Emphysematous cystitis-gas in bladder: a rare urological emergency.

Emphysematous cystitis is an unusual inflammatory condition characterised by the presence of air within the bladder wall and lumen. It is usually seen in patients with diabetes, and in those with long-term catheterisation or of immunocompromised status. Its presentation varies from asymptomatic to frank urosepsis. It is diagnosed after radiological studies. Treatment requires establishment of prompt urinary drainage, antibiotic coverage and correction of offending factors, if any. We report a case of 41-year-old, non-diabetic, otherwise healthy woman presenting with lower urinary tract symptom

Purple urinary bag syndrome: a harmless but alarming problem.

Patients who are not catheterized may notice a discolouration in their urine or faeces, either occasionally or continually. Such discolouration can usually be put down to waste materials caused by medication or food. However, a comment often made by nurses who see catheterized patients is, 'My patient's drainage bag has turned purple'. Patients with this condition can show no underlying urine infection. It is also noticeable that the longer the drainage bag is used, the deeper the colour purple becomes. Over a period of time this discolouration then impregnates their catheter. This condition a

Antimicrobial use and stewardship activities in Swiss long-term care facilities: data from a national point-prevalence survey.

BACKGROUND: The first nation-wide point-prevalence survey (PPS) in Swiss long-term care facilities (LTCF) showed significant geographical differences in antimicrobial use. The aim of this study was to characterize these variations, identify associated factors, and assess antimicrobial stewardship (AMS) practices. METHODS: The PPS was performed in September 2024. A sample of Swiss LTCFs was randomly selected for representativeness according to language region and size. The PPS was also open to all interested Swiss LTCFs. Data were collected using the adapted Healthcare-Associated Infections in

Doripenem in hospital infections: a focus on nosocomial pneumonia, complicated intra-abdominal infections, and complicated urinary tract infections.

Doripenem is the latest carbapenem on the market to date. Although not an antibiotic in a new class, it offers a glimmer of hope in combating serious infections secondary to multidrug-resistant Gram-negative bacteria when we have not seen a new class of antibacterial, particularly for Gram-negative bacteria, for more than 10 years. In vitro, doripenem exhibits a broad spectrum of activity against Gram-positive and Gram-negative bacteria, including extended-spectrum β-lactamase (ESBL) and Amp-C β-lactamase producing Enterobacteriaceae and anaerobes. Doripenem also exhibits better in vitro activ

Bacteruria and Urinary Tract Infections in the Elderly.

Both urinary tract infection (UTI) and asymptomatic bacteriuria (ASB) are common problems among elderly adults and represent a significant health care burden. Despite their frequency, differentiating between ASB and true UTI remains controversial among health care providers. Several challenges exist in the evaluation of urinary symptoms in the elderly patient. Symptoms of UTI are variable; problems are encountered in the collection, testing, and interpretation of urine specimens; and results of urinalysis are often misinterpreted and mishandled. Multiple studies have shown no morbidity or mort

Effect of silver-coated urinary catheters: efficacy, cost-effectiveness, and antimicrobial resistance.

BACKGROUND: Urinary tract infections (UTIs) are the most common nosocomial infection experienced by patients in United States hospitals and are responsible for significant morbidity and excess hospital costs. The purpose of this study was to determine the efficacy of a silver alloy, hydrogel-coated, urinary catheter in the prevention of catheter-associated UTI, to assess the cost effectiveness of the coated catheter, and to test for the emergence of silver-resistance in urinary microbial isolates. METHODS: A 2-year prospective surveillance study in 10 patient care units was conducted to deter

Meningoencephalitis Caused by Escherichia coli in an Adult With No Known Immunocompromising Conditions: A Case Report.

BACKGROUND E. coli is one of the most common causative agents of neonatal meningitis. However, it rarely affects the central nervous system (CNS) in immunocompetent adults or adults with no known immunocompromising conditions, with less than 1 case per year. Gram-negative community-acquired bacillary meningitis seldom occurs without pre-existing conditions such as trauma, organ dysfunction, and immunocompromised state, and there have been very few case reports with E. coli. CASE REPORT In this report, we present a case of a 69-year-old man who was proven to have meningoencephalitis caused by E

An overview of reactive arthritis.

Reactive arthritis, also known as Reiter syndrome, is a spondyloarthropathy that typically follows a urogenital or gastrointestinal infection, and is characterized by conjunctivitis, urethritis, and arthritis. The frequency of reactive arthritis in the United States is estimated at 3.5 to 5 patients per 100,000. Physician assistants (PAs) can manage the condition; therefore, they should be familiar with the disease's signs and symptoms, diagnostic criteria, and treatment regimens. Without proper management, reactive arthritis can progress to a chronic destructive arthritis. Prompt recognition

Asymmetric Cu─N─Ru Bridgedsite Nanozyme-Loaded Injectable Thermogel Boosts Cuproptosis-Like Death for Multidrug-Resistant Urinary Tract Infections.

Multidrug-resistant urinary tract infections (MDR-UTIs) remain one of the most challenging clinical infections globally owing to the paucity of effective therapeutic agents. Nanozymes-triggered cuproptosis-like bacterial death has emerged as a promising bactericidal strategy. However, its efficacy is compromised by insufficient catalytic sites and suboptimal synergy among active centers. Consequently, the development of nanozymes capable of efficiently inducing cuproptosis-like death holds considerable potential for treating MDR-UTIs. Herein, the Cu-ZIF8-Ru nanozyme was engineered that leverag

Oral β-Lactams for Complicated Urinary Tract Infections: A Systematic Review and Point-Counterpoint Comparison With Trimethoprim/Sulfamethoxazole and Fluoroquinolones.

BACKGROUND: Rising resistance and toxicity concerns with fluoroquinolones (FQs) and trimethoprim/sulfamethoxazole (TMP/SMX) have prompted interest in oral β-lactams as alternatives for complicated urinary tract infections (cUTIs). This review evaluates the effectiveness, safety, and pharmacokinetic (PK) considerations of oral β-lactams compared to FQs and TMP/SMX in these clinical contexts. A structured point-counterpoint discussion explores their role in antimicrobial stewardship and patient-centered therapy. METHODS: A systematic PubMed search identified studies published between January 20

[Urinary tract infections : new 2026 guidelines].

Urinary tract infections are a major source of antibiotic prescriptions. The 2025 guidelines from the European Association of Urology and the Infectious Diseases Society of America introduce a new classification based on clinical presentation and the presence of systemic signs, rather than on patient groups considered at increased risk. This shift aims to better target diagnostic investigations, reduce overtreatment of localized infections, and enable early identification of potentially severe cases. In the context of rising antimicrobial resistance, antibiotic-sparing strategies remain centra

The role of pharmacological interventions in managing urological complications during pregnancy and childbirth: A review.

Pregnancy leads to a number of structural and functional changes in the urinary system, which makes females susceptible to urological complications. This review aims to discuss the epidemiology, complications and prevention and management of urinary tract infections (UTIs), kidney stones and bladder dysfunction in pregnancy. UTIs are the most common urological problem presenting in 10% of pregnant women; Escherichia coli is the most common causative organism. If left untreated, UTIs lead to acute pyelonephritis which occurs in about 2% of pregnant women and which has serious consequences for b

Cefazolin as a predictor of urinary cephalosporin activity in indicated Enterobacterales.

Traditionally, cephalothin susceptibility results were used to predict the susceptibility of additional cephalosporins; however, in 2013-2014, the Clinical and Laboratory Standards Institute (CLSI) revisited this practice and determined that cefazolin is a more accurate proxy than cephalothin for uncomplicated urinary tract infections (uUTIs). Therefore, a cefazolin surrogacy breakpoint was established to predict the susceptibility of seven oral cephalosporins for Escherichia coli, Klebsiella pneumoniae, and Proteus mirabilis in the context of uUTIs. Clinical microbiology laboratories face sev

Acute Uncomplicated UTIs in Adults: Rapid Evidence Review.

An acute uncomplicated urinary tract infection (UTI) is a bacterial infection of the lower urinary tract with no sign of systemic illness or pyelonephritis in a noncatheterized, nonpregnant adult with no urologic abnormalities or immunocompromise. In women, a self-diagnosis of a UTI with the presence of typical symptoms (e.g., frequency, urgency, dysuria/burning sensation, nocturia, suprapubic pain), without vaginal discharge, is accurate enough to diagnose an uncomplicated UTI without further testing. Urine culture and susceptibility testing should be reserved for women with recurrent infecti
