The lethal factor (LF) of anthrax toxin is the toxic component of the exotoxin (lethal toxin) secreted by toxic strains of Bacillus anthracis. Current events have heightened the importance of understanding the risks from inhalation exposure to small numbers of spores of Bacillus anthracis. Previously reported data sets have not been fully assessed using current understanding of microbial dose response. This article presents an assessment of the reported primate dose-response data. At low doses, the risk to large populations of low doses of inhaled spores (e.g., < 100) is not insignificant.

Anthrax, caused by the Gram-positive, rod-shaped, spore-forming bacterium Bacillus anthracis, is rarely seen in industrialized nations but is common in developing countries. Cutaneous anthrax accounts for 95% of cases and usually develops on exposed sites. Although human anthrax has become rare, endemic outbreaks still occur in tropical countries, parts of South America and Europe. We report 23 cases of cutaneous anthrax due to an endemic outbreak of animal and human anthrax in South India. These patients were admitted to our hospital between July 1998 and July 2001. Children outnumbered adults and most of them had lesions on the exposed sites. A 54-year-old female farmer with anthrax infection of the eyelids is presented. She was initially managed with high dose intravenous penicillin G treatment. Following complete healing of the eyelid lesions, significant cicatricial ectropion resulted. Her right lower eyelid ectropion was corrected by surgical reconstruction using full thickness skin graft after a period of 6 months during which the cicatrization process stabilized.
