A rare case of dual origin of the left vertebral artery without convergence.

A case of dual origin of the left vertebral artery was encountered in a dissection course for medical students in 2014. Two vertebral arteries were observed on the left side. One arose from the aortic arch between the origin of the left common carotid artery and the left subclavian artery, entered the transverse foramen of the 4th cervical vertebra, and coursed upward into the transverse foramen. The other arose from the left subclavian artery as expected, divided into two branches anterior to the cervical vertebrae, and entered the transverse foramina of the 6th and 7th cervical vertebrae. Cervical Artery Dissections: Etiopathogenesis and Management.

Cervical Artery Dissection (CeAD) is a frequent stroke etiology for patients younger than 50 years old. The most common immediate complications related to CeAD are headache and neck pain (65-95%), TIA/ischemic stroke (>50%), and partial Horner's syndrome (25%). The prevailing hypothesis regarding the pathogenesis of sCeAD is that the underlying constitutional vessel wall weakness of patients with sCeAD is genetically determined and that environmental factors could act as triggers. The stroke prevention treatment of CeAD remains controversial, involving anticoagulation or antiplatelet therapy a

The double lumen: a pathognomonic angiographic sign of arterial dissection?

A case is reported which questions the absolute significance of the "double-lumen sign" in the diagnosis of arterial dissection. I suggest that when demonstrated in isolation, this sign should be interpreted with caution, giving consideration to the possibility of arterial fenestration, and appropriate diagnostic measures implemented.

Vertebral artery pseudoaneurysm complicating posterior C1-2 transarticular screw fixation: case report.

BACKGROUND: Vertebral artery injury during posterior C1-2 transarticular screw fixation occurs in approximately 3% of patients and may remain asymptomatic or result in arteriovenous fistulae, occlusion, narrowing, or dissection of the vertebral artery, and lead to transient ischemic attacks, stroke, or death. CASE DESCRIPTION: This is the first report of a pseudoaneurysm resulting from damage to the vertebral artery during the procedure. This 31-year-old male underwent posterior C1-2 transarticular screw fixation for unstable os odontoideum. Injury to the left vertebral artery occurred while

Selective endovascular treatment of cervical arterial dissection using quantitative magnetic resonance angiography.

INTRODUCTION: The role of endovascular treatment in cervical artery dissection (CAD) is equivocal. This study compared cerebral blood flow in CAD between medically and endovascularly treated patients using quantitative magnetic resonance angiography (QMRA). METHODS: Retrospective chart review was completed for patients with CAD. Inclusion criteria were adults (>18 years) with diagnosis of dissection of the internal carotid artery or vertebral artery who received QMRA. The cases were reviewed for clinical presentation, diagnosis, management, and imaging, and in particular, patients who underwe

Anatomic basis of the anterior surgery on the cervical spine: relationships between uncus-artery-root complex and vertebral artery injury.

Vertebral artery injury is a serious complication during anterior surgery on the cervical spine. However, little information is available in the literature concerning the mechanism of vertebral artery laceration during the procedures of the anterior cervical decompression. In the current study twenty-eight cadavers were dissected to determine the location and relationships of the fibro-ligamentous tissues to the uncinate process, vertebral artery and nerve roots from the C3 to C6 levels. The vertebral artery and nerve root are encased by a fibro-ligamentous band at the level of the intertransv

Cervical spinal cord infarction due to impingement of an anomalous right vertebral artery by thoracic osteophyte.

INTRODUCTION: Spinal cord infarction in a young, otherwise healthy individual is a rare occurrence. The anterior spinal artery and posterior spinal arteries are the primary contributors to the vascular supply of the cervical supply, and these arteries arise as descending branches of the vertebral arteries. Historically, many cases have demonstrated individual variations in the vertebral arteries, such as differences in dominancy, patency, origin, and insertion. The clinical significance of these variations remains poorly understood. CASE PRESENTATION: We present a patient who sustained a spin

[Dissecting vertebral aneurysm with subarachnoid hemorrhage after ischemic onset on the same day: a case report].

We report a case of a dissecting vertebral aneurysm with subarachnoid hemorrhage (SAH) after ischemic onset on the same day. A 48-year-old man had abrupt vertigo and nausea. CT & MRI on admission showed no abnormality, but he complained of left hemiparesis after admission. Twelve hours after the ischemic onset he suddenly complained of severe headache and his consciousness deteriorated. The follow-up CT showed diffuse SAH. Cerebral angiography showed occlusion of the right vertebral artery at the origin of the posterior inferior cerebellar artery (PICA) and segmental stenosis of the left verte

Subarachnoid hemorrhage caused by ruptured posterior inferior cerebellar artery aneurysm arising from the extracranial portion of an anomalous vertebral artery.

A 42-year-old woman presented with subarachnoid hemorrhage caused by a ruptured posterior inferior cerebellar artery aneurysm arising from the extracranial portion of an anomalous vertebral artery. We speculated that the aneurysm formed by dissection of the media because of its shape and location. Therefore, the aneurysm was isolated by trapping and excised, with occipital artery to posterior inferior cerebellar artery bypass. Histological examination showed a typical true aneurysm without evidence of dissection. This case suggests that an aneurysm of an anomalous artery is caused by hemodynam

Dissecting aneurysms of all four cervicocranial arteries in fibromuscular dysplasia: treatment with self-expanding endovascular stents, coil embolization, and surgical ligation.

A patient with cervicocranial fibromuscular dysplasia (FMD) presented with subarachnoid hemorrhage. A ruptured dissecting distal vertebral artery aneurysm required clip ligation of the parent artery; a contralateral dissecting proximal vertebral aneurysm was occluded with detachable coils. Progressive dissecting, extracranial aneurysms of the internal carotid artery were treated with self-expanding stents. Subsequent angiography and intravascular sonography revealed patent stents, a smooth luminal surface, and total occlusion of the aneurysm. Clinical outcome was excellent.

Vascular anomaly at the craniocervical junction presenting with sub arachnoid hemorrhage: Dilemma in Imaging Diagnosis, Endovascular Management and Complications.

We present a case of a ruptured vertebral artery dissecting aneurysm that mimicked a presumed vascular anomaly by CTA (Computerized Tomographic Angiography). A parenchymal arteriovenous malformation (AVM) or a dural arteriovenous fistula (DAVF) at the craniocervical junction can present with a subarachnoid hemorrhage and cannot be differentiated from a vertebral artery dissection by non invasive imaging. Catheter based cerebral angiography revealed a dissecting pseudoaneurysm of a diminutive right vertebral artery terminating in the posterior inferior cerebellar artery (PICA) that to our knowl…

Utility of T2-weighted high-resolution vessel wall imaging for the diagnosis of isolated posterior inferior cerebellar artery dissection at acute and early subacute stages.

Recent advances in magnetic resonance high-resolution vessel wall imaging (HRVWI), which can detect intramural hematomas (IMH), improve the noninvasive diagnostic accuracy of isolated posterior inferior cerebellar artery dissection (iPICA-D). However, despite the risk of overlooking minute IMH, the utility of T2-weighted HRVWI has not been thoroughly evaluated. This study aimed to compare the utility of T2-weighted HRVWI with that of T1-weighted HRVWI, basiparallel anatomical scanning (BPAS), and magnetic resonance angiography (MRA) for the diagnosis of iPICA-D mainly in the acute and early su

Seasonal Variation in Spontaneous Cervical Artery Dissection: Comparing between UK and Australian Sites.

BACKGROUND: Cervical artery dissection (CAD) is a leading cause of stroke among middle-aged adults, but the etiology is unclear. Some reports of seasonal variation in CAD incidence have been suggested but may reflect extreme climatic conditions. Seasonal variation may implicate more transient seasonal causes such as proinflammatory or hypercoagulable states. This study aimed to assess whether CAD incidence varied with season between UK and Australian sites. Also, this study aimed to determine whether there was a different pattern of seasonal variation between arteries (carotid and vertebral) a

Left Internal Mammary Artery Dissection: Use of Dual Balloon Angioplasty in Treatment of Ostial Dissection Involving Subclavian Artery.

The left internal mammary artery (LIMA) is the preferred arterial graft to be used for the left anterior descending (LAD) artery for coronary artery bypass graft (CABG) due to high graft patency rate. LIMA dissection is a rare, but dreadful complication of graft angiography and may lead to serious complications including death, myocardial infarction (MI), and re-do CABG. Transcatheter management of LIMA dissection involves multiple stenting. However, this may leave a dissection flap at the ostium of the LIMA that may extend to the left subclavian artery jeopardizing flow to the left arm and ve

A syndrome of spontaneous cerebral and cervical artery dissections with angiolipomatosis. Report of two cases.

A primary or systemic arteriopathy is frequently suspected in patients with spontaneous cerebral or cervical artery dissections. The authors report on two patients with such dissections accompanied by angiolipomatosis, a previously unreported association, and propose a common developmental defect in these patients. A 50-year-old man with subcutaneous angiolipomatosis developed painful monocular blindness. Angiography studies revealed a spontaneous extracranial internal carotid artery (ICA) dissection and an ipsilateral fusiform intracranial ICA aneurysm. The ICA dissection was treated with asp
