Risk factors and outcomes associated with vancomycin-resistant Enterococcus faecium and ampicillin-resistant Enterococcus faecalis bacteraemia: A 10-year study in a tertiary-care centre in Mexico City.

OBJECTIVES: We sought to identify risk factors associated with vancomycin-resistant Enterococcus faecium (VRE) and ampicillin-resistant Enterococcus faecalis (ARE) bacteraemia, predictors of 30-day mortality, and 90-day recurrence-free survival according to resistance. METHODS: We evaluated clinical records of patients with E. faecalis and E. faecium bacteraemia (2007-2017). We performed bivariate and multivariate logistic regression analyses to identify factors associated with VRE and ARE bacteraemia and predictors of 30-day mortality. A Kaplan-Meier estimate of 90-day recurrence-free surviv

Determining the clinical significance of co-colonization of vancomycin-resistant enterococci and methicillin-resistant Staphylococcus aureus in the intestinal tracts of patients in intensive care units: a case-control study.

BACKGROUND: The emergence of vancomycin-resistant Staphylococcus aureus (VRSA) has become a global concern for public health. The proximity of vancomycin-resistant enterococcus (VRE) and methicillin-resistant S. aureus (MRSA) is considered to be one of the foremost risk factors for the development of VRSA. This study aimed to determine the incidence, risk factors, and clinical outcomes of intestinal co-colonization with VRE and MRSA. METHODS: A case-control study was conducted in 52-bed intensive care units (ICUs) of a university-affiliated hospital from September 2012 to October 2017. Active

Genomic analysis of teicoplanin resistance emerging during treatment of vanB vancomycin-resistant Enterococcus faecium infections in solid organ transplant recipients including donor-derived cases.

OBJECTIVES: We noted four cases of apparent in vivo emergence of teicoplanin resistance during failed therapy for initially teicoplanin-susceptible vanB vancomycin-resistant Enterococcus faecium (VREfm) infections in solid organ transplant recipients at our institution over a 12 month period. We investigated if in vivo emergence of resistance had occurred, if transplant-related vancomycin-resistant Enterococcus (VRE) infections had occurred and identified clinical predictors of resistance emergence. METHODS: Whole genome sequencing was performed on nine VREfm isolates for phylogenetic analysi

Dispersion of the Vancomycin Resistance Genes vanA and vanC of Enterococcus Isolated from Nile Tilapia on Retail Sale: A Public Health Hazard.

Although normally regarded harmless commensals, enterococci may cause a range of different infections in humans, including urinary tract infections, sepsis, and endocarditis. The acquisition of vancomycin resistance by enterococci (VRE) has seriously affected the treatment and infection control of these organisms. VRE are frequently resistant to all antibiotics that are effective treatment for vancomycin-susceptible enterococci, which leaves clinicians treating VRE infections with limited therapeutic options. With VRE emerging as a global threat to public health, we aimed to isolate, identify …

Gallbladder Cancer Presenting as Mirizzi Syndrome Complicated by Rapidly Evolving 23 rRNA Gene-Linezolid Resistance with Vancomycin-Resistant Enterococcus Infection Resulting in Fatal Cholangial Sepsis.

We describe the case of a 71-year-old woman who presented with obstructive jaundice and subhilar bile duct stenosis. MRI showed extensive cholecystolithiasis with an impacted bile stone in the cystic duct suggesting Mirizzi syndrome. Delayed enhancement of the thickened gallbladder wall suggested inflammation instead of carcinoma. After drainage of the obstructed bile duct via ERCP, the patient developed liver abscesses with a nosocomial vancomycin-resistant enterococcus infection treated by linezolid. After 4 weeks, the VRE infection was complicated by a new-onset 23 rRNA gene-mediated linezo

A Discrete Event Simulation Model of Patient Flow in a General Hospital Incorporating Infection Control Policy for Methicillin-Resistant Staphylococcus Aureus (MRSA) and Vancomycin-Resistant Enterococcus (VRE).

BACKGROUND: Hospitalized patients are assigned to available staffed beds based on patient acuity and services required. In hospitals with double-occupancy rooms, patients must be additionally matched by gender. Patients with methicillin-resistant Staphylococcus aureus (MRSA) or vancomycin-resistant Enterococcus (VRE) must be bedded in single-occupancy rooms or cohorted with other patients with similar MRSA/VRE flags. METHODS: We developed a discrete event simulation (DES) model of patient flow through an acute care hospital. Patients are matched to beds based on acuity, service, gender, and k

Phenotypic and Molecular Characterization of Clinical Isolates of Vancomycin-Resistant Enterococcus faecium in the Health District of Bolzano (Italy) During 2021-2023.

Vancomycin-resistant Enterococcus faecium (VREfm) is an emerging pathogen responsible for healthcare-associated infections. For this reason, 44 VREfm isolates collected during 2021-2023 were characterized using phenotypic and genomic approaches. VREfm isolates were identified by MALDI-TOF and antimicrobial susceptibility tests were performed with Vitek 2, Sensititre, or E-test. Sequence type (ST), antibiotic resistance genes, virulence factors and genetic relatedness were determined using Next Generation Sequencing. Forty-three isolates had a VanA phenotype and vanHAX genotype and one had a Va

Vancomycin-Resistant Enterococcus Often Spreads From Hospitalized Patients Into the Local Environment and Less Often Spreads From the Environment Into Patients.

BACKGROUND: Uncertainty remains about how gut-based organisms such as vancomycin-resistant Enterococcus (VRE) are spread within the local hospital environment. We hypothesized that, in the medical intensive care unit (ICU), VRE is spread predominantly patient-to-environment rather than environment-to-patient. METHODS: Medical ICU patients with sepsis and receiving broad-spectrum antibiotics were sampled via deep rectal swabs at ICU admission and on ICU days 3, 7, 14, and 30. Corresponding ICU room environmental samples were taken at the same time points. All samples were analyzed with 16S seq

Application of matrix-assisted laser desorption ionization time-of-flight mass spectrometry in the detection of vancomycin-resistant and-susceptible Enterococcus faecium.

INTRODUCTION: In recent years, the escalating prevalence of Vancomycin-resistant Enterococcus faecium (VREfm) has emerged as a formidable challenge to global healthcare systems. While Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) has become an indispensable tool for bacterial identification, its potential for rapid discrimination between Vancomycin-susceptible Enterococcus faecium (VSEfm) and VREfm through characteristic peak analysis remains an area of active investigation. METHODS: In this study, we conducted literature search through databases

Next generation antimicrobial peptide disinfectant targeting biocide and vancomycin resistant staphylococcus aureus through integrated in Silico and in vitro validation.

The rise of biocide- and antibiotic-resistant microbes in hospital settings demands urgent and innovative strategies to curb the spread of antimicrobial resistance (AMR). This study presents an innovative disinfectant strategy that leverages antimicrobial peptides (APep) extracted from antimicrobial-producing (AMP) strains isolated from hospital environments under intense antibiotic pressure. Unlike traditional probiotic disinfectants that rely on live bacterial strains prone to AMR, this approach directly utilizes APep from non-resistant strains with a high antioxidant potential to combat AMR

Investigation of biofilm formation in methicillin resistant Staphylococcus aureus isolates by genotypic and phenotypic methods and effect of vancomycin and teicoplanin on biofilm inhibition.

The pathogenicity factors of Staphylococcus aureus include biofilm production. In this study, the biofilm production abilities of methicillin-resistant S. aureus (MRSA) strains were investigated using genotypic and phenotypic methods. Additionally, the effect of glycopeptides on biofilm was examined. This study included 130 MRSA isolates. Biofilm was detected by the microtiter plate method. The minimum inhibitory concentration (MIC) of glycopeptides was evaluated through the broth microdilution method. The biofilm inhibitor concentration (BIC) values were investigated in isolates with strong b

Hit-to-Lead Optimization of Acetazolamide-Based Bacterial Carbonic Anhydrase Inhibitors with Efficacy In Vivo for Treatment of Vancomycin-Resistant Enterococci Septicemia.

As one of the leading causes of hospital-acquired infections reported by the National Healthcare Safety Network, vancomycin-resistant enterococci (VRE) continue to afflict patients in healthcare facilities, with limited FDA-approved drugs available for treating systemic infections. Our group previously showed the 1,3,4-thiadiazole acetazolamide human carbonic anhydrase inhibitor scaffold can be repositioned with potent in vitro efficacy against enterococcal pathogens. However, only acetazolamide has been explored for in vivo efficacy in murine septicemia models. Herein, we report a hit-to-lead
