Thirty-one hearts with aortic arch obstruction and patent ductus arteriosus were examined with special reference to associated cardiac anomalies. Six presented with complete interruption of the aortic arch, four with atretic isthmus, twelve with coarctation, and three with tubular hypoplasia. Associated cardiac anomalies were divided into two main groups: (1) septal defect with left-to-right shunt, and (2) left ventricular inflow and/or outflow obstruction. A high incidence (9/19=47.4%) of ventriculo-infundibular malalignment type of ventricular septal defect with subaortic stenosis was observ

BACKGROUND: Giant aneurysmal degeneration of a classic Blalock-Taussig (BT) shunt is a rare, life-threatening late complication of tetralogy of Fallot (TOF) palliation.
CASE: A 47-year-old woman with unrepaired TOF palliated in childhood presented with exertional dyspnea and a right thoracic mass. Echocardiography revealed severe right ventricular outflow tract obstruction, a large ventricular septal defect, and a pulsatile extracardiac structure. Computed tomography and cardiac magnetic resonance imaging confirmed a giant (72 × 66 mm) true aneurysm of a right BT shunt. She underwent successf

Since 1960, at the National Institutes of Health, ventricular septal myotomy and myectomy has been the mode of treatment for severely symptomatic patients with hypertrophic cardiomyopathy and obstruction to left ventricular outflow who do not respond to medical therapy. Our long-term results of operation for hypertrophic cardiomyopathy are reviewed in 240 patients operated upon prior to 1980. Postoperatively, most patients had improved symptomatically (i.e. 70%) and manifested marked reduction or abolition of the basal left ventricular outflow gradient (i.e. 98%). However, 8% of the patients d

From July 1988 through September 1993, 30 neonates with symptomatic tetralogy of Fallot underwent complete repair. Sixteen patients had tetralogy and pulmonary stenosis, 9 had pulmonary atresia, 3 had nonconfluent pulmonary arteries, and 2 had both pulmonary atresia and nonconfluent pulmonary arteries. The median age at operation was 11 days (mean +/- standard error of the mean, 12.6 +/- 2.9 days), with a mean weight of 3.1 +/- 0.1 kg (range 1.5 to 4.4 kg). Preoperatively, 14 patients were receiving an infusion of prostaglandin, 13 were mechanically ventilated, and 6 required inotropic support

We report a case of a patient who presented with aortic stenosis and a borderline left ventricle during foetal life. A balloon aortic valve valvuloplasty was performed in uterus, and in the postnatal period for relief of the left ventricular outflow tract obstruction followed by a Ross-Konno procedure with fibroelastosis resection. These successful interventions allowed left ventricular growth and the conversion to a biventricular circulation after a single-stage surgery.

Reconstructing the right ventricular outflow tract and pulmonary valve via a bovine-derived valve conduit such as Matrix-P-Xenograft is a common surgical repair technique for pulmonary atresia and ventricular septal defect. After conduit degeneration due to calcification or aneurysmal dilatation, percutaneous transvenous stenting of the right ventricular outflow tract followed by pulmonary valve implantation has become the standard interventional treatment. Applied to stenotic conduits, the method is considered safe and effective. An important but seldom-reported problem is graft failure relat

BACKGROUND: We reviewed the outcomes of double-chambered right ventricle repair.
METHODS: Between 1969 and 1998, 40 patients underwent surgical repair of a double-chamber right ventricle. The patients ranged in age from 3 months to 52 years (mean, 12.8 +/- 11.6 years). Right ventricular outflow tract pressure gradients were from 20 to 170 mm Hg (mean, 65.0 +/- 38.5 mm Hg) An associated ventricular septal defect was present in 27 patients (67.5%). Four patients were older than 30 years of age.
RESULTS: There were no hospital or late deaths. Mean postsurgical follow-up was 16.5 +/- 8.9 years (

Infants with single ventricle and transposition of the great arteries with or without aortic arch obstruction have a poor prognosis due in large part to the development of systemic outflow obstruction, a frequent consequence of pulmonary artery banding. Thus, the initial palliation and long-term treatment options are critical in terms of surgical choices and timing. We report our experience with 9 patients managed by neonatal pulmonary artery banding and early debanding, a Damus-Kaye-Stansel procedure, and either a modified Glenn shunt or a modified Fontan procedure. Some evidence of subaortic

RATIONALE: Noonan syndrome (NS) is a RASopathy most frequently associated with mutations in HRAS, NRAS, KRAS, and RRAS2. The contribution of MRAS variants to NS pathogenesis remains poorly characterized, and infective endocarditis (IE) is extremely rare in patients with NS.
PATIENT CONCERNS: A 22-year-old woman presented with typical dysmorphic features of NS, including short stature, broad forehead, hypertelorism, low-set posteriorly rotated ears, and a broad neck. She developed fever and progressive exertional dyspnea.
DIAGNOSES: Echocardiography demonstrated obstructive hypertrophic cardi

Supravalvular aortic stenosis is the rarest form of left ventricular outflow tract obstruction. Aspergillus endocarditis is also rare and generally reported in immunocompromised hosts. Here we present a case of an immunocompetent patient with supravalvular aortic stenosis complicated by aortic mycotic pseudoaneurysm due to invasive aspergillosis.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) affects approximately 20% of patients with Noonan syndrome and contributes significantly to mortality, particularly in neonates with severe HCM and left ventricular outflow tract obstruction, where controlling heart rate (HR) and maintaining systemic vascular resistance can be challenging.
CASE SUMMARY: A full-term neonate with an Raf-1 proto-oncogene, serine/threonine kinase mutation and severe HCM developed diastolic heart failure requiring high-dose esmolol for rate control and phenylephrine for systemic vascular resistance support. After inade

BACKGROUND: Myosin inhibitor therapy is a novel option for hypertrophic cardiomyopathy (HCM) with left ventricular outflow tract obstruction. Noonan syndrome (NS), a RASopathy, can mimic HCM but was excluded from myosin inhibitor trials.
CASE SUMMARY: A 60-year-old woman with obstructive HCM (HOCM) presented with progressive dyspnea (NYHA functional class III). Echocardiography showed severe septal hypertrophy (19 mm) and persistent left ventricular outflow tract obstruction (peak gradient: 51 mm Hg during Valsalva) despite beta-blocker therapy. Mavacamten, a myosin ATPase inhibitor, was init

OBJECTIVE: To examine whether maternal hair or cord blood selenium levels are associated with infant congenital heart defects (CHDs).
METHOD: A hospital-based case-control study was performed with samples collected from 2010 to 2014. Selenium levels in maternal hair and fetal cord serum were measured using Inductively Coupled Plasma-Mass Spectrometry (ICP-MS). Data from medical records of infants with CHDs were collected. Logistic regression analysis was employed to examine the independent association between maternal selenium exposure and fetal CHD.
RESULTS: In total, 888 pregnant women wer

Takotsubo cardiomyopathy (TTC) is a well-recognised entity that commonly manifests with chest pain, ST segment abnormalities and transient left ventricular apical ballooning without coronary artery obstructive disease. This syndrome usually portends a favourable outcome. In the rare haemodynamically unstable TTC patients, acute mitral regurgitation (MR) related to systolic anterior motion (SAM) of the mitral valve and left ventricular outflow tract obstruction (LVOTO) is to be considered. Bedside echocardiography is key in recognition of this latter condition as vasodilators, inotropic agents

The patient, a 17 year old boy, had undergone a Rastelli operation for transposition of the great arteries plus ventricular septal defect and left ventricular outflow tract obstruction at the age of 7 years. Associated tricuspid valve stenosis was treated with insertion of a second homograft shortly after the Rastelli operation. The patient remained symptom-free for 10 years, when he suffered femoral artery embolus. Investigation showed severe obstruction of both conduits and biventricular dysfunction with thrombus in the left ventricle. After embolectomy and stabilisation of his general condi

Rhabdomyomas are benign cardiac tumors often occurring as multiple lesions. We describe an infant with multiple cardiac rhabdomyomas causing severe right-ventricular and left-ventricular outflow tract obstructions. The intracavitary parts of the rhabdomyomas were successfully resected when the patient was 5 weeks old. Echocardiography showed regression in size of the remaining intramural tumors. Since there is only a low growth potential in cardiac rhabdomyomas, surgical management is indicated only in patients with hemodynamic obstruction or significant arrhythmias. Surgery should be primaril

OBJECTIVE: One of the major causes of cerebral ventricular shunt failure is proximal catheter occlusion. We describe a novel ventricular cerebrospinal fluid (CSF) flow replicating system that assesses pressure and flow responses to varying degrees of catheter occlusion.
METHODS: Ventricular catheter performance was assessed during conditions of partial and complete occlusion. The catheters were placed into a three-dimensionally-printed phantom ventricular replicating system. Artificial CSF was pumped through the ventricular system at a constant rate of 1 mL/min to mimic CSF flow, with the pro

Tetralogy of Fallot consists essentially of two abnormalities: a large, unrestrictive ventricular septal defect and severe obstruction to the outflow of the right ventricle. A clear understanding of the principles described in this article should result in a mortality of under 5% after complete correction of tetralogy of Fallot.
