Treatment results of 214 patients with post bulbar duodenal stenosis were analyzed. Penetrative ulcer was diagnosed in 85.9%. Surgical treatment, and notably, selective proximal vagotomy with duodenoplasty and Roux-en-Y gastric resection are considered to be methods of choice. Selective proximal vagotomy with duodenoplasty provides the best long-term results and quality of life restoration.

A gastrointestinal fistula is a late complication of peptic ulcer disease and forms when gastric resection is inadequate or vagotomy is incomplete or not performed. Such cases are extremely rare and are occasionally reported in the literature. Although peptic gastroduodenal ulcers are rarely treated surgically today, when surgery is required a laparoscopic approach is possible with its well known advantages. An experience with the treatment of 106 patients with pyloroduodenal stenosis of ulcerous etiology by vagotomy with draining operations has shown high efficiency of these measures resulting in lower rate of postoperative lethality. Selective proximal and stem vagotomy does not aggravate gastric atony in patients with stenosis. Electro-stimulation was found to be an effective method of treatment of this complication in the postoperative period of patients with gastric atony.

Under consideration were results of treatment of 71 patients with associated ulcers of the stomach and duodenum. Resection of the stomach was performed in 11 patients, various kinds of vagotomy--in 61 patients. Long-term results of operations in 69 patients were analyzed. Truncal vagotomy with an obligatory dissection of the gastric ulcer is a reliable method of treatment of the associated ulcers.

The results of operative treatment of thirty-six patients with acute gastric mucosal hemorrhage are presented. Alcohol and surgery/trauma were the most frequent causes, and renal failure, sepsis, or multisystem disease caused most deaths; overall mortality was 44 per cent. Vagotomy-pyloroplasty and vagotomy-resection are compared, and no statistically significant difference in rebleeding, reoperation for bleeding, or mortality is identified.

BACKGROUND/AIMS: The role of Helicobacter pylori as a cause of peptic ulcer is still subject to controversy. The Kock's postulates have not been yet fulfilled; the bacteria can be found in normal persons, and it persists in the stomach after the ulcer is healed.
MATERIAL AND METHODS: The authors analyzed 41 persons formerly submitted to surgery (after 8 years and 4 months, as a mean), 31 to highly selective vagotomy, and 10 to truncal or selective vagotomy plus gastroduodenal drainage.
