BACKGROUND: A shunt is usually created from the distal arm (wrist) to the proximal arm (axillary loop) as long as no central stenosis has occurred. Creating vascular access in a patient with central vein stenosis could induce venous hypertension in the upper extremities. In such patients, an ipsilateral internal jugular vein (IJV) as an arteriovenous (AV) outflow vein should be the last option for using a particular arm.
METHODS: Thirty-two patients who had AV hemodialysis access via a jugular vein were analyzed retrospectively from 2001 to 2011. All patients had an ipsilateral subclavian or

BACKGROUND: Interventional procedures in adults with congenital cardiac conditions often require insertion of large-sized sheaths into the femoral veins. Data on the use of suture-mediated devices for femoral venous access site closure are scant and no data are available regarding venous patency after device use.
OBJECTIVE: To assess the efficacy of the 6Fr Perclose (Abbott Vascular Devices, CA, USA) suture-mediated device in achieving haemostasis and venous patency after closure.
DESIGN AND SETTING: 146 consecutive patients (80 women, mean (SD) age 45 (14) years) undergoing closure of 205 f

BACKGROUND: Color-duplex ultrasound (CDU) surveillance of arterial bypass grafts has been validated, but the natural history of "failing" grafts remains poorly defined. Our purpose was to compare failing grafts having prophylactic revision with those that did not.
METHODS: Postoperative duplex surveillance was performed in an accredited vascular laboratory for all lower extremity bypass grafts performed at a single institution. Eighty-five infrainguinal grafts (57 vein, 21 polytetrafluoroethylene (PTFE), and 7 composite grafts) in 83 patients were identified as failing by accepted criteria. T

BACKGROUND: Cardiovascular bypass grafting is an essential treatment for complex cases of atherosclerotic disease. Because the availability of autologous arterial and venous conduits is patient-limited, self-assembled cell-only grafts have been developed to serve as functional conduits with off-the-shelf availability. The unacceptably long production time required to generate these conduits, however, currently limits their clinical utility. Here, we introduce a novel technique to significantly accelerate the production process of self-assembled engineered vascular conduits.

There is a variety of conduits utilized as vascular grafts in coronary artery bypass grafting (CABG). Post-CABG graft rate of failure varies depending on the type of conduit used, with the highest failure rates seen in saphenous vein grafts (SVG). Patency rates of SVG are reported to be about 75% at 12-18 months. Left internal mammary artery (LIMA) grafts have shown higher long-term patency rates when compared to other arterial and venous grafts; however, LIMA occlusions occur, most commonly in the early postoperative period. Percutaneous coronary intervention (PCI) of LIMA graft can be challe

OBJECTIVE: To investigate the application of free anterior serratus musculo-fascial flap in bridge style for the soft tissue defect at leg.
METHODS: From Sept. 2006 to Jan. 2009, the free anterior serratus musculo-fascial flaps were used in bridge style in 7 cases with soft tissue defects at legs. The anterior serratus musculo-fascial flaps were elevated with subscapular and circumflex scapular vessels forming a T-shaped vascular pedicles. The T-shaped pedicle was end-to-end anastomosed with the two ends of the posterior tibial artery at the healthy leg. The musculo-fascial flap and its pedic

Modern heart surgery began with operative intervention for patent ductus arteriosus in 1938. Half a century later, ligation and division of patent ductus arteriosus in an infant or child remains a simple and safe surgical procedure for a lesion identified relatively easily. In the intervening years, paediatric cardiologists and surgeons have also directed their attention to the management of more complex congenital cardiac defects. Recently, however, there has been a significant reappraisal and re-emphasis of the role of patent ductus arteriosus in the context of neonatal cardiopulmonary disea

PURPOSE: To evaluate the clinical effectiveness and safety of drug-coated balloons (DCBs) compared with those of percutaneous transluminal angioplasty (PTA) for arteriovenous fistula (AVF) stenosis via a review of systematic reviews (SRs) and an update of the current meta-analysis.
MATERIALS AND METHODS: Literature was searched to retrieve SRs comparing DCBs and PTA for AVFs. A narrative review of SRs and pooled analysis were performed.
RESULTS: Eleven SRs were included. DCBs demonstrated favorable outcomes at 6 and 12 months compared with PTA, with improved patency in 7 SRs and a trend towa

OBJECTIVE: Previous studies demonstrating a correlation between low shear stress (tau = 5-15 dyne/cm(2)) and experimental vein graft neointimal thickening (NIT) support the role of low tau in vein graft failure. However, a simple linear relationship between low tau and NIT would underestimate the degree of NIT evident in high-grade occlusive lesions of failing human vein grafts. In this study we used a new experimental model that maintains patency at low tau (< 2 dyne/cm(2)), to delineate possible deviations from linearity in the low tau --> NIT hypothesis.

To report a case of acute arm ischemia and prosthetic shoulder infection treated by extra-anatomical great saphenous vein graft with external vascular scaffolding. A 65 year-old man with multiple surgical interventions for soft tissue sarcoma of the right shoulder, local radiotherapy with residual brachial plexus neuropraxia, was referred to our attention for signs of arm ischemia. Two weeks before, the patient was submitted to prosthetic shoulder replacement complicated with prosthetic infection. Considering the mechanism of vascular injury, an open surgical revascularization was planned with

Introduction Catheter-related bloodstream infection remains a major complication of neonatal central venous access. Although taurolidine lock therapy has been used in neonatal populations, experience with taurolidine locking in ultra-small (1-2 Fr) neonatal peripherally inserted central catheters (n-PICCs) remains extremely limited. This study describes the feasibility of repeated 2% taurolidine lock therapy in neonates requiring prolonged central vascular access. Methods We retrospectively reviewed three neonates who received repeated 2% taurolidine lock therapy during routine care in a terti

BACKGROUND AND OBJECTIVES: Improper techniques in cerebrovascular end-to-side bypass, such as back-wall injury or skiving bites, can lead to stenosis or thrombosis. To address this challenge, we developed a needle tip quality-controlled (TQC) training protocol and evaluated its training value and clinical significance.
METHODS: The TQC protocol mandates continuous visualization of the needle tip within the vessel lumen during each stitch, guided by strict suturing principles that prohibit intraluminal blind suturing. An attending neurosurgeon underwent training using the TQC protocol on a rat

BACKGROUND: Extrahepatic portal vein obstruction (EHPVO) can result in various complications, including life threatening gastrointestinal bleeding. The Meso-Rex bypass (MRB) restores a physiological portal flow. However a history of umbilical vein catheterization (UVC) has sometimes been considered as a contraindication to MRB.
OBJECTIVE: To evaluate the feasibility and results of MRB in children with EHPVO secondary to UVC.
METHODS: We performed a retrospective, single-center study of 51 children with EHPVO in native (non-transplanted) livers treated between 2009 and 2023. All patients unde

BACKGROUND: The no-touch (NT) technique for saphenous vein harvesting aims to preserve vascular integrity and improve graft performance in coronary artery bypass grafting (CABG). However, concerns remain regarding wound healing disorders and local complications at the graft harvest site.
METHODS: This prospective observational comparative study included 60 consecutive patients undergoing CABG, allocated to conventional saphenous vein harvesting (CONV, n=30) or the NT technique (NT, n=30). Group allocation was based on routine clinical practice and surgeon preference, without formal randomizat

Dual-antiplatelet therapy is commonly prescribed after endovascular intervention for peripheral artery disease. However, it is not known whether therapeutic anticoagulation affects outcomes after peripheral endovascular intervention. We sought to investigate whether therapeutic anticoagulation after peripheral endovascular intervention is associated with lower risk of major adverse limb events (MALEs) and all-cause mortality. We studied patients who underwent index endovascular intervention for peripheral artery disease in the Vascular Study Group of New England (2010 to 2018). The main exposu
