Opiate detoxification under general anesthesia by large doses of naloxone.

For opiate detoxification 6 volunteer opiate addicts were intravenously administered 10 mg naloxone within one hour while under barbiturate anesthesia. During administration of naloxone none of the patients demonstrated significant changes in the hemodynamic parameters of heart rate, mean arterial pressure, cardiac index, peripheral resistance or in the oxygen saturation. After patients awoke from anesthesia, they experienced no or only minimal withdrawal symptoms. Possible explanations for the suppression of withdrawal symptoms are discussed.

Anaesthetic management in a neonate with a double-outlet right ventricle undergoing repair of leaking myelomeningocele.

Neonates comprise a special population of patients undergoing surgery, and the presence of congenital defects further complicates their anaesthetic management. This report describes a neonate born with Arnold-Chiari malformation and a ruptured myelomeningocele, who was later found to also have a double-outlet right ventricle. This report outlines the careful selection of anaesthetic agents and perioperative strategies tailored to the patient's compounded neurologic and cardiac congenital defects while catering to the surgical requirements. The patient underwent repair of the MMC under total in

Effect of intravenous esketamine in emergency cesarean deliveries: a retrospective analysis of maternal and neonatal outcomes.

BACKGROUND: This study investigates the effects of administering intravenous esketamine at a dose of 0.25 mg/kg to pregnant patients receiving epidural anesthesia for emergency cesarean section on both maternal and neonatal outcomes. METHODS: Medical records of pregnant patients transitioning from labor analgesia to epidural anesthesia for emergency cesarean sections between January 2020 and December 2022 were analyzed. The patients were categorized based on whether they received esketamine infusions during the incision-to-delivery interval. The variables compared between the groups included

General anaesthesia for external electrical cardioversion of atrial fibrillation: experience of an exclusively cardiological procedural management.

AIMS: External electrical cardioversion (EC) usually requires brief general anaesthesia involving anaesthetists. The aim of this study was to evaluate the feasibility and safety of inducing anaesthesia for EC of atrial fibrillation (AF) exclusively by the cardiologic team with anaesthetists on-hand. METHODS AND RESULTS: A retrospective analysis of 624 elective EC, over a 6-year period, was made. No patients were excluded due to the severity of pathology or comorbidities. The protocol of the intravenous anaesthesia was 5 mg bolus of midazolam and subsequent increasing doses of propofol startin

PULMONARY ASPIRATION OF GASTRIC ACID--MENDELSON'S SYNDROME.

Three cases of pulmonary aspiration of gastric acid as a complication of obstetrical anesthesia are described. The clinical picture consists of dyspnea, cyanosis, tachycardia and shock appearing several hours after the aspiration has occurred. On examination, the chest may be quite clear, but the chest radiograph shows a picture indistinguishable from that of pulmonary edema. The most important therapeutic measure is the intravenous administration of corticosteroids in large doses for several days. Bronchoscopy is contraindicated. With routine use of epidural anesthesia, this obstetrical compl

[Cesarean Section Under Combined Spinal and Epidural Anesthesia for a Pregnant Woman with Primary Biliary Cirrhosis].

We report the successful anesthetic management of a pregnant woman with primary biliary cirrhosis (PBC) and autoimmune hepatitis who underwent cesarean section. A 35 year-old pregnant woman with PBC was diagnosed with preterm rupture of membranes (PROM). Emergent cesarean section was scheduled. AST and ALT were elevated and she complained of itching due to PBC. Hydrocortisone 50 mg was intravenously administered. Spinal anesthesia was initiated with 2.4 ml of 0.5% bupivacaine hydrochloride and 10 µg fentanyl at L3-4; sensory loss (T2) was confirmed. Morphine was not included in spinal anesthes

Pulmonary oedema in two parturients with hypertrophic obstructive cardiomyopathy (HOCM).

Two patients with hypertrophic obstructive cardiomyopathy (HOCM) presented for delivery. The first had a repeat Caesarean section with general anaesthesia and the second gave birth vaginally with epidural analgesia. Both patients developed pulmonary oedema in the peripartum period. These cases highlight the delicate fluid requirements of the pregnant patient with HOCM. The fluid management of the parturient is discussed with particular emphasis on the pathophysiology of HOCM. The indications for invasive monitoring are presented.

[Role of nitric oxide (NO) in the mechanism of anesthetic action].

Published reports on modifications of nitric oxide (NO) synthase (NOS) activity and NO production in the brain during development of anesthesia induced by the most common inhalational (halothane, isoflurane, sevoflurane, enflurane) and intravenous (ketamine, barbiturates, propofol, etomidate) anesthetics are reviewed. According to a popular universally acknowledged hypothesis, inhibition of NOS activity and blockade of NO neurotransmitter function are important steps in the mechanism of action of anesthetics. There are data which confirm the validity of this hypothesis for all above-listed dru

A prospective evaluation of anesthesia for posterior lumbar spine fusion: the effectiveness of preoperative epidural anesthesia with morphine.

STUDY DESIGN: A prospective, randomized, single-blinded study. OBJECTIVES: To prospectively compare the anesthesia for posterior lumbar spinal fusion between the age- and gender-matched groups with and without preoperative epidural anesthesia with morphine. SUMMARY OF BACKGROUND DATA: To reduce the blood loss, hypotensive anesthesia is frequently used in the spinal fusion. However, in spite of administration of the hypotensive agents, inadequate analgesia during operation often causes unfavorable and unexpected elevation of blood pressure resulting in excessive bleeding. As well, intractable

[Anesthetic Management of a Patient with Double Outlet Right Ventricle and Situs Inversus for Dilatation and Curettage of the Uterus].

A 30-year-old woman with a double outlet right ven- tricle and situs inversus was scheduled for dilatation and curettage of the uterus. We selected intravenous anesthesia which allowed spontaneous respiration, because of the short operation time and the low invasive surgery. We administered dexmedetomidine hydrochloride 1.1 μg · kg⁻¹ · hr⁻¹ and fentanyl (total amount 150 μg, three doses of 50 μg each) during operation. We accomplished anesthesia without appar- ent hemodynamic change or respiratory depression. We should select anesthetic method and anesthetic cautiously, considering patient's s

Combined use of infraorbital and external nasal nerve blocks for effective perioperative pain control during and after cleft lip repair.

Perioperative analgesia in patients undergoing cleft lip and palate repair is complicated by the risk of postoperative airway obstruction. We describe a technique of combined infraorbital and external nasal nerve blocks to reduce the need for opioid analgesia. Using this technique, we have successfully performed cleft lip repair under local anesthesia alone, without general anesthesia or intravenous sedation, in adolescents and adults. In children, this technique can reduce the need for postoperative opioids. We describe this novel analgesic approach to decrease opioid requirements and minimiz

[Pulmonary edema due to upper airway obstruction after extubation].

We report a patient who developed pulmonary edema due to airway obstruction after extubation. A 22-year-old man underwent removal of the nails for thoracoplasty under general anesthesia combined with epidural anesthesia. Upper airway obstruction occurred after extubation. SpO2 decreased to 70%. Insertion of an oral airway relieved the airway obstruction. However, inspiratory wheezing was heard over both lung fields. Chest X-ray taken 90 minutes after the event revealed pulmonary edema. Pulmonary edema gradually resolved after intravenous furosemide and oxygen inhalation by mask.

Diazepam and local anesthesia in anorectal surgery.

At the Ferguson Clinic, 161 anorectal procedures utilizing intravenously administered diazepam and lidocaine 0.5%, with epinephrine, 1:200,000, were done. Of the patients so treated 12% needed additional medication in order for anesthesia to be effective. No complication resulted from the use of these combinations. It is recommended that the combination of intravenously administered diazepam with local infection of lidocaine and epinephrine be given wider usage because of its ease of handling and very low complication rate.

The role of magnesium as an adjuvant during general anaesthesia.

Magnesium sulphate is used extensively in the treatment of eclampsia, and is also used to treat refractory arrhythmias, asthma, myocardial ischaemia and acute respiratory failure. We studied the interaction between magnesium sulphate and the anaesthetic agents propofol, rocuronium bromide and fentanyl citrate. This randomised, double blind study was conducted in 50 patients. The magnesium group A (n = 25) received 30 mg x kg(-1) magnesium sulphate before induction of anaesthesia and 10 mg x kg(-1) continuously intra-operatively until the end of surgery. Group B (n = 25) received the same volum

Anesthetic management of a dog undergoing unilateral adrenalectomy for phaeochromocytoma excision using a partial intravenous anesthetic protocol.

BACKGROUND: The anesthetic management of adrenalectomies for phaeochromocytoma excision, a catecholamine-secreting tumor, is challenging due to the potential for fatal complications following severe hemodynamic variations, including hypertensive crisis following tumor manipulation or sympathetic stimulation, but also severe hypotension and volume depletion post resection. CASE DESCRIPTION: An 11 kg, 15-year-old male neutered Jack Russel Terrier, with mitral valve disease stage B2, was referred for adrenalectomy for phaeochromocytoma resection. The patient was administered per os prazosin 0.11
