OBJECTIVES: To describe a large multigenerational family with migraine-associated vertigo (MAV) combining a detailed phenotypic and genetic analysis.

OBJECTIVE: To verify the result in the use of intratympanic corticosteroids, combined with intravenous, in patients with Meniere disease (MD).
MATERIAL AND METHODS: Ten patients with vertigo due to MD in stages 4-5 or 6 were included in the study. Combined intratympanic and intravenous dexamethasone was infiltrated. According to the canalolithiasis theory, benign paroxysmal vertigo (BPPV) is caused by gravity-dependent movements of otoconial debris that collects in the endolymph of the posterior semicircular canal. We have been clinically treating vertigo patients for the last 45 years. As is well-known, vertigo is not a simple otolaryngological disease. In fact, it is associated with various diseases in various clinical fields, including internal medicine, neurosurgery, and ophthalmology. As otolaryngologists, we have learned a few lessons from our experiences with patients having vertigo and in working with other otolaryngologists. Here, we discuss and share pitfalls in which otolaryngologists often are caught in diagnosing and treating vertigo.

Otitis media and middle ear effusion could be the most common causes of vertigo in children, but there are some problems in detecting the other causes for vertigo because they are one of most frequent diseases of childhood. In 507 vertigo cases in which circulatory velocity in the vertebral artery was determined, using ultrasonic blood rheography, the results in 150 cases (32%) were abnormal, falling appropriately into a 'laterality' group and a 'lower' group. Vertigo is very often an unspecific symptom. The author differentiates between vestibular or systematic vertigo and non-vestibular or unsystematic vertigo. The diseases of the central vestibular system are described. Not only the special diagnostic methods (e.g. electronystagmography) but also the anamnesis and simple clinical investigation are presented. The overview includes drug therapy and physical therapy of vertigo from a neurological point of view.

We evaluated 102 patients with dizziness or vertigo who were 50 years of age and over, using magnetic resonance imaging (MRI). Slow blood flow (SBF) in the vertebrobasilar system was detected in 36 patients (35%). The patients with SBF experienced dizziness or vertigo for a longer period than those without SBF. The apogeotrophic type of direction-changing nystagmus was observed in 10 of 36 patients with SBF. We reviewed the clinical and electronystagmographic findings of 84 patients who presented to our neuro-otology clinic with vertigo of presumed cerebrovascular origin. There was a surprisingly high incidence of isolated episodes of vertigo (abrupt in onset, lasting minutes). In some patients these episodes preceded other symptoms of vertebrobasilar insufficiency or infarction by months. Peripheral vestibular abnormalities were common on electronystagmographic testing; 42% had unilateral hypoexcitability to caloric stimulation.
