BACKGROUND: Brugada syndrome (BrS) is an inherited cardiac channelopathy associated with a predisposition to malignant ventricular arrhythmias and sudden cardiac death (SCD). It demonstrates a notably higher prevalence in Asian populations and exhibits profound phenotypic heterogeneity, ranging from lifelong asymptomatic carriers to individuals whose initial presentation is sudden cardiac arrest. Early diagnosis and appropriate long-term management are critical to reduce mortality.
CASE DESCRIPTION: A 42-year-old Asian male presented with recurrent ventricular fibrillation (VF) and syncope, o…

BACKGROUND: Atrial fibrillation (AF) and sick sinus syndrome (SSS) frequently coexist and may share common atrial structural and electrophysiological remodeling substrates. Left ventricular (LV) diastolic dysfunction frequently coexists with AF; however, its association with subsequent detection of SSS and AF remains incompletely understood. This study investigated whether LV diastolic dysfunction is associated with the detection of SSS and AF.
METHODS: This study included patients who underwent insertable cardiac monitor implantation for recurrent unexplained syncope. Patients with prior AF,

Long QT syndrome (LQTS) is a rare cardiac electrophysiological disorder that predisposes individuals to life-threatening arrhythmias such as torsades de pointes and ventricular fibrillation (VF). It may be congenital or acquired, with many cases triggered by specific medications or electrolyte disturbances. However, presentations in elderly individuals without prior cardiac history or drug exposure are exceedingly rare. This case report highlights a unique case of congenital LQTS presenting with VF and cardiac arrest in a 66-year-old woman. She presented with recurrent dizzy spells accompanied…

BACKGROUND: The optimal timing of catheter ablation in individuals with atrial fibrillation (AF) and left ventricular systolic dysfunction (LVSD) remains uncertain.
OBJECTIVE: We examined whether AF diagnosis to ablation time (DAT) influences outcomes following catheter ablation (CA) in patients with persistent AF (PsAF) and LVSD from the CAMERA-MRI and CAPLA randomized studies.
METHODS: We evaluated clinical outcomes according to DAT < 1 year ("shorter DAT") and ≥1 year ("longer DAT"), comparing AF recurrence, AF burden, left ventricular ejection fraction (LVEF), and LV recovery (LVEF ≥ 50%

BACKGROUND: Right ventricular (RV)-pulmonary artery (PA) uncoupling is associated with poor outcomes in heart failure patients. We aimed to elucidate the relationship between RV-PA uncoupling and late arrhythmia recurrence after ablation in persistent atrial fibrillation (PerAF) patients whose phenotypes have impaired right ventricular function and pulmonary hypertension.
METHODS: The present study included 203 PerAF patients from the Osaka Rosai Atrial Fibrillation ablation (ORAF) registry who underwent an initial ablation. We assigned the patients based on the value of tricuspid annular pla

BACKGROUND: Right ventricular (RV) morphology has been associated with drivers of atrial fibrillation (AF) risk, including left ventricular and pulmonary pathology, systemic inflammation, and neurohormonal activation. The aim of this study was to investigate the association between RV morphology and risk of incident AF.
METHODS AND RESULTS: We interpreted cardiac magnetic resonance imaging in 4204 participants free of clinical cardiovascular disease in the MESA (Multi-Ethnic Study of Atherosclerosis). Incident AF was determined using hospital discharge records, study electrocardiograms, and M

We report the case of a 56-year-old woman with newly diagnosed atrial fibrillation (AF) and severe left ventricular (LV) dysfunction caused by rapid conduction via an accessory pathway (AP), mimicking left bundle branch block, as the first clinical manifestation of Wolff-Parkinson-White (WPW) syndrome. Electrical cardioversion of the AF revealed a short PR interval and a delta wave, which was positive in leads I, II, aVL, and V2 and negative in lead V1 with a transition zone between V1 and V2. Radiofrequency catheter ablation of a superoparaseptal pathway was accompanied by rapid recovery from

BACKGROUND: Paroxysmal atrial tachyarrhythmias occur frequently in myotonic dystrophy type 1 (MD1) patients. Pacemakers, implanted for the treatment of bradyarrhythmias and including detailed diagnostic functions, may facilitate the diagnosis and management of frequent paroxysmal atrial fibrillation (AF) that may remain undetected during a conventional clinical follow-up. The effect of right ventricular pacing on AF incidence is still controversial.
AIM: To evaluate the influence of a high percentage of right ventricular pacing on AF in MD1 patients during a 12-month follow-up period.

BACKGROUND: Using a goat animal model, we tested the hypothesis that angiotensin-II inhibition reduces fibrotic degeneration of both the atrial and ventricular myocardium as well as AF induction susceptibility.
METHODS: We studied three groups of five goats over a 6-month period. The study animals in the first two groups were implanted with a pacemaker capable of maintaining AF with burst pacing. Additionally, in one group, goats were administered candesartan (AF+candesartan group). The third group (SR group) of animals served as control. Animals were tested for AF induction on day 0, 1, 30,

BACKGROUND: Inhibition of the sarcolemmal sodium-hydrogen exchanger isoform-1 (NHE-1) is emerging as a promising novel strategy for ameliorating myocardial injury associated with ischemia and reperfusion. We investigated whether NHE-1 inhibition (with cariporide) could minimize mechanical and electrical myocardial abnormalities that develop during ventricular fibrillation (VF) and improve outcome using a porcine model of closed-chest resuscitation.
METHODS AND RESULTS: Two groups of 8 pigs each were subjected to 8 minutes of untreated VF and randomized to receive either a 3-mg/kg bolus of car…

Ventricular single chamber permanent cardiac pacing undoubtedly eliminates symptoms related to extremely low cardiac rate, but also contributes to increased morbidity due to onset of permanent atrial fibrillation (PAF). Many studies have shown the superiority of atrial and dual chamber cardiac pacing in reducing atrial fibrillation risk and in preventing correlated embolic complications. It is not known, however, if this advantage is maintained in the elderly and in particular in the oldest old people. From 1986 onwards, we analyzed 690 chronically paced patients, 472 received a single chamber

An investigation was undertaken to determine the ventricular fibrillation (VF) threshold of anaesthetised dogs subjected to external application of electric shocks between a foreleg and a hindleg. The shocks were 2-15 ms sections of 50 Hz sine waveform starting at peak current and were applied at a known time in the heart cycle. The object of the experiment was to determine if there was an increase in cardiac susceptibility to electrically induced VF at 31 atmospheres absolute (atm abs) in a helium and oxygen environment. The duration and position of that part of the cardiac cycle most vulnera…

OBJECTIVES: The purpose of this study was to evaluate left ventricular function and atrial and ventricular diameters in patients with persistent atrial fibrillation (AF) treated with rate or rhythm control.
BACKGROUND: Restoration of sinus rhythm in patients with persistent AF may improve left ventricular function and reduce atrial dimensions. Adequate rate control in AF may preserve ventricular function.
METHODS: In 335 patients included in the RAte Control versus Electrical Cardioversion for Persistent Atrial Fibrillation (RACE) study, echocardiography was performed at baseline and 1- and

BACKGROUND: The relationship between right atrial (RA) structural and functional remodeling and risk of incident atrial fibrillation/flutter (AF/AFL) in pulmonary arterial hypertension remains poorly defined.
METHODS: In a multicenter observational cohort study of 326 patients with pulmonary arterial hypertension, we evaluated associations between RA structure and function, as determined by echocardiography, and incident AF/AFL. Incidence rates were calculated using time-to-event analysis. Cox proportional hazards models identified factors associated with incident AF/AFL, and Harrell's C-stat

BACKGROUND: Septic cardiomyopathy (SCM) is a reversible acute cardiac dysfunction caused by sepsis. The use of QT-prolonging agents such as moxifloxacin and amiodarone may induce life-threatening arrhythmias, including torsades de pointes (TdP) and ventricular fibrillation (VF), both associated with high morbidity and mortality, posing significant challenges in clinical management.
CASE PRESENTATION: We report the case of an 89-year-old male who developed septic cardiomyopathy secondary to enterogenic septic shock, presenting with marked corrected QT (QTc) prolongation and impaired cardiac fu…

BACKGROUND: New-onset postoperative atrial fibrillation (POAF) is common after transcatheter aortic valve replacement (TAVR). At present, the impact of POAF on cardiac remodeling after TAVR has not been thoroughly studied.
OBJECTIVES: To investigate the impact of POAF on cardiac remodeling and its association with clinical outcomes after TAVR.
METHODS: 601 patients undergoing TAVR for severe aortic stenosis were evaluated. Of these, 253 patients were identified to have POAF, which was categorized as normal left ventricular mass index (LVMI) in 54 (21%) and high LVMI in 199 (79%). The primary
