In vivo measurements of subacromial impingement: substantial compression develops in abduction with large internal rotation.

BACKGROUND: Subacromial impingement is a widely recognized mechanism of chronic shoulder pain. The magnitudes of the compressive forces that impinge the subacromial structures were often measured from cadaveric specimens, but it is questionable to use this data as a sole basis to determine the shoulder motions and/or shoulder configurations that induce impingement in live subjects performing active motion. The purpose of the present study was to determine in vivo the magnitude of the compressive force at selected shoulder configurations. METHODS: The subacromial structures may be impinged by

A study evaluating differences in 3D upper limb kinematics and surface electromyography measures in adults with and without facioscapulohumeral dystrophy.

BACKGROUND: Facioscapulohumeral dystrophy (FSHD) is a rare disease that causes progressive muscle wasting and loss of function, with the upper limb being the most affected. Factors leading to loss of arm function are poorly understood. A better understanding of movement profiles determined using 3D movement analysis could help inform treatment selection and development; however, limited evidence is available for combined 3D movement and surface electromyography (sEMG) studies that include the scapula. Our null hypothesis is that there are no differences between the movement and muscle activity

Scapulothoracic dissociation: level of vascular insult, an indirect prognostic indicator for the final outcome?

Scapulothoracic dissociation is the result of severe blunt trauma or sudden forceful traction applied to the shoulder, simulating a traumatic forequarter amputation but with sparing of the skin. This grievous injury results in injury at three levels: neural, vascular and musculoskeletal. Since the neural damage cannot be ascertained in the acute stage of this injury, wheras the vascular injury can be defined, this study aimed to find out any correlation of the eventual neural damage with the level of vascular injury. It became evident, after compilation of the authors' 8 cases and the 37 relev

Distal augmentation in unstable distal clavicle fractures: a retrospective cohort study of 101 cases.

INTRODUCTION: The optimal management strategy for unstable distal clavicular fractures remains controversial. Recent studies on plate techniques have reported good-to-excellent outcomes with no serious complications. The questions are that: (1) Does the use of wire augmentation with locking plate in distal part (distal wire augmentation) reduce radiographic loss of reduction (RLOR) and get earlier bony union in distal clavicular fractures? (2) Which fixation methods are associated with a higher incidence of acromioclavicular (AC) joints arthritis or subluxation? We collected and analyzed clini

MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

Observational case series on the clinical performance of the Variable Angle Clavicle Plate and Clavicle Hook Plate 2.7 systems: a study protocol.

BACKGROUND: Reoperation rates in clavicular fractures and acromioclavicular (AC) joint dislocations are high, often indicated for plate removal due to plate prominence and poor cosmesis. The recently designed 2.7 mm variable angle locking compression plate (VA-LCP®) Clavicle Plate and 2.7 mm VA-LCP® Clavicle Hook Plate (J&J MedTech) are intended to have an improved plate-to-bone fit, are precontoured, and have a low profile. Additionally, the Clavicle Hook Plate has a more anatomically adapted hook design to reduce the risk of painful osteolysis leading to earlier plate removals. All screw hol

Atypical presentation of carcinoid tumor with unresolved right shoulder pain: a case report.

INTRODUCTION: Carcinoid tumors are variants of neuroendocrine tumors that typically arise from the gastrointestinal tract and the bronchus, but they can involve any organ. Unresolved right shoulder pain manifesting as the first clinical presentation of carcinoid tumor with unknown primary origin is a rare clinical entity. To the best of our knowledge, herein we present the first case report describing metastasis to the right shoulder joint in a patient who presented with bone pain as the first clinical manifestation of metastatic carcinoid tumor of unknown primary origin. Metastasis to the rig

Shoulder injuries during alpine skiing.

We retrospectively reviewed alpine skiing injuries at a destination ski resort during three seasons to characterize the incidence and types of shoulder injuries. A total of 3451 injuries in 3247 patients were reviewed. The overall injury rate was 4.44 injuries per 1000 skier-days. Injuries to the upper extremity represented 29.1% (N = 1004) of all alpine ski injuries. Injuries involving the shoulder complex (393 injuries in 350 patients) accounted for 39.1% of upper extremity injuries and 11.4% of all alpine skiing injuries. The rate of shoulder injury was 0.51 injuries per 1000 skier-days. Pa

The influence of the acromial coverage index in rotator cuff tears.

Several intrinsic and extrinsic factors have been advocated in the pathogenesis of rotator cuff tears, but it is still unclear whether the origin of the tear is related to tendon degeneration itself or induced by several morphologic changes. The purpose of this study is to determine the relationship between the acromial coverage of the humeral head and the presence of a cuff tear. We evaluated 148 shoulders, including 45 that underwent surgical rotator cuff repair (group I), 26 with documented rotator cuff tears treated conservatively (group II), and 77 with no cuff pathology as a control grou

Release of growth factors after arthroscopic acromioplasty.

It has recently been postulated that a variety of growth factors may be released from cancellous bone after an acromioplasty. The aim of this study was to demonstrate the presence of growth factors in the subacromial space after acromioplasty. Between October 2006 and March 2007, 23 patients underwent arthroscopic acromioplasty. A sample of at least 3 ml of fluid from the shoulder was obtained 15 min after the end of the procedure. At the same time another sample of 3 ml of the patient's venous blood was obtained as a control. The concentrations of growth factors in the fluids collected were d

Clinical features of septic arthritis of sternoclavicular joint.

We studied 21 patients with septic arthritis of the sternoclavicular joint at Chulalongkorn University Hospital between January 1987 and January 1997. There were 15 males (71.4%) and 6 females (28.6%). The mean age was 47.4 years with a range of 16 to 69. More than half of the patients (57.1%) were aged more than 50 years and most had associated diseases including diabetes mellitus and cirrhosis. Almost all of the younger age group had a history of intravenous drug abuse. All of the patients had fever and sternoclavicular joint pain. Most of the patients (66.7%) had monoarticular arthritis, wh

[Early and intermediate results of conservatively and surgically treated lateral clavicular fractures].

Between 1986 and 1988 67 patients with distal clavicular fracture were treated at the trauma hospital in Innsbruck. Depending on type of fracture conservative treatment (using a spica or other bandages) was performed in those cases without dislocation. Operation was necessary if coracoclavicular ligament was ruptured and dislocation of the distal clavicular end was found in X-rays. The most common practice was the use of K-wires and wire-loops. Plate fixation was necessary in one case of delayed fracture healing. No pseudarthrosis was seen in X-rays, mobility of the shoulder girdle was good an

Contact pressure of the coracoacromial arch in shoulders with joint contracture: A cadaveric study.

BACKGROUNDS: There have been many reports describing that the capsular fibrosis of the shoulder joint is the main cause of frozen shoulder, whereas others reported the significance of subacromial impingement as an etiological factor. The purpose of this study was to investigate the contact pressure between the coracoacromial arch and the rotator cuff tendons to clarify the contact phenomenon in shoulders with joint contracture. METHODS: Fourteen fresh-frozen cadaveric shoulders were used. Specimens were divided into two groups: normal group (8 shoulders, definition: more than 61° of flexion a

The use of hook plate in type III and V acromio-clavicular Rockwood dislocations: clinical and radiological midterm results and MRI evaluation in 42 patients.

BACKGROUND: Acromio-clavicular (AC) joint dislocations are very common following falls on the shoulder or an overstretched hand. The best treatment for such lesions remains a matter of debate. Several studies have, however, lent support to the surgical role of the hook plate in Rockwood type III and V AC dislocations. The aim of this study was to evaluate the midterm clinical results and magnetic resonance imaging (MRI) features of coraco-clavicular ligaments 18 months after an AC dislocation treated with an AC Dreithaler hook plate. PATIENTS AND METHODS: The cohort was made up of a consecuti

First case of intrafamily transmission of a new MRSA clone with toxic shock syndrome toxin-1.

We report the first case of intrafamily transmission of a C-MRSA clone harbouring toxic shock syndrome toxin-1 (TSST-1). Because of the risk of this clone to spread in the community, family members of these patients should be screened to detect and prevent the diffusion of recurrent or new infections.

Cross-country skiing injuries and biomechanics.

Cross-country skiing exercises most of the joints, muscles and tendons in the body giving the skier an all around workout. This, in combination with a low incidence of injury, makes cross-country skiing an ideal recreational and competitive sport. The new skating techniques developed during the last decade have resulted in greater velocity. The maximum speed during the diagonal stride technique is 6 m/sec compared to 8 to 9 m/sec when skating and double poling. Top-level skiers today use strong and ultra light skis of fiberglass and graphite. The ski weight is less than 500g. Today's skating t

Implementing acromioclavicular and coracoclavicular ligaments with a novel sternoclavicular posterior rotation estimation approach improves agreement between simulated muscle activation and surface electromyography in shoulder musculoskeletal simulation.

Acromioclavicular (AC) and coracoclavicular (CC) ligaments stabilize the shoulder, but they are often omitted from musculoskeletal (MSK) simulations. Because ligament force depends on clavicle-scapula kinematics, sternoclavicular posterior rotation (SCpr) estimation may affect simulated muscle activation when AC-CC ligaments are implemented. This study aimed to determine how AC-CC ligament implementation and SCpr estimation affect the agreement between simulated muscle activation and surface electromyography (sEMG) during arm elevation-lowering. Eight healthy males performed arm elevation-lowe

[Right median nerve injury following laparoscopic sigmoidectomy].

Brachial plexus injury has often occurred secondary to malposition of the patient during general anesthesia. We have experienced median nerve injury following laparoscopic sigmoidectomy. A 61-year-old man with diabetes and hypertension received laparoscopic sigmoidectomy under general and epidural anesthesia. Tracheal intubation was easy without excessive retroflextion of his neck after anesthesia induction with fentanyl, propofol, and vecuronium. Both his upper arms were abducted to 80 degrees and his elbows were extended on a padded arm board. The shoulder braces were placed over both his ac

Three-dimensional scapulothoracic motion following treatment for breast cancer.

Varying levels of shoulder morbidity following treatment for breast cancer have been reported. Patients report pain, weakness, tightness and reduced functional capacity. Normal painfree motion of the arm and shoulder requires mobility in the scapulothoracic, glenohumeral, acromioclavicular and sternoclavicular joints. Under healthy conditions elevation of the arm is accompanied by scapula retraction, lateral rotation and posterior tilt. However, when scapulothoracic motion is disproportionate to glenohumeral motion, the potential exists for microtrauma and long term pain. A number of studies o
