Measuring Patient Satisfaction With Advanced Practice Clinicians: Identifying a Critical Gap in Palliative Care Measurement Instruments.

ContextMedical advancements have led to patients living longer, often with serious illness(es), making palliative care involvement prudent. To address the deficit of specialty palliative care clinicians, the integration of advanced practice clinicians (APCs) alongside physicians is a critical workforce shortage solution. However, a gap exists in the standardized methods for evaluation of patient satisfaction with APCs in this setting.ObjectivesThis paper evaluates the existing instruments measuring patient satisfaction with APCs to highlight the current gap in palliative care-specific tools an

Staff experiences of percutaneous tracheostomy in intensive care: challenges, complications and potential solutions.

Tracheostomies are performed in 10-13% of UK intensive care admissions. While the percutaneous technique is well established, accurate needle insertion can be difficult in patients with obesity or neck swelling. These challenges increase the risk of bleeding, airway loss and injury to neck structures. In anatomically complex cases, patients are referred for surgical tracheostomy, causing delays and additional healthcare costs. Semi-structured interviews and focus groups were conducted with 32 staff across three intensive care units. Participants included consultants, resident doctors, nurses,

Integrating X-waiver training for opioid use disorder into the nurse practitioner and physician assistant curricula.

Starting in 2016, physician assistants (PAs) and Advanced Practice Registered Nurses could prescribe buprenorphine with an approved waiver, which improves health care through expansion of access to care, reduction of health care costs, and provision of high-quality care. This study addresses barriers to student NP's and PA's ability to prescribe buprenorphine by examining knowledge and attitudes toward prescribing medications for opioid use disorder (MOUD), subjective norms and perceived behavioral control surrounding prescribing MOUD, and the intention to prescribe MOUD among NPs and PAs. Par

Device Therapy and Arrhythmia Management in Left Ventricular Assist Device Recipients: A Scientific Statement From the American Heart Association.

Left ventricular assist devices (LVADs) are an increasingly used strategy for the management of patients with advanced heart failure with reduced ejection fraction. Although these devices effectively improve survival, atrial and ventricular arrhythmias are common, predispose these patients to additional risk, and complicate patient management. However, there is no consensus on best practices for the medical management of these arrhythmias or on the optimal timing for procedural interventions in patients with refractory arrhythmias. Although the vast majority of these patients have preexisting

Promoting health equity through health literacy interventions in educational institutions: a scoping review.

BACKGROUND: Disparities in health outcomes have been associated with inequitable access to health literacy and services. Additionally, reduced health literacy may lead to an unhealthy lifestyle, causing a decrease in quality of life and social inequities. Advancing health literacy is vital in educational organizations to curb gaps in social inequities. Adapting strategies to promote health literacy is essential for advancing equitable and effective educational organizations. Therefore, policies and interventions should aim to address any barriers that threaten inequities in the educational org

Protocol for the process evaluation of a randomised clinical trial of incremental-start versus conventional haemodialysis: the TwoPlus study.

INTRODUCTION: Process evaluation provides insight into how interventions are delivered across varying contexts and why interventions work in some contexts and not in others. This manuscript outlines the protocol for a process evaluation embedded in a hybrid type 1 effectiveness-implementation randomised clinical trial of incremental-start haemodialysis (HD) versus conventional HD delivered to patients starting chronic dialysis (the TwoPlus Study). The trial will simultaneously assess the effectiveness of incremental-start HD in real-world settings and the implementation strategies needed to su

Development of pediatric multidisciplinary extracorporeal membrane oxygenation simulations: A novel educational program to enhance team communication and emergency preparedness.

INTRODUCTION: Pediatric extracorporeal membrane oxygenation (ECMO) is a high risk, low-volume technology. Infrequency of this technology and associated complications may translate to unfamiliarity of identification and management of potentially life-threatening events, which may require knowledge and procedural skills to be performed quickly. Providers involved in managing ECMO must be able to promptly identify and initiate management for such events, particularly when surgical colleagues are not readily available. METHODS: A multidisciplinary ECMO simulation program was implemented in a tert

How Advanced Practice Clinicians Support Education for and Contribute to Education of Graduate Medical Education Trainees: A Critical Literature Review.

PURPOSE: With the introduction of physician assistants and nurse practitioners (i.e., advanced practice clinicians [APCs]), the landscape of graduate medical education (GME) has fundamentally changed. Whereas APCs' role in GME settings has been mainly described as substitutes for postgraduate medical trainees, APCs are increasingly considered integrated and collaborative team members. However, APCs' contributions to trainees' education and learning remain underexplored. This critical review synthesized the literature available on how APCs contribute to trainees' workplace learning and how thes
