Arrhythmic complications associated with the treatment of patients with congenital cardiac disease: consensus definitions from the Multi-Societal Database Committee for Pediatric and Congenital Heart Disease.

A detailed hierarchal nomenclature of arrhythmias is offered with definition of its applications to diagnosis and complications. The conceptual and organizational approach to discussion of arrhythmias employs the following sequence: location--mechanism--aetiology--duration. The classification of arrhythmias is heuristically divided into an anatomical hierarchy: atrial, junctional, ventricular, or atrioventricular. Mechanisms are most simplistically classified as either reentrant, such as macro-reentrant atrial tachycardia, previously described as atrial flutter, or focal, such as automatic or

The use of calcium salts in the prevention and management of verapamil-induced hypotension.

OBJECTIVE: To review the available literature on the use of intravenous calcium salts for the prevention of hypotension associated with intravenous verapamil. METHODS: A MEDLINE search (1966-June 1999) identified pertinent articles; references from these articles were identified to serve as additional resources. DISCUSSION: Verapamil is effective in inhibiting atrioventricular nodal conduction, thereby controlling ventricular rate in patients with atrial fibrillation/flutter and terminating paroxysmal supraventricular tachycardia. However, hypotension may be caused by the negative inotropic

[Time of occurrence of paroxysmal atrial fibrillation in Holter monitoring in patients with coronary heart disease and in patients with hypertension].

Paroxysmal atrial fibrillation (PAF) often accompany coronary heart disease (CHD), and primary hypertension (PH). The aim of the study was to evaluate the time of occurrence and duration of paroxysmal atrial fibrillation (PAF) identified from Holter recordings in 63 patients (27 women and 36 men) with CHD (n = 45) and PH (n = 18). No pharmacological treatment was applied before and during the examination. All patients were in sinus rhythm at the start and the end of the recording which lasted for 24 hours. PAF were defined as the occurrence of at least four beats of supraventricular origin, wi

Advances in surgical care of infants and children with congenital heart disease.

Recent important technical developments in the field of surgery for congenital heart disease have included the Ross pulmonary autograft replacement of the aortic valve, video-assisted thoracoscopic surgery, and the double-switch and switch-Rastelli procedure for congenitally corrected transposition. Although the growth potential of the pulmonary autograft has been confirmed, an important incidence of late aortic regurgitation has been noted. Expanding indications for video-assisted thoracoscopic surgery have been described, including division of the vascular ring. Late hemodynamic assessment a

Doppler study of precordial musical murmurs.

Using phonocardiography, continuous- and pulsed-wave Doppler, 51 patients with precordial "musical" murmurs (49 with cardiac abnormalities) and 21 patients with noisy murmurs were examined. With M-mode echocardiography, fine fluttering of the structure generating the murmur was evident in 23 patients with musical murmurs and in 5 with noisy murmurs. A continuous-wave Doppler spectral signal characterized by parallel harmonics (Doppler musical signal) was evident in all patients with musical murmurs and in none with a noisy murmur. With pulsed-wave Doppler, the musical signal had less defined s

Rate-related accelerating (autodecremental) atrial pacing for reversion of paroxysmal supraventricular tachycardia.

Twenty consecutive patients with paroxysmal intra A-V nodal or atrio-ventricular tachycardia had a new tachycardia reversion pacing modality evaluated during routine electrophysiological study. The pacing was controlled by a micropressor interfaced with a stimulator connected to a right atrial pacing electrode. On detection of tachycardia the first pacing cycle interval is equal to the tachycardia cycle length minus a decrement value D. Each subsequent pacing cycle is further reduced by the same value of D, thus accelerating the pacing burst until a plateau of 100 beats/min faster than tachyca

[Late arrhythmias in the operated interatrial communication. Analysis of sinus node function and the conduction pathways by His bundle electrocardiography].

Four hundred patients with atrial septal defect treated surgically were reviewed. Thirty five (8.7%) developed arrhytmias post-surgery which persisted for over a year. Sinus bradycardia was found in 10 patients, nodal rhythm in 21, and atrial fibrilation and flutter in 4 patients. Thirty five per cent of the patients with late arrhythmias developed related symptomatology. In 14 patients the function of the sinus node was studied with electrical stimulation of the atrium and with His registry. The interatrial conduction time, AV node and His Purkinje were analized employing various stimulation

Case Report: 26-year follow-up after total cavopulmonary connection in crisscross heart-longitudinal assessment of anastomotic patency, biomarkers, and arrhythmia progression.

Crisscross heart (CCH) is an extremely rare congenital cardiac malformation, and literature on long-term outcomes after total cavopulmonary connection (TCPC) in CCH patients remains limited. We report a unique case demonstrating sustained anastomotic patency and objective postoperative evolution 26 years after TCPC in a patient with CCH. A 39-year-old female with crisscross heart and double-outlet right ventricle underwent TCPC at age 13 in 2000. Serial Doppler echocardiography (2013-2026) consistently demonstrated patent superior vena cava-right pulmonary artery anastomosis and inferior vena

High Diagnostic Accuracy of Long-Term Electrocardiogram Interpretation by General Practitioners.

AIMS: Traditional long-term ECG monitoring systems have primarily been used by cardiologist. New remote and wearable easy-to-use devices have led to increased use of ECG recordings also outside cardiology clinics. The aims of this study were to assess the feasibility and diagnostic accuracy of interpretation of the one-lead ECG recordings from a patch ECG device (ECG247 Smart Heart Sensor system) by general practitioners (GP). METHODS: Norwegian GPs were invited to digitally assess 10 long-term ECG recordings with different arrhythmias performed by the ECG247 Smart Heart Sensor system. For al

Familial left ventricular hypertrabeculation in myotonic dystrophy type 1.

BACKGROUND: Familial left ventricular hypertrabeculation (LVHT)has not been described in myotonic dystrophy type 1 (MD1). CASE REPORT: Two MD1 patients are described, father and daughter, both presenting with LVHT. The father was a 45-year-old man with the typical MD1 phenotype starting in 1992. DNA analysis revealed a heterozygous expansion of 300 CTG repeats in the myotonic dystrophy protein kinase (DMPK) gene on chromosome 19q13.3. He had a history of successful electrocardioversion of atrial flutter into sinus rhythm. Cardiac history and clinical cardiologic examination were otherwise nor

ECG interpretation: what is different in children?

While the electrical waveforms and basic approach to electrocardiogram (ECG) interpretation are similar for pediatric and adult patients, some important differences must be recognized to provide appropriate care for infants and children. Maturational changes that occur over the early childhood years result in differences in normal heart rates, interval durations, and ventricular dominance. Any arrhythmia seen in adults can occur in the pediatric patient; however, the most common variations from normal rhythms are sinus arrhythmia, bradycardia, and supraventricular tachycardia. Fetal arrhythmias: the Saint-Justine hospital experience.

We intend to review our experience with the investigation and management of foetal arrhythmia on the basis of superior vena cava/ascending aorta (SVC/AA) Doppler flow velocity recordings. Irregular rhythms n = 307. Premature atrial and ventricular contractions were easily identified and generally self-limited in time. Sustained bradycardia n = 19. Four had sinus bradycardia, six presented with blocked atrial bigeminism, three showed 2:1, and five had a complete atrio-ventricular (AV) block. Another foetus that presented with first-degree AV block developed a Luciani-Wenckebach phenomenon 1 wee

[Evaluation of Hancock mitral valve by M-mode echocardiography. Clinical significance of the timing of closing and opening of the valve].

The valve function of a Hancock xenograft in the mitral position was evaluated by M-mode echocardiograms guided by the two-dimensional echocardiogram. From M-mode echocardiograms, the intervals from the second heart sound to mitral valve opening (II-MVo) and from the Q wave to mitral valve closure (Q-MVc) were measured in 24 patients with a Hancock xenograft, 16 with mitral stenosis (MS) and 20 normal controls. Twenty-four patients with a Hancock xenograft were divided into four groups according to the echocardiographic pattern of the xenograft. Fourteen with normal echocardiograms (I: N.P.),

[Histiocytoid cardiomyopathy in an infant].

We present a description of a clinical observation of a histiocytoid cardiomyopathy in a female patient aged 4 months. This pathology is rare in pediatric cardiology. Its etiology is linked with mutation of the gene encoding mitochondrial cytochrome B (mitochondrial transport of electrons). This mutation leads to a specific morphological and functional abnormalities of cardiomyocytes. Purkinje cells and cells of conduction system at microscopy appear as histiocytolike foam cells cytoplasm of which contain large amount of lipids and glycogen. Girls prevail among those affected. The case reflect

C-Reactive Protein as a Predictor of Cardiovascular Events and Mortality in Obstructive Sleep Apnea: A Propensity-Matched Analysis Using the TriNetX Research Network.

INTRODUCTION: Obstructive sleep apnea (OSA) increases cardiovascular disease risk through systemic inflammation, particularly through elevated C-reactive protein (CRP). This study investigated whether CRP stratification predicts mortality and cardiovascular events in OSA patients. METHODS: We conducted a retrospective cohort analysis using the TriNetX Research Network, encompassing electronic health records from 130 healthcare institutions. Adults (ages 18-90 years) with OSA were stratified by CRP concentration: elevated (≥ 3.0 mg/L) versus low (≤ 2.9 mg/L). After propensity score matching fo

Subclinical Cardiac Disturbances After Rickettsia spp. Infection in an Endemic Region of Mexico.

Background: Rickettsial diseases are endemic in southeastern Mexico, yet their potential subclinical cardiac effects remain poorly understood. Although severe spotted fever and typhus group infections may cause myocarditis and arrhythmias, limited evidence exists regarding cardiac alterations in individuals previously diagnosed with rickettsiosis who later show Rickettsia spp. IgG seropositivity. Methods: This follow-up observational study was conducted at a tertiary referral hospital in the Yucatan Peninsula. From an initial cohort of 390 patients evaluated for suspected rickettsial disease,
