A Statewide Assessment of Rib Fixation Patterns Reveals Missed Opportunities.

BACKGROUND: Rib fractures are a common consequence of traumatic injury and can result in significant debilitation. Rib fixation offers fract … e of pulmonary complications had a similar distribution. The frequency of rib fixation rates within trauma centers was not associated with rib fracture patient volume, and 37.1% of multiple rib fracture patients were cared for at centers that did not perform rib fixation. CONCLUSIONS: Rib fixation is infrequently used at trauma centers in Pennsylvania. It is us …

Current management of severe pelvic and perineal trauma.

Mortality associated with pelvic and perineal trauma (PPT) has fallen from 25% to 10% in the last decade thanks to progress accomplished in medical, surgical and interventional radiology domains (Dyer and Vrahas, 2006) [1]. The management strategy depends on the hemodynamic status of the patient (stable, unstable or extremely unstable). Open trauma requires specific treatment in addition to control of bleeding. All surgical centers can be confronted some day with patients with hemorrhagic PPT and for this reason, all surgeons should be familiar with the initial management. In expert centers, m

Implementing a Trauma Hybrid Operating Room from an Interventional Radiology Perspective.

Hybrid operating rooms have been increasingly adopted in trauma centers to enable integrated surgical and endovascular care in hemorrhagic emergencies, but practical guidance on their operational implementation from the interventional radiology perspective remains limited. We describe our experience implementing a trauma hybrid operating room at an American College of Surgeons-verified Level I trauma center, addressing room design, zoning for rapid transitions between laparotomy and angioembolization, a mobile supply cart, and a pre-embolization checklist. We outline a training approach center…

The Santa Monica crash: an urban multicasualty event.

Mass casualty events provide dramatic challenges for trauma centers and trauma systems. We analyzed the management of victims and assessed the response of the UCLA Healthcare System to the Santa Monica multicasualty event of July 16, 2003, when an elderly man drove his car through a crowded outdoor market and injured 73 people, 10 of whom died (eight at the scene). Of the victims, 26 were treated at UCLA (n = 15) and Santa Monica (n = 11) Medical Centers. Fourteen patients (54%) were female; average age was 41.9 years (range 7 months to 88 years). Fifteen patients were treated in the ER only, …

Current issues in the diagnosis of pediatric cervical spine injury.

Cervical spine injury in pediatric trauma occurs rarely; however, there is significant potential for considerable morbidity when it does occur. Screening for cervical spine injuries has been shown to be most sensitive in adult trauma centers when combined with reliable physical examination findings. Because pediatric trauma patients suffer from a different range of injuries than adults, and often are not reliable due to age limitations or associated head injury, the same strategies employed in adult trauma do not always hold true in children. We look at the differences in adult and pediatric c…

[The epidemiology of multiple trauma].

Trauma is the leading cause of death for people up to 40 years of age in Germany. Most of the patients were injured in traffic accidents whe … and reintegration. The number of preventable trauma deaths can be significantly reduced in special trauma centers, and the high costs could be damaging for smaller hospitals. A plea is made for more preventive measures and legislative changes to reduce the number of traffic accidents. The medical care of trauma patients should be in special trauma centers.

The A+ criteria for pediatric blunt cerebrovascular injury: An ATOMAC+ multicenter study.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is rare but significant among injured children. Current BCVI screening criteria lack adequate diagnostic accuracy for pediatrics. This preplanned secondary analysis identified common risk factors for BCVI among a multicenter cohort of pediatric trauma centers within the ATOMAC+ Pediatric Trauma Research Network (APTRN) and derived a new set of screening criteria. METHODS: A prospective, multi-institutional observational study of children <15 years old who sustained blunt trauma to the head, face, or neck (AIS > 0) who presented at one of six lev…

The reconstructive burden of inter-hospital transfers: resource utilization and clinical implications in tertiary trauma care.

INTRODUCTION: Despite the presence of well-established trauma centers, many trauma patients are transferred due to unresolved wound complications. This often-overlooked issue highlights the critical need for specialized wound management and the essential role of plastic surgery (PS) in regional trauma centers. This study aims to emphasize the importance of effective wound management in trauma patients and examine the clinical outcomes of interhospital transfers. METHODS: We performed a retrospective chart review of 92 trauma patients transferred to a regional trauma center from January 2020 t…

Body temperature as a predictor of mortality in multiple trauma patients: aprospective single-centre cohort study.

Trauma is the leading cause of death among working-age people. Individuals who have experienced trauma are more susceptible to developing accidental hypothermia. This condition has been demonstrated to be associated with poor clinical outcomes. The primary objective of this study was to identify the association between body temperature (BT) and mortality in patients with multiple trauma. A secondary objective was to ascertain whether this association remained significant in patients with body temperature < 36 °C. A prospective cohort study was conducted in a level-3-trauma centre in Barcelona,

Pediatric trauma transfer patients have low rates of additional traumatic injuries.

BACKGROUND: Pediatric trauma patients often require interfacility transfer for subspecialty management of presumed isolated injuries. Understanding the frequency of additional injury in these low-acuity patients may improve resource utilization. METHODS: Pediatric trauma patients transferred to a level 1 trauma center in 2019 were compared by type of presenting injury in a retrospective review. Primary outcome was additional traumatic injuries identified. RESULTS: 530 pediatric trauma patients were transferred, most commonly for an isolated orthopedic injury (56.5 %). The overall rate of ad

Bicycle helmets are associated with fewer and less severe head injuries and fewer neurosurgical procedures.

PURPOSE: This study explores the protective capabilities of bicycle helmets on serious head injury among bicyclists hospitalized in a Norwegian level 1 trauma centre. METHOD: Information on helmet use, demographic variables, Abbreviated Injury Scale (AIS) and surgical procedure codes was retrieved from the Oslo University Hospital Trauma Registry for patients with bicycle-related injuries from 2005 through 2016. Outcomes were serious head injury defined as maximum AIS severity score ≥ 3 in the AIS region Head, any cranial neurosurgical procedure, and 30-day mortality. RESULTS: A total of 125

The impact of obesity on Singaporean trauma patients and their venous thromboembolism risk.

BACKGROUND: The impact of obesity on trauma outcomes had been equivocal and the majority of these studies were conducted in the Western population. The aim of our study is to evaluate the impact of obesity on morbidity and mortality in Singaporean patients after trauma. METHODS: A retrospective case control study of trauma patients admitted to a tertiary institution was performed. Patients admitted for both blunt and penetrating trauma who are aged 16 years and above were included. Propensity score matching (PSM) was performed to reduce selection bias. RESULTS: A total of 5239 patients were

Failure-to-rescue and mortality after emergent pediatric trauma laparotomy: How are the children doing?

INTRODUCTION: Emergent trauma laparotomy is associated with mortality rates of up to 40%. There is a paucity of data on the outcomes of emergent trauma laparotomies performed in the pediatric population. The aim of our study was to describe the outcomes, including mortality and FTR, among pediatric trauma patients undergoing emergent laparotomy and identify factors associated with failure-to-rescue (FTR). METHODS: We performed a one-year (2017) retrospective cohort analysis of the American College of Surgeons Trauma Quality Improvement Program dataset. All pediatric trauma patients (age <18 y

What a headache! Reviewing mild traumatic brain injury management in a new trauma service.

AIM: Mild traumatic brain injury (mild TBI) is a common, poorly managed condition with an underestimated impact and inadequate follow-up. This study aimed to assess local practice in terms of assessment and follow-up. METHODS: A retrospective review of all patients presenting to Christchurch Hospital between 1 August 2019 and 30 September 2019 with ICD-10 coded diagnosis of head trauma was conducted. Patients younger than 16 or older than 80 years who had a concurrent medical illness or who did not meet diagnostic criteria for mild TBI were excluded. This was to minimise diagnostic uncertaint

Decelerational thoracic injury.

BACKGROUND: Among the five major decelerational thoracic injuries [myocardial contusion (MC), traumatic aortic disruption (TAD), sternal fracture (SF), flail chest (FC), and tracheobronchial disruption (TBD)], coexisting injuries are seemingly rare. METHODS: To test this hypothesis, we reviewed the records of all patients, with final diagnosis (FDX) codes of these injuries, treated at our Level I trauma center for the 10 years preceding 1997. RESULTS: Among 142 patients, all victims of motor vehicle crashes, there were 38 MC, 36 TAD, 33 FC, 28 SF, and 7 TBD. There were six coexisting injurie

The motorcyclist: easy rider or easy victim? An analysis of motorcycle accidents in Germany.

This report reviews the findings from 86 motorcycle accidents during a 1-year period at the Trauma Center "Bergmannsheil" in Bochum, Germany. A study of the case histories supplemented by telephone conversations yielded the following results: 90.7% of the patients were men, and the average age was 28.8 years; most of the accidents occurred in the 25- to 30-year-old age group (27.9%). Motorcycle accidents happened mostly during recreational rides on weekends in the summertime. Although there was a high rate of helmet use (98.8%), the head region was affected in 12 victims. Two patients died bec

A National Comparison of Volume and Acuity for Adult and Pediatric Trauma: A Trauma Quality Improvement Program Cohort Study.

INTRODUCTION: Most injured children receive trauma care outside of a pediatric trauma center. Differences in physiology, dosing, and injury pattern limit extrapolation of adult trauma principles to pediatrics. We compare US trauma center experience with pediatric and adult trauma resuscitation. MATERIALS AND METHODS: We queried the 2019 Trauma Quality Improvement Program to describe the experience of US trauma centers with pediatric (<15 y) and adult trauma. We quantified blunt, penetrating, burn, and unspecified traumas and compared minor, moderate, severe, and critical traumas (ISS 1-8 Mino…

Surgical stabilization of rib fractures for flail chest: Analysis of center-based variability in practice and outcomes.

BACKGROUND: Given the lack of high-quality data on patient selection for surgical stabilization of rib fractures (SSRF), significant variability in practice likely exists across trauma centers. We aimed to determine whether centers with a more liberal approach to SSRF had improved outcomes. METHODS: We performed a retrospective cohort study of adult patients with flail chest admitted to Level I or II trauma centers participating in the American College of Surgeons' Trauma Quality Improvement Program. The primary outcome was hospital mortality; secondary outcomes included discharge status, tra…
