved referral and survival of newborns after scaling up of intensive care in Suriname.

BACKGROUND: Scaling up neonatal care facilities in developing countries can improve survival of newborns. Recently, the only tertiary neonatal care facility in Suriname transitioned to a modern environment in which interventions to improve intensive care were performed. This study evaluates impact of this transition on referral pattern and outcomes of newborns. METHODS: A retrospective chart study amongst newborns admitted to the facility was performed and outcomes of newborns between two 9-month periods before and after the transition in March 2015 were compared. RESULTS: After the transiti

Healthcare transition in patients with rare genetic disorders with and without developmental disability: neurofibromatosis 1 and Williams-Beuren syndrome.

There are between 5,000 and 8,000 distinct rare diseases (RDs) affecting 6-8% of the population, most of which are caused by genetic defects. Many are highly complex, childhood-onset, multi-system disorders that are often associated with developmental disability, and require lifelong, highly specialized care and support. As larger numbers of children with previously fatal RDs survive into adulthood, they encounter significant challenges in transitioning from family-centered, developmentally focused, multidisciplinary pediatric care to a less supportive adult healthcare system that is often unf

The after discharge care management of low income frail elderly (AD-LIFE) randomized trial: theoretical framework and study design.

Interdisciplinary care management is advocated for optimal care of patients with many types of chronic illnesses; however, few models exist that have been tested using randomized trials. The purpose of this report is to describe the theoretical basis for the After Discharge Management of Low Income Frail Elderly (AD-LIFE) trial, which is an ongoing 2-group randomized trial (total n = 530) to test a chronic illness management and transitional care intervention. The intervention is based on Wagner's chronic illness care model and involves comprehensive posthospitalization nurse-led interdiscipli…

A meta-analysis and systematic review of the use of transition tools for patients transitioning from paediatric to adult rheumatology services.

OBJECTIVES: Transitioning from paediatric to adult care can be challenging, but transition tools are designed to increase successful rates of transfer. We aimed to conduct a meta-analysis and systematic review of the use of transition tools in the transfer of care from paediatric to adult services for rheumatology patients. METHODS: An extensive literature search was conducted using MEDLINE, PubMed and Embase. A total of 12 papers were included in the systematic review and 8 in the meta-analysis looking at the use of transition tools in successful follow-up in adult clinics. RESULTS: The met

Initial outcomes in a sickle cell disease transition clinic.

BACKGROUND: Transition in sickle cell disease (SCD) is associated with an alarming increase in acute care utilization, cost, and risk of early mortality. Effective transition preparation is crucial to address these issues. We established a multidisciplinary transition clinic at our urban SCD center in the fall of 2021. At each visit patients were introduced to the transition process and met with a pediatric and adult SCD provider. METHODS: We reviewed charts of patients attending the clinic from September 2021 to May 2023. Data were collected on the number of visits in the transition and adul

Delphi consensus finding on paediatric-adult transition: results from the epilepsy transition working group of the italian league against epilepsy (LICE).

PURPOSE AND RATIONALE: The care for paediatric and transitional-age epilepsy patients has expanded significantly, addressing the diverse needs of patients with self-limiting to lifelong conditions. Approximately one-third of patients with childhood-onset epilepsy remain dependent on parental care, with transition influenced by factors such as seizure frequency, drug resistance, comorbidities, and developmental disabilities. The Italian League Against Epilepsy (LICE) initiated a Delphi consensus to establish common guidelines for effective transition practices. METHODS: The consensus process i

Passing the baton: The pediatric surgical perspective on transition.

BACKGROUND/PURPOSE: Transition of care from pediatric to adult providers is garnering intense national interest in pediatrics but is largely driven by nonsurgeons. We sought to describe the preferences of pediatric surgeons and their current transition practice patterns for various populations and wondered whether gender, experience, or practice setting affected surgeons' perspectives. METHODS: All American Pediatric Surgical Association (APSA) members were surveyed electronically. Responses were voluntary and confidential. RESULTS: Survey response rate was 22%. Most surgeons (95%) treated p

Unpaid caregivers' process of collaborating with others during older adult hospital-to-home transitions: A grounded theory study.

Unpaid/family caregivers provide support critical to older adult hospital-to-home transitions, but lack time and preparation. There is limited evidence regarding important collaboration for caregivers during the transition. The objective was to examine caregivers' process of collaborating with others, including other family members, healthcare professionals, and community, social, and professional networks, during older adult hospital-to-home transitions. This study utilized grounded theory methodology. One-on-one interviews were conducted with unpaid/family caregivers of an older adult during

Adaptation of Social Health Care Tools From Paper-Based to Web-Based Formats: Scoping Review.

BACKGROUND: The transition of social health care tools from paper-based to web-based formats is increasingly common in social and health care settings. While digital delivery may improve accessibility, flexibility, and scalability, such transitions require careful adaptation of content, design, and workflows. However, evidence on how these transitions are carried out in practice, and which challenges and strategies are involved, remains fragmented. OBJECTIVE: This scoping review aimed to map how social health care tools have transitioned from paper-based to web-based formats and to synthesize

Initial linkage and subsequent retention in HIV care for a newly diagnosed HIV-infected cohort in Denver, Colorado.

This is a retrospective cohort study of 352 newly diagnosed HIV-infected individuals in Denver, from 2005 to 2007. Utilizing data from 3 health care systems, 2 clinical trials units, and statewide Colorado HIV laboratory reporting databases, we tracked initial linkage to HIV care, retention in care, loss to follow-up, and transitions between HIV care providers. After more than 2.6 years of follow-up, 256 (73%) individuals linked to HIV care within 180 days. Of the 301 individuals who eventually linked to care, 168 (56%) had at least one 180-day gap in care, while 49 (16%) had a 360-day gap. Tr

Hypochloremia and Diuretic Resistance in Heart Failure: Mechanistic Insights.

BACKGROUND: Recent epidemiological studies have implicated chloride, rather than sodium, as the driver of poor survival previously attributed to hyponatremia in heart failure. Accumulating basic science evidence has identified chloride as a critical factor in renal salt sensing. Our goal was to probe the physiology bridging this basic and epidemiological literature. METHODS AND RESULTS: Two heart failure cohorts were included: (1) observational: patients receiving loop diuretics at the Yale Transitional Care Center (N=162) and (2) interventional pilot: stable outpatients receiving ≥80 mg furo…

Challenges and barriers in pediatric to adult Transition in Chronic Pulmonary Diseases (CPD) in Italy: a nationwide Survey.

BACKGROUND: Advances in medical research, early diagnosis, and therapeutic strategies have significantly improved survival among children with chronic pulmonary diseases (CPDs), leading to a growing population of adolescents and young adults transitioning to adult care. Despite this demographic shift, healthcare systems, including Italy's, remain inadequately equipped to manage the evolving needs of these patients. This study aims to identify key barriers and unmet needs within the Italian healthcare system regarding the transition from pediatric to adult care for individuals with CPDs. METHO
