The approach to chronic pelvic pain in the adolescent.

Adolescents present to outpatient and acute care settings commonly for evaluation and treatment of chronic pelvic pain (CPP). Primary care providers, gynecologists, pediatric and general surgeons, emergency department providers, and other specialists should be familiar with both gynecologic and nongynecologic causes of CPP so as to avoid delayed diagnoses and potential adverse sequelae. Treatment may include medications, surgery, physical therapy, trigger-point injections, psychological counseling, and complementary/alternative medicine. Additional challenges arise in caring for this patient p

EGFR activation mediates inhibition of axon regeneration by myelin and chondroitin sulfate proteoglycans.

Inhibitory molecules associated with myelin and the glial scar limit axon regeneration in the adult central nervous system (CNS), but the underlying signaling mechanisms of regeneration inhibition are not fully understood. Here, we show that suppressing the kinase function of the epidermal growth factor receptor (EGFR) blocks the activities of both myelin inhibitors and chondroitin sulfate proteoglycans in inhibiting neurite outgrowth. In addition, regeneration inhibitors trigger the phosphorylation of EGFR in a calcium-dependent manner. Local administration of EGFR inhibitors promotes signifi

[Peri-anaesthetic anaphylactic shock: myths and facts].

Peri-anaesthetic anaphylactic shock is a fortunately rare event, which nevertheless often turns out to be life-threatening. Only fast recognition and reaction can permit to avoid its sometimes dramatic consequences. This implies, from the anaesthetist, good knowledge of the mechanisms, of the events'sequence, of the usual triggering drugs and, of course, of the anaphylactic shock's treatment. After such an accident has occured the patient should be referred to an allergy specialist in order to perform a thorough exploration, thus pointing out which drug is responsible for the reaction, and by

Idiopathic long QT syndrome with late onset of bradycardia-dependent and short-coupled variant of torsade de pointes.

We report an idiopathic variant of the long QT syndrome in a 72 year old woman with a history of recurrent syncope over 3 years who presented with repeated episodes of cardiac arrest due to bradycardia-dependent torsade de pointes (TdP), which continued long after possible triggers were eliminated. During these episodes a short coupling interval of the first tacycardia beat was also observed. The patient's stormy course was only controlled with pacing. She received a permanent dual-chamber pacemaker and prophylactically a beta-blocker was added. Over the subsequent 14 months she has remained c

Geospatial Correlation of Amyopathic Dermatomyositis With Fixed Sources of Airborne Pollution: A Retrospective Cohort Study.

Objective: Dermatomyositis (DM) may result from exogenous triggers, including airborne pollutants, in genetically susceptible individuals. The United States Environmental Protection Agency's 2011 National Air Toxics Assessment (NATA) models health risks associated with airborne emissions, available by ZIP code tabulation area (ZCTA). Important contributors include point (fixed), on-road, and secondary sources. The objective of this study was to investigate the geospatial distributions of DM and subtypes, classic DM (CDM) and clinically amyopathic DM (CADM), and their associations with airborne

Prevention of B cell antigen receptor-induced apoptosis by ligation of CD40 occurs downstream of cell cycle regulation.

Cross-linking of the B cell antigen receptor (BCR) on germinal center B cells can induce growth arrest and apoptosis, thereby eliminating potentially autoreactive B cells. Using the Burkitt lymphoma cell line Ramos as a model, we studied the commitment to apoptosis following growth arrest, as well as how triggering of CD40 or addition of tumor necrosis factor (TNF)-alpha can interfere to block cell death. Both BCR triggering and direct induction of growth arrest by sodium butyrate (n-But) caused hypophosphorylation of the retinoblastoma protein (pRb), followed by apoptosis. Interestingly, alth

Persistent Methicilin-Resistant Staphylococcus aureus Bacteremia: Resetting the Clock for Optimal Management.

A positive follow-up blood culture for methicillin-resistant Staphylococcus aureus (MRSA) while on seemingly appropriate therapy is a common and ominous development. However, the definition and management of persistent MRSA bacteremia is unstandardized. In this Opinion Paper, we identify the presence of bacteremia for > 1 calendar day as a "worry point" that should trigger an intensive diagnostic evaluation to identify metastatic infection sites. Next, we define the duration of MRSA bacteremia that likely constitutes antibiotic failure and outline a potential management algorithm for such pati

Melasma pathogenesis: a review of the latest research, pathological findings, and investigational therapies.

Melasma is an acquired hyperpigmentation disorder most commonly affecting females with darker skin types. It is triggered by several factors including sun exposure, genetic influences, and female sex hormones. The pathology of melasma extends beyond melanocytes and recent literature points to interactions between keratinocytes, mast cells, gene regulation abnormalities, neovascularization, and disruption of basement membrane. This complex pathogenesis makes melasma difficult to target and likely to recur post treatment. A better understanding of the latest pathological findings is key to devel

Cardiovascular safety of antihistamines.

Histamine is a mediator, which increases the permeability of capillaries during the early phase of allergic reaction, causes smooth muscle contraction of bronchi and stimulates mucous glands in the nasal cavity. Antihistamines are the basis of symptomatic treatment in the majority of allergic diseases, especially allergic rhinitis, allergic conjunctivitis, urticaria and anaphylaxis. The cardiotoxic effects of the two withdrawn drugs, terfenadine and astemizole, were manifested by prolonged QT intervals and triggering torsades de pointes (TdP) caused by blockade of the 'rapid' I Kr potassium ch

Fluid resuscitation in sepsis: reexamining the paradigm.

Sepsis results in widespread inflammatory responses altering homeostasis. Associated circulatory abnormalities (peripheral vasodilation, intravascular volume depletion, increased cellular metabolism, and myocardial depression) lead to an imbalance between oxygen delivery and demand, triggering end organ injury and failure. Fluid resuscitation is a key part of treatment, but there is little agreement on choice, amount, and end points for fluid resuscitation. Over the past few years, the safety of some fluid preparations has been questioned. Our paper highlights current concerns, reviews the sci

A case report of postulated 'Barré Liéou syndrome'.

The case history presented is of a 32 year old woman suffering with severe occipital and bilateral temporal pain together with autonomic disturbances affecting her vision, balance and breathing, symptoms which have been postulated as 'Barré Liéou syndrome'. She complained of pain referred to the left arm and associated circulatory and sensory disturbance in keeping with the diagnosis of complex regional pain syndrome type I. Traditional Chinese and Western trigger point acupuncture techniques were used in order to treat her pain and autonomic dysfunction. Acupuncture was successful in reducing

Spatial decoupling of recognition and signaling in aptamer switches via tri-strand intermolecular competition.

Despite decades of progress in aptamer engineering, the development of a single, modular signaling architecture capable of transducing target recognition into a measurable output across structurally diverse analytes remains an unmet challenge in biosensing. We report the Competitor Bridge Aptamer (CoBrA) framework: a modular DNA tridomain architecture that functionally isolates target recognition from signal transduction. Unlike conventional sensors where signal output depends on unpredictable aptamer folding, the CoBrA system uses an aptamer switch to trigger a predictable, concerted spatial

Hijacking client instability to selectively disrupt HSP90-driven glucocorticoid receptor activation.

The molecular chaperone HSP90 drives the folding and activation of a broad spectrum of client proteins through dynamic, transient multiprotein assemblies. Although HSP90 has been widely pursued as an anticancer target, ATP-competitive inhibitors can trigger indiscriminate client depletion and protective heat shock responses, limiting clinical utility. Here, we report a client-directed strategy to perturb HSP90-dependent maturation by targeting unstable client regions that act as transient recognition sites for chaperone networks. Using energy-decomposition analysis of the ligand-binding domain

How I Investigate Borderline Prolonged aPTT: An Integrated Laboratory Approach to Mixing Studies and Factor Analysis.

Borderline prolongation of routine clotting assays-particularly the activated partial thromboplastin time (aPTT)-is a common interpretative challenge in laboratory hematology. These reproducible but mildly prolonged results often lie just beyond the upper reference limit and can trigger unnecessary follow-up, delays, or misinterpretation. Conventional tools such as 1:1 mixing studies may appear corrective due to dilutional masking of weak inhibitors, residual anticoagulants, or low-grade lupus anticoagulant (LA) activity, especially in assays with limited sensitivity. In our laboratory, border

Taming Crystallization Kinetics via Side-Chain Steric Engineering Enables Over 21% Efficiency in Non-Halogenated-Solvent-Processed Organic Solar Cells.

The development of high-performance, non-halogenated organic solar cells (OSCs) is essential for sustainable commercialization. However, the high boiling points of halogen-free solvents often trigger excessive pre-aggregation of non-fullerene acceptors (NFAs) and unfavorable phase separation, severely limiting device performance. Herein, a side-chain steric engineering strategy is employed to develop a weakly crystalline NFA, TPD-Y, featuring a thieno[3,4-c]pyrrole-4,6-dione unit. The reduced crystallinity of TPD-Y effectively suppresses aggregation-caused quenching (ACQ), yielding a high phot

Of mice and men-The emerging oral-gut-brain axis of health and disease.

OBJECTIVES: Oral health's inextricable links to systemic health are highlighted by the emerging oral-gut-brain axis and other well-known axes. There is growing evidence of a complex oral-gut-brain axis linking mouth and gut microbiomes with the central nervous system. Axis disruptions, characterized as oral and gut dysbiosis or microbial imbalances, can trigger oral and systemic inflammation and neuroinflammation, contributing to diseases such as Alzheimer's disease and Parkinson's disease. MATERIALS AND METHODS: We summarize the oral-gut-brain axis mechanistic pathways, key evidence from hum

Time-resolved proteomic profiling of Cupriavidus metallidurans CH34 in the copper-induced viable-but-nonculturable state.

Copper-based materials are actively explored for their potential as antimicrobial agents. However, recent studies show that sublethal concentrations of Cu ions can induce the viable-but-nonculturable (VBNC) cell state in certain bacteria, hampering contamination control, and monitoring. In this study we contribute to the unravelling of this largely enigmatic phenomenon by determining the time-resolved proteome of Cu-treated Cupriavidus metallidurans CH34 during VBNC induction and resuscitation. High-throughput quantitative liquid chromatography tandem mass spectrometry (LC-MS/MS) analysis was

Long-Term Neural Recording Performance of PEDOT/CNT/Dexamethasone Coated Electrode Array Implanted in Visual Cortex of Rats.

Implantable neural electrode arrays can be inserted in the brain to provide single-cell electrophysiology recording for neuroscience research and brain-machine interface applications. However, maintaining signal quality over time is complicated by inflammatory tissue responses and degradation of electrode materials. Organic electrode coatings offer a solution by enhancing recording and stimulation capabilities, including reduced impedance, increased charge injection capacity, and the ability to incorporate and release anti-inflammatory drugs. In this study, acid-functionalized multi-walled car

Regularity of Conjugacies of Linearizable Generalized Interval Exchange Transformations.

UNLABELLED: Epstein-Barr virus (EBV) is a rare but well-known trigger for acute myositis. Diagnosis is primarily based on the clinical presentation and an accounting laboratory profile. Patients with mitochondrial dysfunction are potentially at a higher risk of myopathic exacerbations upon exposure to acute insults. This is due to the high energy requirements of myofibers and their reliance on sufficient mitochondrial performance. Hence, any mitochondrial insult can compromise the function of myofibers. This has implications on the management of people with pre-existing mitochondrial dysfuncti
