Comparative ultrasonographic examination and measurements of the urethra and penis of castrated and intact male lambs.

Early castration of male small ruminants is regarded as a risk factor for urolithiasis, although the underlying correlations are still unclear. One possible reason is a deferred development of the penis and the urethra after castration. Therefore, we examined the penis and urethra of castrated and intact lambs by ultrasonography to determine the correlation between urethral area and pe- nile cross-sectional area. Ultrasonography was performed in 6-month-old Lacaune crossbred lambs (early castrated, late castrated, and intact; each group, n = 11). Sectional images at 5 loca- tions (glans penis,…

Squamous cell carcinoma of the renal pelvis presenting as an integumentary neoplasm of the flank: A case report.

RATIONALE: Although chronic pyelonephritis and urolithiasis are established risk factors for squamous cell carcinoma (SCC), only a minority of patients with chronic urolithiasis eventually develop SCC. It is believed that the chronic irritation leads to squamous cell metaplasia that may subsequently develop into SCC. Although studies show that SSC generally spreads locally with associated symptoms of lymphadenopathy, metastasis to the lungs and liver have also been reported. However, cases spreading to the flank have yet to be reported. Therefore, the use of reconstructive techniques for the r…

Hereditary xanthinuria is not so rare disorder of purine metabolism.

Hereditary xanthinuria (type I) is caused by an inherited deficiency of the xanthine oxidorectase (XDH/XO), and is characterized by very low concentration of uric acid in blood and urine and high concentration of urinary xanthine, leading to urolithiasis. Type II results from a combined deficiency of XDH/XO and aldehyde oxidase. Patients present with hematuria, renal colic, urolithiasis or even acute renal failure. Clinical symptoms are the same for both types. In a third type, clinically distinct, sulfite oxidase activity is missing as well as XDH/XO and aldehyde oxidase. The prevalence is no…

Risk Factors Associated with Urologic Intervention in Emergency Department Patients with Suspected Renal Colic.

BACKGROUND: Whereas most patients with urolithiasis pass their stones spontaneously and require only symptomatic management, a minority will require urologic intervention. OBJECTIVE: Our primary objective was to confirm previously reported risk factors and to identify additional predictors of urologic intervention within 90 days, for emergency department (ED) patients with suspected renal colic. METHODS: We conducted a prospective cohort study of adult patients presenting to one of two tertiary care EDs with suspected renal colic over a 20-month period. Multivariate logistic regression model…

Flexible ureteroscopy in children with von Willebrand disease.

BACKGROUND AND PURPOSE: Treatment options for urolithiasis include shockwave lithotripsy (SWL), percutaneous nephrostolithotomy (PCNL), and ureteroscopy. While these treatment options are becoming the standard of care in the pediatric population, children with a bleeding dyscrasia must be approached differently. We report our flexible ureteroscopic experience in children with von Willebrand disease (vWd). PATIENTS AND METHODS: A review of a prospectively maintained database of stone patients was performed. Children with a bleeding diathesis were identified. Stone burden, operative access, int…

Analysis of urinary calculi composition by infrared spectroscopy: a prospective study of 625 patients in eastern China.

Urolithiasis is a common urologic disease whose prevalence is about 1-20% and increasing throughout the world. The recurrence rate after treatment is more than 50%. Urinary stone analysis is important in determining the possible etiology and the pathophysiology of stone formation. A better understanding of the stone composition may help prevent urinary stone formation. From March 2007 to December 2008, physical analysis of urolithiasis in patients who lived in eastern China for more than 5 years and underwent surgery or shock wave lithotripsy in our hospital or passed their stones spontaneousl…

Five-year clinical experience with the Dornier Delta lithotriptor.

OBJECTIVES: Multiple studies support the clinical value of extracorporeal shock wave lithotripsy (ESWL) in the treatment of symptomatic urolithiasis. Although prior investigations have examined such efficacy among several commercially available lithotriptors, no published data exist to support the efficacy of the Dornier Delta lithotriptor. METHODS: A retrospective review of 4621 patients with urolithiasis treated with the Dornier Delta lithotriptor from 1999 through 2004 was undertaken. Data were obtained regarding stone location, stone size, shockwave usage, total success rate, and complica…

A Medical Rarity: Isolated Toxoplasma gondii Abscess in the Pituitary Gland of an Immunocompetent Patient.

The occurrence of sellar toxoplasmosis in the pituitary gland is exceedingly rare, and only a few reports have been published in the literature, primarily impacting immunocompromised patients. We report an intriguing case of a 54-year-old man with an initial asymptomatic panhypopituitarism diagnosed under investigation for urolithiasis. Cranial CT and MRI revealed a large sellar lesion first suspectable of a nonfunctioning pituitary macroadenoma. An endonasal transsphenoidal surgery was performed, and the histopathological diagnosis was surprisingly a pituitary Toxoplasma gondii abscess. This …

Retained foreign body post-PCNL: an unusual complication and endoscopic management.

During this era of advanced and minimally invasive procedures for treating urolithiasis, percutaneous nephrolithotomy (PCNL) remains the primary choice for removing large renal calculi. While there are various known complications associated with PCNL, such as bleeding, sepsis and injury to neighboring organs, the occurrence of retained foreign bodies as a result of the procedure is rarely reported. In this case report, we present a unique instance of encrustation involving a retained guidewire sheath following PCNL, which was initially mistaken for a residual stone fragment in imaging studies.…

Laparoscopic Dismembered Repair in Two Patients with Retrocaval Ureter.

Retrocaval ureter (RCU) or circumcaval ureter is a rare cause of congenital hydronephrosis. The surgical correction of RCU should be performed in all patients with obstruction and hydronephrosis symptoms, lumbar pain, urinary tract infections, hematuria, or urolithiasis. Traditionally, an open surgical approach was used for the treatment of RCU. Nowadays, surgical correction of these anomalies is performed using minimally invasive techniques. We report on two cases treated with our standardized laparoscopic technique using only three 5-mm trocars. The proposed approach could be considered as t…

Diagnostic Imaging of the Renal System in Exotic Companion Mammals.

Radiographs can be used to easily visualize common types of urinary calculi in all parts of the urinary tract. Positive-contrast excretory urography and cystourethrography are sensitive to diseases within the ureters and urethra, most commonly obstruction. Ultrasound is widely available and noninvasive and can be used to evaluate renal architecture, ureteral dilation, urinary bladder wall disease, and urolithiasis. Computed tomography is increasing in availability and provides a large amount of cross-sectional information quickly and noninvasively. Multiple imaging modalities can be used to es…

Analysis of a 1-year-old cystinuric patient with recurrent renal stones.

Cystinuria is a hereditary disorder of cystine and dibasic amino acids (lysine, arginine, ornithine) transport across the luminal membrane of renal tubules and intestine, resulting in recurrent nephrolithiasis. Cystine stones frequently occur in the second or third decade of life with an occasional occurrence in infancy and in old age. Herein is presented the case of a 1-year-old girl with cystinuria and recurrent urolithiasis; the genetic basis of the disease was investigated by mutational analysis of the SLC3A1 gene. The data show that the present patient has an increased cystine (923.08 mic…

Initial manifestation of primary hyperoxaluria type I in adults-- recognition, diagnosis, and management.

Primary hyperoxaluria type I may initially manifest as urolithiasis, renal insufficiency, or symptoms of systemic oxalosis. This hereditary disorder was fatal until effective therapies evolved during the past two decades. Difficulty in recognizing and diagnosing this disorder in adults is illustrated in a report of a patient eventually restored to good health by high-flux dialysis and combined renal and hepatic transplantation. I explore the molecular processes of the genetic defect and discuss clinical indicators of primary hyperoxaluria type I, manifestations of oxalosis, the pathogenesis of…

[A case of mediastinal parathyroid adenoma with primary hyperparathyroidism].

A 21-year-old man with a history of recurrent urolithiasis was admitted. Hypercalcemia was found and a diagnosis of primary hyperparathyroidism was made. Neck exploration was performed without success. Localization studies were done. Repeated selective arteriography revealed a mediastinal parathyroid adenoma. CT scan as well showed the adenoma in the anterior mediastinum. Ultrasonotomography, 201Tl-chloride and 131I scintigraphy with subtraction image and two venous samplings were negative. Mediastinal exploration with partial sternotomy was performed and a parathyroid adenoma was subsequently…

Back-Table Stone Removal During Renal Transplantation: A Systematic Review.

Nephrolithiasis was traditionally considered a relative contraindication to kidney donation; however, to combat ongoing donor kidney shortages, ex vivo stone management during renal transplantation has increasingly been adopted to expand the donor pool. This review aims to explore the current state of back-table ureteroscopy and other ex vivo techniques for the treatment of urolithiasis at the time of transplantation. Existing literature was reviewed to summarize the available evidence on safety, feasibility, stone characteristics and location, as well as the techniques used for stone removal.…

Predictive Modeling of Urinary Stone Composition Using Machine Learning and Clinical Data: Implications for Treatment Strategies and Pathophysiological Insights.

Purpose: Preventative strategies and surgical treatments for urolithiasis depend on stone composition. However, stone composition is often unknown until the stone is passed or surgically managed. Given that stone composition likely reflects the physiological parameters during its formation, we used clinical data from stone formers to predict stone composition. Materials and Methods: Data on stone composition, 24-hour urine, serum biochemistry, patient demographics, and medical history were prospectively collected from 777 kidney stone patients. Data were used to train gradient boosted machine …
