The main indication for using buccal mucosa in the urinary tract is in those who require complex secondary hypospadias surgery. Twenty-two children had inner lower lip mucosa onlay patch to reconstruct the urethra after 1 to 20 prior failed hypospadias/epispadias repairs. The mucosa was harvested from the lower inner lip by microsurgical dissection using optical magnification. The donor site was not sutured but was sealed with fibrin glue. The results observed in a series of 40 patients treated for urethral stricture by endoscopic resection of the callus after internal endoscopic urethrotomy (Guillemin's technique) are presented. The mean age of the patients was 57 years, with a follow-up of five years. The authors obtained 80% (32 patients) of very good results for urethral strictures less than 1 cm long. The modified Barcat technique was used to treat 168 patients with hypospadias during the last 5 years. The technique, which is applicable for distal hypospadias, uses apposing fully mobilized meatal based skin flaps. In a double-blind, randomised trial, 62 postmenopausal women with genito-urinary symptoms were treated with oestriol or matching placebo for 4 weeks. Urethral strictures resultant from hip fractures were examined and treated in 140 patients admitted to hospital with multiple attendant traumas: rupture of the bladder and abdominal organs, rib and limb fractures, renal injuries. In 10 cases of obstetric and surgical necrosis of the urethra and in one of congenital aplasia, the canal was reconstructed using as an intermediate layer the roots of the clitoris, freed and applied. In the ten cases in which treatment has been completed, continence was restored in 8, either with one operation only (3 cases), or with the addition of various procedures for suspension of the bladder neck using autografts (5 cases).

We have described two cases of urethro-rectal fistulae: outstanding by their absence of anorectal abnormality. This type of malformation must be separated from the fistula observed in imperforated anus. We consider it as an incomplete duplication of the urethra; the posterior part ending in the rectum: this is a very rare abnormality. The treatment involves a separation of the posterior urethra from the rectum to bring it up to the preneum then continue as in an hypospadias usig the Duplay and Leveuf (Cecil) techniques. Seemingly normal urethra is hypoplastic in part or entirely.
