Anesthesia in multiple sclerosis.

The effect of general anesthesia on 42 multiple sclerosis (MS) patients who underwent 88 episodes of general anesthesia was analyzed. One patient experienced a relapse after a procedure under general anesthesia, which is compatible with the natural history of the disease. A literature review revealed little information on this subject or on the use of particular anesthetic agents in MS. Our experience with spinal and local anesthesia is reported. In the evaluation of the former our limited data suggested that spinal anesthesia is less preferable than other alternatives in MS. Local anesthetics

Paracentral Acute Middle Maculopathy After Sinus Surgery: A Case Report and Literature Review.

Purpose: Herein, we describe a case of paracentral acute middle maculopathy occurring after routine sinus surgery. Methods: A single case was evaluated. Results: The patient presented with an inferocentral scotoma in the left eye 2 days after an uneventful sinuplasty procedure performed under local anesthesia. Examination and optical coherence tomography demonstrated paracentral acute middle maculopathy. A complete workup and urgent stroke evaluation revealed no clear underlying etiology. Conclusion: Sinus procedures may rarely be associated with paracentral acute middle maculopathy, possibly

Perioperative pain control: a strategy for management.

A thorough understanding of the anatomy and neurophysiology of the pain response is necessary for the effective treatment of perioperative pain. This article describes the mechanisms that produce pain, including those related to inflammation. Other topics include the pharmacologies of nonopioid and opioid analgesics. Nonopioid analgesics can be separated into two categories: nonsteroidal anti-inflammatory drugs, such as salicylates, and acetaminophen. Opioids include morphine, fentanyl, and meperidine. The pharmacology of local anesthesia is discussed. The six major adverse reactions to local a

[Infiltration with ropivacaine improves postoperative analgesia following caesarean section].

The analgesia produced by parietal infiltrations with local anesthetics (ALs) results from a block of conduction and the anti-inflammatory effect of the ALs. The duration of the reported analgesia is variable : limited to the duration of action of the ALs, or sometimes very prolonged. The purpose of this randomized, double-blind, study is to investigate the duration of the analgesia of a parietal infiltration after caesarian.

Drugs of choice for sedation and analgesia in the neonatal ICU.

Painful procedures in the neonatal ICU are common, undertreated, and lead to adverse consequences. The drugs most commonly used to treat neonatal pain include the opiates, benzodiazepines, barbiturates, ketamine, propofol, acetaminophen, and local and topical anesthetics. This article discusses the indications for and advantages and disadvantages of the commonly used analgesic drugs. Guidance and references for drugs and dosing for specific neonatal procedures are provided.

Transient abducens nerve palsy during endoscopic sinus surgery: report of three cases.

Dysfunction of one or more of the ocular motor nerves or extraocular muscle injury commonly results in diplopia. Diplopia during endoscopic sinus surgery is caused by surgical trauma or local anesthetics. Transient abducens nerve palsy rarely occurs after an intranasal sphenopalatine nerve block. We report three cases of transient abducens nerve palsy during endoscopic sinus surgery and possible explanations are proposed with a review of literature.

Single Injection of Twin Nerve Block in Longstanding Unilateral Temporomandibular Joint Dislocation: A Novel Technique.

Unilateral temporomandibular joint (TMJ) dislocation is a rare condition often triggered by yawning, vomiting, or other activities involving wide opening of the mouth, resulting from the forward movement of the condylar head. This case report highlights a longstanding unilateral TMJ dislocation managed by the single injection of local anesthetics into the deep temporal and masseteric nerve peripheries, alleviating muscle spasm and pain to restore TMJ function.

Brugada-type electrocardiographic pattern induced by epidural bupivacaine.

In this case report, we describe the postoperative occurrence of electrocardiogram changes suggestive for the Brugada syndrome in a patient receiving a continuous epidural bupivacaine infusion. After withdrawal of bupivacaine, the electrocardiogram changes were reversible. The patient's history was unremarkable except for an incomplete right bundle branch block. We conclude that local anesthetics, particularly bupivacaine, have the potential to induce serious arrhythmias in patients with Brugada syndrome.

[Interaction of calcium and some neuropharmacological agents on the isolated nerve fiber].

It has been suggested that calcium interferes with the effect of the local anesthetics (Blaustein and Goldman, 1966). Experiments were therefore made with the anionic phenobarbital and the cationic procaine and chlorpromazine in high (18 mM/1) and in low (0,18 mM/1) calcium concentration. These experiments showed that the effect of drugs used, and of changes in calcium concentration were independent of each other.

Updates on Wound Infiltration Use for Postoperative Pain Management: A Narrative Review.

Local anesthetic wound infiltration (WI) provides anesthesia for minor surgical procedures and improves postoperative analgesia as part of multimodal analgesia after general or regional anesthesia. Although pre-incisional block is preferable, in practice WI is usually done at the end of surgery. WI performed as a continuous modality reduces analgesics, prolongs the duration of analgesia, and enhances the patient's mobilization in some cases. WI benefits are documented in open abdominal surgeries (Caesarean section, colorectal surgery, abdominal hysterectomy, herniorrhaphy), laparoscopic cholec

Initial injection pressure for dental local anesthesia: effects on pain and anxiety.

This study quantitatively assessed injection pressure, pain, and anxiety at the start of injection of a local anesthetic into the oral mucosa, and confirmed the relationship between injection pressure and pain, as well as between injection pressure and anxiety. Twenty-eight healthy men were selected as subjects and a 0.5-inch (12 mm) 30-gauge disposable needle attached to a computer-controlled local anesthetic delivery system (the Wand) was used. A 0.5 mL volume of local anesthetic solution was injected submucosally at a speed of either 30 or 160 s/mL. Three seconds after the start of local an

Horner's syndrome following very low concentration bupivacaine infusion for labor epidural analgesia.

Horner's syndrome is a complication of epidural analgesia and anesthesia, encountered more commonly in pregnant women than in other patients. Previous reports described the appearance of Horner's syndrome following epidural injection of concentrated local anesthetic solutions. We report unilateral Horner's syndrome occurring in the setting of lumbar epidural analgesia for labor with a very low local anesthetic concentration (bupivacaine 0.04%) in an epidural infusion. We discuss the possible factors that could have contributed to this occurrence despite the extremely dilute concentration of lo

Propofol binding to the resting state of the gloeobacter violaceus ligand-gated ion channel (GLIC) induces structural changes in the inter- and intrasubunit transmembrane domain (TMD) cavities.

General anesthetics exert many of their CNS actions by binding to and modulating membrane-embedded pentameric ligand-gated ion channels (pLGICs). The structural mechanisms underlying how anesthetics modulate pLGIC function remain largely unknown. GLIC, a prokaryotic pLGIC homologue, is inhibited by general anesthetics, suggesting anesthetics stabilize a closed channel state, but in anesthetic-bound GLIC crystal structures the channel appears open. Here, using functional GLIC channels expressed in oocytes, we examined whether propofol induces structural rearrangements in the GLIC transmembrane

Comparison of Surgical Wait Times and Procedure Length in the Management of Postaxial Polydactyly Using Local or General Anesthesia.

Introduction: For infants with ulnar polydactyly, surgical removal of the supernumerary digit can be performed under general or local anesthetic. This study evaluated the wait times, surgical duration, and sedation times associated with performing the procedure under local versus general anesthetic in infants with ulnar polydactyly. Methods: The databases of three surgeons at our institution were reviewed for children less than 2 years of age who underwent surgery for non-syndromic ulnar polydactyly. Data collection included patient demographics, wait times, duration of surgery and sedation an
